Postoperative accuracy analysis of three-dimensional corrective osteotomy for cubitus varus deformity with a custom-made surgical guide based on computer simulation.

BACKGROUND: For correction of cubitus varus deformity resulting from supracondylar fracture of the humerus, we developed an operative method with use of a custom-made surgical guide, designed on the basis of 3-dimensional (3D) computer simulation with computed tomography data. The purpose of this study was to investigate the postoperative accuracy of this system in clinical cases. METHODS: Subjects included 17 consecutive patients (13 males and 4 females) with cubitus varus deformity after supracondylar fracture. Patients underwent 3D corrective osteotomy with use of a custom-made surgical gu

Linked elbow replacement: a salvage procedure for distal humeral nonunion. Surgical technique.

BACKGROUND: Nonunion is a challenging and not uncommon complication of distal humeral fractures. Our long-term experience with linked semiconstrained total elbow arthroplasty as a salvage procedure for patients with distal humeral nonunion not amenable to internal fixation was investigated. METHODS: Ninety-one consecutive patients (ninety-two elbows) underwent total elbow arthroplasty for the treatment of a distal humeral nonunion, and the results were reviewed at a mean of 6.5 years postoperatively. Patients' charts and anteroposterior and lateral radiographs made prior to and immediately af

The use of pre-contoured plates in the treatment of C2-C3 fractures of the distal humerus: clinical experience.

Open reduction and internal fixation is the treatment of choice in distal humerus fractures. The aims of the treatment are to obtain anatomic reconstruction and restoration of the elbow's geometry followed by stable internal fixation that allows immediate rehabilitation. The plates are pre-contoured to fit the natural anatomy of the elbow and in the case of complex fractures they provide a guide for the anatomic restoration of the distal humerus. The two plates are placed parallel with the screws locked together by interdigitation creating a fixed angle in the distal fragments that provides st

[Total elbow joint allograft for long term posttraumatic osteoarticular loss. Follow-up results at twelve years].

PURPOSE OF THE STUDY: Elbow joint allograft (EJA) involving the entire joint (distal humerus, proximal radius and ulna, capsuloligament structures) is a salvage technique proposed in massive bone loss, particularly in young subjects where total elbow prosthesis is contraindicated. We report our experience with seven patients, analyzing the long-term clinical and radiological outcome. MATERIAL AND METHODS: This retrospective study included seven patients, mean age 42 years (21-70). All had experienced severe elbow trauma. Two patients had associated neuromuscular or vascular lesions. All patie

Early mobilisation versus plaster immobilisation of simple elbow dislocations: results of the FuncSiE multicentre randomised clinical trial.

BACKGROUND/AIM: To compare outcome of early mobilisation and plaster immobilisation in patients with a simple elbow dislocation. We hypothesised that early mobilisation would result in earlier functional recovery. METHODS: From August 2009 to September 2012, 100 adult patients with a simple elbow dislocation were enrolled in this multicentre randomised controlled trial. Patients were randomised to early mobilisation (n=48) or 3 weeks plaster immobilisation (n=52). Primary outcome measure was the Quick Disabilities of the Arm, Shoulder, and Hand (Quick-DASH) score. Secondary outcomes were the

Obstetric brachial plexus palsy in newborn babies of diabetic and non-diabetic mothers.

Compared to non-diabetic mothers, diabetic mothers are known to deliver larger babies who are at higher risk for shoulder dystocia and obstetric brachial plexus palsy. The intrapartum forces applied during delivery of larger babies are expected to be higher. Hence, the chances of these babies for good spontaneous recovery are expected to be lower; and this is a generally believed hypothesis. The objective of this retrospective study was to compare obstetric brachial plexus palsy in newborn babies of diabetic and non-diabetic mothers. There were a total of 655 cases of obstetric palsy: 253 case

Delayed Horner's syndrome during a continuous infraclavicular brachial plexus block.

Horner's syndrome is a potential, albeit rare, feature of continuous infraclavicular brachial plexus local anesthetics infusion, mainly the result of anatomical considerations. Horner's syndrome may be described as an "unpleasant side effect" because it has no clinical consequences in itself. Nevertheless, patient discomfort and anxiety may reduce acceptance of the analgesic technique. Reassurance and close clinical monitoring of the patient are essential to enhance patient's safety and acceptance of the technique.

Cubital tunnel syndrome in adolescent baseball players: a report of six cases with 3- to 5-year follow-up.

