New trends in cranio-orbital and midface distraction for craniofacial dysostosis.

PURPOSE OF REVIEW: Syndromic craniofacial dysostosis remains one of the most challenging diagnoses in congenital craniomaxillofacial surgery. Distraction osteogenesis has held the promise of greater facial movements with improved stability, less morbidity and more favorable functional and esthetic outcomes. Current literature is starting to determine whether this promise is being fulfilled. RECENT FINDINGS: The current trend in the literature is increasing indications for distraction in treating syndromic synostosis. Posterior vault, anterior vault, early monobloc and segmental Lefort distrac

Subcaruncular approach for the reconstruction of blowout fractures of medial orbital walls.

To obtain a wide and clean operative field for anatomic reconstruction of medial orbital blowout fractures, an alternative method called the subcaruncular approach has been performed. Between March 2008 and June 2010, this method was applied to the orbits of 41 patients with isolated pure medial orbital blowout fractures. A medial half conjunctival incision was made and extended meticulously to the subcaruncular area through preseptal plane under direct vision. Pre- and postoperative computed tomographic scans and ophthalmic examinations were performed, and clinical results were assessed. Post…

Optimizing the timing and technique of Treacher Collins orbital malar reconstruction.

The optimal timing and treatment of Treacher Collins syndrome with regard to zygomatico-orbital osseous reconstruction has not been fully established. Osseous reconstruction performed at an early age may result in bone graft resorption; however, delays in surgical improvement may result in adverse psychosocial effects on the patient. To study the optimal age for reconstruction clinically, we examined 3 age groups based on timing of malar and eyelid reconstruction using a three-dimensional computed tomographic scan and three-dimensional photometric volume assessment. In addition, we collected o…

Recurrent Orbital Cavernous Hemangioma due to Overlooked Multiple Tumors.

PURPOSE: To report late recurrence of orbital cavernous hemangioma in a patient ten years after complete resection of the primary tumor. CASE REPORT: A 32-year-old woman with a history of progressive visual loss and proptosis underwent lateral orbitotomy for resection of a large cavernous hemangioma. Ten years later, proptosis recurred and the patient developed progressive ocular deviation. Imaging studies were in favor of a recurrent cavernous hemangioma and the tumor was excised via the previous incision site. Reassessment of previous orbital images suggested the presence of two separate tu…

Compression and stretching in Graves orbitopathy: emergency orbital decompression techniques.

Visual loss from orbital compression and stretching of the optic nerve is an infrequent but well-recognized cause of dysthyroid optic neuropathy, occurring in less than 5% of thyroid eye disease cases. Another important but less recognized cause of vision loss due to thyroid-associated optic neuropathy is protracted stretching of the optic nerve. Of the reported cases of compression and stretch optic neuropathy, none have documented the rate of vision loss due to continuous stretching of the optic nerve.

Unusual case of orbital apex syndrome after chalazion excision.

Orbital apex syndrome represents a heterogeneous group of disorders. It involves damage to the third, fourth and sixth nerves and the ophthalmic division of the fifth cranial nerve, and results in optic neuropathy. An 80-year-old man presented to our emergency clinic with left acute visual loss and three-day history of ocular pain. His medical and ophthalmic history involved diagnosis of lung cancer and chalazion excision two weeks prior to presentation. There was total left ophthalmoplegia, vision was 'no light perception' and he had a relative afferent pupillary defect. An urgent computed to…

[Endoscopic treatment of orbital cellulitis in pediatric patients: transethmoidal approach].

Orbital cellulitis is a septic process of the soft tissues behind the orbital septum and is the most frequent cause of the monolateral exophthalmos in pediatrics. Approximately 90% of the orbital cellulitis in pediatrics are associated to acute ethmoiditis. From the 01.01.2001 to 31.12.2010 we treated 36 patients, less than 18 years-old affected by Chandler stage II, III, or IV orbital cellulitis. The inflammation was resolved medically in 6 patients. In the 30 cases that showed no improvement in 48-72 h, an endoscopic drainage of the pus was performed by the transethmoidal route. In children,…

Overcoming low orbital overlap and triplet instability problems in TDDFT.

