Rush-pin fixation of supracondylar and intercondylar fractures of the femur.

A study was done of ninety-eight patients who had supracondylar and intercondylar fractures of the femur that were treated by the insertion of Rush pins. A system of classification relating the type of fracture to the method of Rush-pin fixation is described, and the importance of proper surgical technique is emphasized. The patients were followed for periods ranging from one to seventeen years (mean, 3.5 years) and 84 per cent of them had either excellent or good results, as evaluated by both radiographic and clinical examination. There were two non-unions and one deep infection. Angulation a

External fixation for complicated tibial fractures.

An account is given of 38 patients with complicated tibial fractures who were treated by the Portsmouth method of external fixation. Twenty-one patients had multiple injuries and 30 had compound fractures of the tibia. Eighteen fractures wounds were infected, 17 cases required bone grafts and 13 had skin grafts. Thirty-four fractures united in an average time of six months; three patients underwent below-knee amputations; one with neurofibromatosis remains ununited. Those treated primarily by external fixation did better than those in whom external fixation was used after failure of another me

[Diagnosis and treatment of postoperative infections following hip injuries].

The authors studied the diagnosis and treatment of posttraumatic infections of the hip joint, based on their own material. In case of latent infection the increased sedimentation rate and taking of comparative X-ray films are held important, that shows a narrowing and disappearance of the sclerosed edge of the acetabulum. Following dislocation of the hip joint ankylosis may be attained, but after fractures of the proximal and of the femur the infection cannot be cured, unless the nail and the head of the femur are removed.

A Randomized Trial Comparing Outcome of Expert Tibial Nail versus Proximal Tibial Plate in the Management of Fractures of Proximal Tibia in Adults.

BACKGROUND: Extra-articular fracture of the proximal tibia frequently results in functional impairment of the knee and lower limb if not managed properly. Any treatment method must achieve the desired anatomical reduction and stable fixation to permit early weight-bearing and joint motion. This type of fracture can be treated using a variety of methods, from conservative casting to surgical management. It is still for debate that, which surgical procedure is best for treating extra-articular proximal tibia fractures. AIM: This study aims to compare the functional and radiological outcome of e

Optimising the tip-apex-distance in trochanteric femoral fracture fixation using the ADAPT-navigated technique, a longitudinal matched cohort study.

INTRODUCTION: The annual incidence of proximal femoral fractures is 100-150/100,000 and continues to increase with an aging population. Cut-out of hip screws after fracture fixation has been quoted as 8% in the literature. The tip-apex distance (TAD) is the strongest predictor for cut-out after operative fracture stabilisation. The aim of this study was to evaluate the novel ADAPT system (Adaptive Positioning Technology, Stryker, USA), a navigation device for intramedullary nailing of trochanteric fractures and its effect on optimising the TAD. This is the first clinical study to evaluate this

Balloon-assisted reduction, pin fixation and tricalcium phosphate augmentation for calcaneal fracture: A retrospective analysis of 42 patients.

INTRODUCTION: The treatment of intra-articular calcaneal fractures is still complex and controversial. Although open reduction and internal fixation (ORIF) is favored by many authors, several percutaneous techniques have been introduced to reduce complications and to obtain satisfactory clinical and radiological results. Among these percutaneous treatments, balloon reduction and bone graft augmentation is gaining an increasing popularity. MATERIALS AND METHODS: We retrospectively examined a series of 42 patients treated operatively with a minimally invasive reduction technique using an inflat

Trochanteric entry femoral nails yield better femoral version and lower revision rates-A large cohort multivariate regression analysis.

INTRODUCTION: Intramedullary nailing (IMN) has become the standard of care for the treatment of most femoral shaft fractures. Different IMN options include trochanteric and piriformis entry as well as retrograde nails, which may result in varying degrees of femoral rotation. The objective of this study was to analyze postoperative femoral version between three types of nails and to delineate any significant differences in femoral version (DFV) and revision rates. MATERIALS & METHODS: Over a 10-year period, 417 patients underwent IMN of a diaphyseal femur fracture (AO/OTA 32A-C). Of these pati

Uncontrolled diabetes as a potential risk factor in tibiotalocalcaneal fusion using a retrograde intramedullary nail.

BACKGROUND: Tibiotalocalcaneal (TTC) fusion using a retrograde intramedullary (IM) nail is an effective salvage option for terminal-stage hindfoot problems. However, as many patients who receive TTC fusion bear unfavorable medical comorbidities, the risk of nonunion, infection and other complications increases. This study was performed to identify the factors influencing outcomes after TTC fusion using a retrograde IM nail. METHODS: Between September 2008 and February 2012, 34 consecutive patients received TTC fusion using a retrograde IM nail for limb salvage. All patients had a minimum foll

External fixation in the elderly.

