Impact of Liver and Primary Tumor Localization on Survival in Lung Metastasectomy for Colorectal Carcinoma.

PURPOSE: Despite the knowledge that right-sided colon tumors are associated with worse overall survival (OS) and disease-free survival (DFS) compared to left-sided and rectal tumors, there are conflicting results on the impact of the primary location of colorectal tumors after lung metastasectomy. In this study, we aimed to investigate this contradiction. METHODS: We conducted a retrospective analysis of 131 patients who underwent lung metastasectomy for colorectal cancer. DFS and OS were evaluated in relation to primary tumor location, liver metastasis, lymph node dissection, nodal status, a

Preoperative assessment of pleural adhesions using computed tomography scan and positron emission tomography in patients with lung cancer.

OBJECTIVES: The presence of pleural adhesions during lung cancer surgery can obstruct the surgical field and inhibit maneuverability, thereby potentially complicating the procedure. The current study examined the potential predictive factors of pleural adhesions using standard preoperative examinations for lung cancer surgery without additional assessments. METHODS: This study included 542 patients with primary lung cancer who underwent chest computed tomography scan and positron emission tomography before undergoing surgery between January 2021 and September 2024. To assess differences in lu

Postoperative Sarcopenia and Association with Recurrence in Resected Early-Stage Non-Small Cell Cancer.

BACKGROUND: We hypothesize that reduction in skeletal muscle volume between the time of surgery and the first postoperative surveillance CT scan is associated with recurrence in resected patients with early-stage non-small cell lung cancer (NSCLC). STUDY DESIGN: Patients who underwent lung resection for pT1-2aN0 NSCLC between 2010 and 2021 were identified. Exclusion criteria included neoadjuvant or adjuvant therapy, missing CT scan data, and steroid use. Volumetric body composition analysis was performed with Data Analysis Facilitation Suite software using preoperative and initial postoperati

A Case of Pulmonary Bulla Volume Reduction by Transbronchial Puncture Under Electromagnetic Navigation Platform Combined With O-Arm CT Guidance.

Pulmonary bullae are a common complication of chronic obstructive pulmonary disease (COPD), but treatment options are limited for patients with poor lung function and giant emphysematous bullae (GEB). We reported a case of a 61-year-old female with severe COPD complicated by a newly developed GEB. Using electromagnetic navigation technology, we precisely located and targeted the bronchi adjacent to the bullae within the lung. Guided by O-arm CT imaging, we punctured the bullae with a biopsy needle, evacuated the air, and administered autologous blood and thrombin. A follow-up pulmonary CT scan

Dynamics of recurrence after curative resection of nonsmall cell lung cancer.

BACKGROUND AND OBJECTIVES: This study examined the trend of hazards for postoperative recurrence of lung cancer according to pathologic stages. METHODS: We reviewed the records of 1987 patients who underwent resection for lung cancer between 2007 and 2012. Postoperative recurrence and development of second primary lung cancer were analyzed to evaluate the trend of hazard rate. RESULTS: Recurrence-free survival (RFS) probabilities at 5 postoperative years in patients with stage I/II/III disease were 87.8%/54.7%,/33.4%, respectively. The hazard rate of RFS was consistently low (<0.005) for sta

Lymphatic invasion is a cause of local recurrence after wedge resection of primary lung cancer.

OBJECTIVE: After securing a sufficient surgical margin at wedge resection and finding no pathologic evidence of residual tumor at the surgical margin, a considerable number of patients develop local recurrence. We investigated the correlation between sub-pleural lymphatic flow and local recurrence. METHODS: We retrospectively reviewed the medical records of 144 non-small cell lung cancer patients who underwent wedge resection between January 2006 and December 2014 at our institution. RESULTS: Postoperative recurrence was observed in 36 patients (25%). Of these, local recurrence was observed

A Rare Complication: Development of an Aspergilloma after Endobronchial Coil Placement in a COPD Patient.

