Right Unilateral versus Bilateral Subcostal Transversus Abdominis Plane Block for Postoperative Analgesia in Patients Undergoing Laparoscopic Cholecystectomy under General Anesthesia: A Randomized Controlled Trial.

BACKGROUND AND AIM: Laparoscopic cholecystectomy necessitates focused analgesia on the right abdominal wall region to address the somatic component of postoperative pain. Ultrasound-guided preemptive subcostal transversus abdominis plane (TAP) block blocks the anterior branches of thoracic spinal nerves (T6-T9). This study aimed to assess whether a right unilateral subcostal TAP block provides a similar duration of operative site analgesia compared to a bilateral subcostal TAP block during laparoscopic cholecystectomy. METHODOLOGY: A prospective randomized controlled trial was conducted on 50

Evaluation of Analgesic Effects of Ketamine versus Dexmedetomidine as an Adjuvant to 0.2% Ropivacaine Infusion in Adductor Canal Block for Postoperative Analgesia following Unilateral Total Knee Replacement Surgery.

BACKGROUND: Postoperative pain management is critical for recovery following total knee replacement (TKR). Significant pain following surgery is linked to TKR, and this discomfort might have a negative impact on the patient's satisfaction, length of stay, and functional recovery. The adductor canal block (ACB) is preferred for its motor-sparing properties. This study compared the analgesic efficacy of ketamine and dexmedetomidine as adjuvants to ropivacaine infusion in ACB for patients undergoing unilateral TKR. METHODS: In this prospective, randomized, comparative study, 44 patients (America

Effect of iPACK block versus femoral nerve block on postoperative movement pain and muscle strength in patients undergoing total knee replacement.

OBJECTIVES: Postoperative pain and quadriceps weakness hinder rehabilitation following total knee arthroplasty (TKA), creating a dilemma when using motor-blocking femoral nerve blocks (FNB). This study compared the effect of the motor-sparing infiltration popliteal artery-knee capsule (iPACK) block relative to FNB on analgesia and functional recovery. METHODS: This retrospective cohort study analyzed patients undergoing unilateral TKA under spinal anesthesia between June 2022 and October 2025, grouped by received block (iPACK or FNB). Resting and activity pain were assessed at 2, 6, 12, and 2

Persistent headache burden after surgical treatment of post-dural puncture headache: a cross-sectional study.

BACKGROUND: Post-dural puncture headache (PDPH) may follow intended or unintended spinal dural puncture and is usually self-limiting. In some patients, however, symptoms persist despite epidural blood patches, while postoperative patient-reported data are largely absent. We assessed headache disability, functional status, and psychosocial burden among respondents self-reporting prior surgery for PDPH or dural-puncture-related CSF leak. METHODS: An online cross-sectional survey was conducted between February and April 2025. Adults who self-reported prior surgical treatment for PDPH or dural-pu

Preemptive Propofol Administration in Spinal Cord Injury: Effects on Pain-Induced Hypertension, Neuroinflammation, and Functional Recovery in Rats.

Spinal cord injury (SCI) triggers secondary damage, including pain-induced hypertension, inflammation, and hemorrhage, impairing recovery. This study evaluated the efficacy of general anesthesia with preemptive propofol administration in mitigating secondary damage in SCI rats. SCI was induced in rats using a contusion model. Propofol (100 mg/kg) was administered intraperitoneally either 30 min before (preemptive) or 30 min after intermittent tail shock. Systolic blood pressure (SBP), body weight, food intake, inflammatory markers (interleukin-1 beta [IL-1β], interleukin-6 [IL-6]), hemorrhage

Perioperative intravenous lidocaine as an analgesic adjunct in adolescent idiopathic scoliosis surgery.

Opioids are the mainstay of pain management in scoliosis surgery. We hypothesized that in adolescent idiopathic scoliosis (AIS) patients undergoing posterior spinal fusion (PSF) surgery, perioperative intravenous (IV) lidocaine would reduce postoperative opioid requirement and pain scores. In this retrospective observational before-and-after study, we identified AIS patients who underwent single-stage PSF at a tertiary university hospital from 2020 to 2022. All patients received total intravenous anesthesia. The Lidocaine group received a bolus of 1.5 mg/kg IV lidocaine prior to induction, fol

Transvesical blockade of the obturator nerve to prevent adductor contraction in transurethral resection of urinary bladder tumor.

INTRODUCTION: Urinary bladder tumors are one of the most common urological malignancies. Traditionally, it has been managed with trans-urethral resection of urinary bladder tumor (TURBT) for both diagnostic and therapeutic purposes. During TURBT of lateral wall tumors, there is risk of obturator nerve reflex (ONR), which can lead to serious complications such as inadvertent bleeding and urinary bladder perforation. To prevent this, obturator nerve block is given after spinal anesthesia. In this study, we have used the transvesical approach to block the obturator nerve. MATERIALS AND METHODS:

Successful Non-fusion Management of a Healed Pediatric Odontoid Synchondrosis Fracture: A Case Report.

Odontoid synchondrosis fractures are rare pediatric spinal injuries that pose significant diagnostic and treatment challenges. These fractures, typically occurring in the C-2 vertebra, often result from high-energy trauma and are difficult to diagnose due to subtle radiographic findings. We present the case of a 4-year-old male who sustained a head injury while playing on a trampoline. Initially diagnosed with a head injury and discharged without noting any neck complaints, the patient returned to the ER two months later with neck pain and stiffness. Radiographs revealed a C2 fracture at the b

The association between gut microbiota and postoperative delirium in patients.

Postoperative delirium is a common postoperative complication in older patients, and its pathogenesis and biomarkers remain largely undetermined. The gut microbiota has been shown to regulate brain function, and therefore, it is vital to explore the association between gut microbiota and postoperative delirium. Of 220 patients (65 years old or older) who had a knee replacement, hip replacement, or laminectomy under general or spinal anesthesia, 86 participants were included in the data analysis. The incidence (primary outcome) and severity of postoperative delirium were assessed for two days.

No clinically significant difference in postoperative pain and side effects comparing conventional and enhanced recovery total hip arthroplasty with early mobilization.

INTRODUCTION: Enhanced recovery after surgery (ERAS) leads to less morbidity, faster recovery, and, therefore, shorter hospital stays. The expected increment of primary total hip arthroplasty (THA) in the U.S. highlights the need for sufficient pain management. The favorable use of short-lasting spinal anesthesia enables early mobilization but may lead to increased opioid consumption the first 24 h (h) postoperatively. METHODS: In a retrospective study design, we compared conventional THA with postoperative immobilization for two days (non-ERAS) and enhanced recovery THA with early mobilizati
