Pre-pregnancy weight, the rate of gestational weight gain, and the risk of early gestational diabetes mellitus among women registered in a tertiary care hospital in India.

BACKGROUND: The impact of pre-pregnancy weight and the rate of gestational weight gain (GWG) together on the risk of early GDM (< 24 weeks gestation; eGDM) has not been studied in the Indian context. We aimed to study the influence of (1) pre-pregnancy weight on the risk of eGDM diagnosed in two time intervals; and (2) in addition, the rate of GWG by 12 weeks on the risk of eGDM diagnosed in 19-24 weeks. METHOD: Our study utilized real-world clinical data on pregnant women routinely collected at an antenatal care clinic at a private tertiary hospital, in Pune, India. Women registering before

Parental coping with uncertainties along the severe combined immunodeficiency journey.

BACKGROUND: Severe combined immunodeficiency (SCID) is a group of rare genetic disorders that cause disruption in immune system functioning. Parents of children with SCID experience many uncertainties related to their child's diagnosis, treatment, recovery, and quality of life. To fully understand parents' experiences throughout their SCID journey, it is important to explore the stressors generated by such uncertainties and how parents cope with these stressors. METHODS: We conducted 26 in-depth interviews with parents whose child was diagnosed with SCID or a SCID-like condition through newbo

Monkeypox 2022: A Primer and Identify-Isolate-Inform (3I) Tool for Emergency Medical Services Professionals.

Monkeypox 2022 exhibits unprecedented human-to-human transmission and presents with different clinical features than those observed in prior outbreaks. Previously endemic only to West and Central Africa, the monkeypox virus spread rapidly world-wide following confirmation of a case in the United Kingdom on May 7, 2022 of an individual that had traveled to Nigeria. Detection of cases with no travel history confirms on-going community spread. Emergency Medical Services (EMS) professionals will likely encounter patients suspected or confirmed to have monkeypox, previously a rare disease and there

The use of inappropriate anal douching tool associates with increased HIV infection among men who have sex with men: a cross-sectional study in Shenyang, China.

BACKGROUND: Rectal douching (RD) is widely practiced by men who have sex with men (MSM), and is associated with increased risk of HIV infection. However, the mechanism of how RD increases the risk of HIV infection is not well understood, and there is limited data on RD behavior in MSM practicing anal sex in China. We examine the purpose of RD, its timing in relation to anal sex, the types of RD products used, and risky sexual behaviors among MSM reporting anal sex. METHODS: Between August 2017 and December 2018, a cross-sectional study was conducted among adult MSM in Shenyang, China. Data we

Pan-London tuberculosis services: a service evaluation.

BACKGROUND: London has the largest proportion of tuberculosis (TB) cases of any western European capital, with almost half of new cases drug-resistant. Prevalence varies considerably between and within boroughs with research suggesting inadequate control of TB transmission in London. Economic pressures may exacerbate the already considerable challenges for service organisation and delivery within this context. This paper presents selected findings from an evaluation of London's TB services' organisation, delivery, professional workforce and skill mix, intended to support development of a strat

Designing acceptable, feasible and engaging social prescribing pathways: the case of connecting from general practice to 'Golf for Health'.

BACKGROUND: Globally social prescribing (SP) is receiving increasing attention for its role in health and social care. There is little evidence and a call for consideration on a seldom considered and hugely important aspect of SP schemes - the connection process (identification, enhancing likelihood of behaviour change, and connection). This was a novel study aiming to design an acceptable, feasible and engaging process for connecting individuals from general practice to a free golf programme in Scotland. METHODS: A two-phase participatory approach drawing on the double diamond process of des

Evaluating the use of the consolidated framework for implementation research in primary care settings: a systematic review.

BACKGROUND: Previous systematic reviews have examined the use of the 2009 version of the Consolidated Framework for Implementation Research (CFIR) in healthcare settings. However, these reviews primarily focused on studies conducted in secondary and tertiary care, with limited attention to its application in primary care. The use of the CFIR in primary care remains underexplored. Given the unique attributes of primary care-guiding principles such as first-contact care, continuity, comprehensiveness, coordination, and people-centeredness-findings from studies in other healthcare settings may no

Development of Guidelines for Implementing Digital Technology Into In-Home Aged Care: Mixed Methods Approach.

BACKGROUND: In-home aged care is a vital part of Australia's formal aged care system, offering essential support for older adults living in their homes. From 2023 to 2024, over 1.1 million Australians received in-home aged care, and this number is expected to grow. However, significant workforce shortages are currently occurring in the sector. One potential solution to workforce shortages is integrating technology into in-home aged care. Technology can support health monitoring, care coordination, emergency detection, social connection, and physical activity. Despite progress in residential ag

Barriers and strategies to enhance HPV vaccine uptake in adolescents living with HIV and their guardians in Lilongwe, Malawi.

Female Adolescents Infected with HIV are at higher risk for Human Papillomavirus -related cancers later in life, making them a priority for HPV vaccination. Since HPV-related cancers typically develop after adolescence, HPV vaccination before exposure to HPV infection is most effective. Increasing vaccination rates in this group is key to reducing the risk of cervical cancer incidence. The Global Action strategy to eliminate cervical cancer requires achieving 90% national coverage and 80% district coverage for HPV vaccination. However, the uptake of HPV vaccine in Malawi has been slow, with an

Profile of gastroduodenal perforation patients admitted in a rural tertiary care hospital: An observational cross-sectional study.

BACKGROUND: Gastro-duodenal perforation is a common surgical emergency that remains a formidable health burden worldwide with significant morbidity and mortality. Ulcer disease remains the most common cause of gastro-duodenal perforation. Diagnosing the presence of H. pylori can help eradicate the infection from the community at large and thereby reduce the chances of gastro-duodenal perforation. AIMS: To assess the clinical presentation of gastro-duodenal perforation patients and to evaluate the detection of Helicobacter pylori infection by available investigations. MATERIALS AND METHODS: A

HIV self-testing implementation, distribution and use among female sex workers in Cotonou, Benin: a qualitative evaluation of acceptability and feasibility.

BACKGROUND: In Benin, the burden of HIV is disproportionately high among female sex workers (FSWs). HIV testing and knowledge of status are starting points for HIV treatment and prevention interventions. Despite the importance given to testing services in HIV control, its uptake among FSWs remains suboptimal in Benin. HIV self-testing (HIVST) may be useful for increasing testing rates in FSWs. METHODS: We conducted a pilot study of the distribution of saliva-based HIVST among FSWs in Cotonou and its surroundings, Benin. The HIVST promotion and distribution model included three complementary s