Endobronchial coils are a relatively novel endoscopic lung volume reduction modality that aims to increase functional capacity in chronic obstructive pulmonary disease (COPD) patients. Two major trials have studied the safety and efficacy of this therapy, but long-term safety has not been studied. Adverse events reported are mainly periprocedural pneumothoraces and early bacterial infectious complications. We report the case of a patient with severe emphysema (Global Initiative for Chronic Obstructive Lung Disease stage IV COPD) who developed endobronchial coil-associated aspergillomas 3 years

Centrally located lung cancer and risk of occult nodal disease: an objective evaluation of multiple definitions of tumour centrality with dedicated imaging software.

INTRODUCTION: Current guidelines recommend invasive mediastinal staging in patients with centrally located radiographic stage T1N0M0 nonsmall cell lung cancer (NSCLC). The lack of a specific definition of a central tumour has resulted in discrepancies among guidelines and heterogeneity in practice patterns. METHODS: Our objective was to study specific definitions of tumour centrality and their association with occult nodal disease. Pre-operative chest computed tomography scans from patients with clinical (c) T1N0M0 NSCLC were processed with a dedicated software system that divides the lungs i

Closure of bronchopleural fistula by a septal occluder device: a case for close collaboration between heart and lung specialists.

We present the case of a 66-year-old woman who underwent right inferior lobectomy for pulmonary carcinoma and developed persistent bronchopleural fistula (BPF) that was not amenable to surgical intervention (two surgical failures). The patient presented with a persistent cough and dyspnoea, which was treated with a hybrid procedure using fluoroscopy and bronchoscopy. A 7 mm Amplatzer septal occluder device (ASOD) was successfully inserted into the BPF. Two weeks after the procedure, a small fistula developed, which was treated by endoscopically guided biologic glue embolisation. At 2-month, 6-

Predicting Pathology From Imaging in Children Undergoing Resection of Congenital Lung Lesions.

BACKGROUND: Prenatal magnetic resonance imaging (MRI) is increasingly obtained to define congenital lung lesions (CLL) for surgical management. Postnatal, preoperative computed tomography (CT) provides further clarity at the cost of radiation. Depending on the lesion identified, the indication for resection remains controversial. We investigated the differences in detail found on prenatal MRI and postnatal CT compared with final pathology to determine their utility in preoperative decision-making. MATERIALS AND METHODS: All children undergoing resection of CLLs at a single institution between

Medicoeconomic analysis of lobectomy using thoracoscopy versus thoracotomy for lung cancer: a study protocol for a multicentre randomised controlled trial (Lungsco01).

INTRODUCTION: In the last decade, video-assisted thoracoscopic surgery (VATS) lobectomy for non-small cell lung cancer (NSCLC) has had a major effect on thoracic surgery. Retrospective series have reported benefits of VATS when compared with open thoracotomy in terms of postoperative pain, postoperative complications and length of hospital stay. However, no large randomised control trial has been conducted to assess the reality of the potential benefits of VATS lobectomy or its medicoeconomic impact. METHODS AND ANALYSIS: The French National Institute of Health funded Lungsco01 to determine w

New tubeless video-assisted thoracoscopic surgery for small pulmonary nodules.

OBJECTIVES: Problems associated with intubation, chest drainage and urinary catheterization can have a negative impact on patient's recovery after thoracic surgery. We therefore evaluated the feasibility of a new tubeless (spontaneous ventilation without tracheal intubation, urinary catheterization, and no post-operative chest drain placement) approach to perform video-assisted thoracoscopic surgery (VATS) for small pulmonary nodules (SPN) less than 2cm in diameter. METHODS: From 1 January 2012 to 31 December 2014, 34 patients with SPNs were treated using tubeless VATS in our centre. To be el

Arterial blood gases predict long-term prognosis in stage I non-small cell lung cancer patients.

BACKGROUND: Preoperative hypercapnia and hypoxemia are reportedly risk factors for postoperative complications. This study aimed to establish the long-term survival risk associated with abnormal preoperative arterial blood gases (ABGs) in patients with non-small cell lung cancer (NSCLC). METHODS: This study involved 414 patients with stage I NSCLC who underwent lobectomy/bilobectomy with mediastinal lymph node dissection. The patients were divided into groups with normal (n = 269) and abnormal (n = 145) ABGs. RESULTS: The patients in the normal ABG group (median age 67 years) were significan