Low orbital overlap and triplet instability problems in time-dependent density functional theory (TDDFT) are investigated for a new benchmark set, encompassing challenging singlet and triplet excitation energies of local, charge-transfer, and Rydberg character. The low orbital overlap problem is largely overcome for both singlet and triplet states by the use of a Coulomb-attenuated functional. For all the categories of functional considered, however, errors associated with triplet instability problems plague high overlap excitations, as exemplified by the excited states of acenes and polyacety…

Rhino-orbital-mucormycosis as a presenting manifestation of gestational diabetes mellitus.

Rhino-orbital mucormycosis is an uncommon and aggressive, angioinvasive fungal infection that occurs in immunocompromised states like diabetes mellitus, chronic renal failure, hematological malignancies and deferroxamine therapy. We report a patient who presented with rhino-orbital mucormycosis at six months of gestation and was incidentally detected to have diabetes. She was successfully treated with amphotericin B and appropriate surgery. To the best of our knowledge, there is no such report in the literature.

Inlay implanting technique for the correction of medial orbital wall fracture.

BACKGROUND: For blowout fractures of the medial orbital wall, the goals of treatment are complete reduction of the herniated soft tissue and anatomical restoration of the orbital wall without surgical complications. Surgeons frequently worry about damage to the optic nerve caused by dissection when the part over the posterior ethmoidal foramen is fractured. The authors performed small incision and inlay implantation of porous polyethylene for reconstruction of medial orbital wall fractures. METHODS: Between January of 2007 and December of 2009, 55 patients were included in an analysis of the …

Retro-orbital intraconal fat injection: an experimental study in rabbits.

PURPOSE: Despite adequate treatment, enophthalmos due to intraconjunctival corticosteroid injection and enlargement of the bony orbit after trauma remains a frequent complication. The use of alloplastic material in addressing this problem is restricted because it may result in allergic reactions and is not cost-effective. The use of retro-orbital intraconal injection is the most effective method for maximum augmentation. An inexpensive and minimally invasive alternative that also allows for reoperation when needed would be a preferred intervention. MATERIALS AND METHODS: We used 24 white rabb…

Repair of exposed hydroxyapatite orbital implants by subconjunctival tissue flaps.

PURPOSE: The purpose of the study was to determine the feasibility of the use of subconjunctival tissue flaps to repair exposed hydroxyapatite (HA) orbital implants. METHODS: One hundred thirty-eight consecutive patients underwent repair of exposed HA orbital implants via a procedure that involved the creation of subconjunctival tissue flaps. After the superior and inferior subconjunctival flaps were created, they were sutured together in the tension-free state to seal the exposure. During follow-up examinations, complications were assessed. RESULTS: Twelve patients were lost after the 3-mon…

Orbital mass as manifestation of Wegener's granulomatosis: an ophthalmologic diagnostic approach.

Most people in modern society are exposed daily to fragrance ingredients from one or more sources. Fragrance ingredients are also one of the most frequent causes of contact allergic reactions. The diagnosis is made by patch testing with a mixture of fragrance ingredients, the fragrance mix. This gives a positive patch-test reaction in about 10% of tested patients with eczema, and the most recent estimates show that 1.7-4.1% of the general population are sensitized to ingredients of the fragrance mix. Fragrance allergy occurs predominantly in women with facial or hand eczema. These women typica

[A case of noninvasive sinus aspergillosis showing orbital apex syndrome].

A 78-year-old man was admitted to our hospital with headache, nasal pain, left-sided ptosis, loss of visual field in his left eye, and left ophthalmoplegia. Serum levels of beta-D-glucan were elevated. T1-weighted magnetic resonance imaging with gadolinium enhancement showed hyperintense lesions in the left orbital apex and dura mater of the left middle cranial fossa. A few days later, culture of specimens collected by surgical debridement from the left sphenoidal sinus revealed numerous branching hyphae. The aspergillus antigen was found in the cerebrospinal fluid (CSF). Therefore, aspergillo…