Orthopaedic trauma is an increasingly common problem in geriatric patients. As demands of daily life and recreational activities are increasing in these patients, surgeons need to be able to manage geriatric fractures to achieve good functional results. Reduced bone quality in the elderly presents a considerable challenge and may preclude the use of established surgical stabilisation techniques that are performed in younger trauma patients. Furthermore, pre-existing medical conditions and considerable comorbidities in the elderly could complicate standard surgical procedures that younger patie

Shortening of the distal ulna to correct ulnar variance in osteoporotic wrist fractures treated with intra-focal pining.

The aim of this study is to correct the ulnar variance in severe osteoporotic wrist fracture treated with intra-focal pining of the distal radius, with shortening osteotomy of the distal ulna. Thirteen patients, 12 females and one male, 61-83 years old, 6-year follow-up, were treated by intra-focal pining (kapandji). When preoperatively the ulnar variance did not return to a normal value comparing to the opposite wrist, a shortening osteotomy of the ulna was performed in one stage, 4 cm proximal to the ulnar head. The fractures healed 6-8 weeks with stability and normal motion: flexion 70°, ex

Sonographically assisted percutaneous removal of screws in dynamization of the interlocking intramedullary nail.

OBJECTIVES: Dynamization is a method of removing the interlocking screw(s) farthest from the fracture site for improving healing in femoral and tibial fractures that show delayed healing after static interlocking nailing. We describe a simple sonographically assisted technique for percutaneous dynamization of deep-seated impalpable screws. METHODS: Between March 2001 and March 2005, 20 dynamization procedures were completed using the developed technique under the diagnosis of delayed healing or a nonunion at a mean of 4.8 months after index surgery. After adequately positioning the involved l

[Therapy of humeral shaft fractures].

Fractures of the humeral shaft are less frequent than those of the proximal humerus. The formerly recommended treatment of humeral shaft fractures was conservative according to Böhler. This still remains an adequate concept of treatment but according to a change in the technical possibilities and the demands of patients and physicians on fast restoration of function and low pain, there is a trend towards surgical stabilization of humeral shaft fractures. The implant of choice is discussed controversially and consists of various types of nails versus plating. The technique of nailing is antegra

Stability of fixation of diacapitular fractures of the mandibular condylar process by ultrasound-aided resorbable pins (SonicWeld Rx® System) in pigs.

To assess the stability of osteosynthesis in diacapitular condylar fractures we compared fixation using ultrasound-aided resorbable pins with poly-(D,L)-lactide (SonicWeld® Rx, KLS Martin, Tuttlingen, Germany) with that of titanium screws in 20 pig mandibles, 10 in each group. Isolated diacapitular fractures were created using a surgical chisel. Ten fractures were each repositioned and fixed by two pins (17 and 11 mm long, 2.1mm in diameter), and 10 fractures were fixed by two titanium screws of equal length, 2.0mm in diameter. Shear tests were done immediately after treatment to measure the m

New intramedullary locking nail for olecranon fracture fixation--an in vitro biomechanical comparison with tension band wiring.

BACKGROUND: The aim of this study was to determine the difference in displacement of a newly designed intramedullary olecranon fracture fixation device compared with multifilament tension band wiring after 4 cycles and 300 cycles of dynamic continuous loading. METHODS: In eight pairs of fresh-frozen cadaver ulnae, oblique olecranon fractures were created and stabilized using either newly designed intramedullary olecranon nail or multifilament tension band wiring. The specimens were then subjected to continuous dynamic loading (from 25 N to 200 N) using matched pairs of cadaveric upper extremi

Lumbar disc rehydration postimplantation of a posterior dynamic stabilization system.

Biological attempts at disc regeneration are promising; however, disc degeneration is closely related to other predisposing factors such as alteration of disc height, intradiscal pressure, load distribution, and motion. The restoration of the physiological status of the affected spinal segment is thus necessary prior to attempts at disc regeneration. Dynamic stabilization systems now offer the potential of a mechanical approach to intervertebral disc regeneration. The authors used decompression and placement of the BioFlex dynamic stabilization device to treat a young male patient with disc de

[Follow-up our patients with proximal humeral fractures between 2000 and 2005].

Authors introduced a new method in 2000 for the operative treatment of proximal end of the humerus. Because of the anatomical properties of the shoulder, great emphasis is placed on the protection of the soft tissues of the joint, the minimal invasive operative technique. On the other hand, the osteoporosis, which presents itself in the old age, makes the osteosynthesis more difficult; the proportion of complication is high in spite of the most careful treatment. Authors collected the materials of these 6 years and they have ordered their operated patients back for examination. After a physica