In this case report, we describe the clinical features and surgical outcome of cubital tunnel syndrome in adolescent baseball players. Two infielders, 2 pitchers, and 2 catchers who suffered cubital tunnel syndrome during adolescence (average age, 14 years) were surgically treated. Symptoms of medial elbow pain first appeared during throwing in competition games in summer or autumn seasons. After the onset, they suffered limitation of elbow extension and weakness on grabbing balls. They could not throw because of recurrent medial elbow pain. Laxity of the medial collateral ligament was not det

Fall arrest strategy affects peak hand impact force in a forward fall.

We measured the peak hand impact force involved in bimanually arresting a forward fall to the ground from a 1-m shoulder height in five healthy young males. The effects of three different subject instruction sets: "arrest the fall naturally"; "keep the head as far from the ground as possible"; and "minimize the peak hand forces" were studied by measuring body segment kinematics, ground reaction forces, and upper-extremity myoelectric activity. The hypotheses were tested that the (a) arrest strategy did not influence peak impact force, (b) arm configuration, impact velocity and upper-extremity

Surgical techniques for reconstruction of chronic insufficiency of the triceps. Rotation flap using anconeus and tendo achillis allograft.

Seven patients with chronic insufficiency of the triceps were treated by either a rotation flap using anconeus (4) or an allograft of tendo Achillis (3). The latter procedure was selected for patients with a large defect in whom the anconeus muscle had been devitalised. Five disruptions were in patients who had previously undergone an elbow replacement. The patients were assessed for subjective satisfaction, pain, range of movement and strength, and the results were graded using the Mayo Elbow Performance Score (MEPS). The mean follow-up was for 33 months (9 to 63). One rotation flap failed si

Surgical treatment of displaced medial epicondyle fractures in adolescent athletes.

Eight adolescent athletes (average age, 11 years; range, 9 to 15) underwent open reduction and internal fixation of acute, displaced medial epicondyle fractures. Fixation was achieved with a screw and washer. Four patients (50%) had associated elbow dislocations. Elbow motion in a brace was initiated 4 days after surgery. The brace allowed full flexion and extension but protected the elbow against valgus stress. Bracing was continued for 4 weeks. The average duration of followup was 10 months (range, 6 to 13). All fractures united, and full motion was achieved in seven patients. One patient lo

Joint-dependent response to impact and implications for post-traumatic osteoarthritis.

OBJECTIVE: The prevalence of osteoarthritis (OA) varies between joints. Cartilage in eight different joints was evaluated to elucidate the disparate susceptibilities between joints to post-traumatic OA (PTOA) and provide evidence for joint-specific clinical treatments. The hypothesis was that cartilage in different joints would have varying cell death and anabolic gene expression profiles after injury. METHODS: Adult equine cartilage explants were harvested from shoulder (SH), elbow (EL), carpal (CA), metacarpophalangeal (MC), patellofemoral (FP), tarsal (TA), metatarsophalangeal (MT), and pr

The effect of axillary crutch length on upper limb kinematics during swing-through gait.

BACKGROUND: Axillary crutches are commonly used in rehabilitation. Inappropriately fit crutches may result in upper limb pain or injury. OBJECTIVE: To investigate the effects of axillary crutch length on upper limb kinematics to better understand potential injury mechanisms. It was hypothesized that crutches that were longer or shorter than standard-fit crutches would alter upper limb kinematics. DESIGN: Cross-sectional. SETTING: Gait laboratory. PARTICIPANTS: Fifteen healthy males with no prior crutch experience. INTERVENTIONS: Participants were fit with axillary crutches using standardi

Outcomes of neonatal brachial plexus reconstruction using the supraclavicular nerve as a donor graft.

OBJECTIVE: Interposition nerve grafting is widely used for reconstructing neonatal brachial plexus palsy (NBPP). While the sural nerve graft has historically been the gold standard, the supraclavicular nerve offers the advantage of plentiful donor nerve graft within the operative field. This study reports the outcomes of using supraclavicular nerve grafts for NBPP reconstruction. METHODS: A retrospective review was conducted on the medical records of patients with NBPP who underwent supraclavicular nerve grafting at the authors' institution between January 2011 and January 2023. Patients with

A multimodal spatio-temporal graph neural network framework for fatigue detection in tennis serving.

BACKGROUND: The tennis serve is a complex, high-velocity motion dependent on the efficient transfer of energy through the kinetic chain, from the lower extremities to the racquet. Fatigue-induced alterations in this chain are primary precursors to overuse injuries, such as rotator cuff tendinopathy and elbow medial collateral ligament stress. However, current wearable monitoring solutions predominantly rely on unimodal kinematic data, failing to capture the neuromuscular compensatory mechanisms that precede mechanical performance degradation. METHODS: This study introduces a novel multimodal
