OBJECTIVE: Based on the anatomical study and clinical trial of transposition of the flexor pollicis brevis muscle (M. f. p.b.) for the reconstruction of thumb opposition, we suggested a new method for the treatment of irreparable median nerve injury which causes paralysis of the opponens pollicis and the abductor pollicis brevis muscles (M.a.p.b) and leads to loss of thumb opposition.
MATERIAL AND METHODS: Anatomical study and biomechanic analysis were performed on 20 cadaveric hands and 8 patients who had been treated and followed up on an average for 12 months.

Interfascicular nerve autografting is a useful method to repair nerves with gaps, mainly if the removal of a nerve does not imply a defect to the donor area. Final results of peripheral nerve repair definitely depend on the size of the nerve gap, the level of the injury and the time elapsed from the time of injury until the nerve is repaired.

Ten patients with true aneurysmal disease of the hand and forearm vessels were treated at our institution between 1981 and 1990. Pseudoaneurysms resulting from penetrating trauma or mycotic aneurysms were specifically excluded. Eight patients were male, two were female; mean patient age was 38 years (range 26 to 72 years). We report a case of a traumatically-induced embolus from a thrombosed PTFE vascular access graft in a patient who had undergone a successful cadaveric renal transplant. Although usually thought to be without subsequent sequelae, thrombosed PTFE grafts can be a source of emboli to the hand in active young patients. Diagnosis, etiology, and management are discussed. Therapeutic intervention includes exclusion of the graft from the arterial circulation to the hand and removal of all existing thrombus.

A case of penetrating metal fragment injury to the cubital fossa with subsequent development of a false aneurysm of the brachial artery with a fistulae to the vena comitans and median basilic vein is described. Surgical management involved aneurysmectomy, interposition cephalic vein graft and lateral repair of the vena comitans. Early angiography should be considered in penetrating injuries near major limb vessels, as the initial signs and symptoms of vascular injury may be minimal.

Serum parathyroid hormone (PTH) levels in 6 patients who had received parathyroid tissue autografts were studied. Samples were taken from graft and non-graft arms both pre- and post-dialysis. Each sample was analysed using two PTH assays, one measuring C-terminal PTH and the other measuring the intact PTH molecule. For both pre- and post-dialysis samples an appreciably greater difference was seen between the graft and non-graft arms using the intact assay. Multiple level amputation has been described as a contraindication to replantation. This is a case report of a patient with a double level amputation through the palm and forearm that was successfully replanted. Because of the need for multiple stages of reconstructive surgery, it was not clear until the end result that the replantation was worthwhile. Abnormalities of division of the median nerve are exceptional. Several cases are found in the literature. The authors report a case of high bilateral division of the median nerve into two trunks of identical volume in a patient with Hansen's disease. The external trunk of the division supplies the thenar branch and the first commissure and the internal trunk supplies the second and third commissures.

A case of fasciitis with eosinophilia and hypergammaglobulinaemia (Shulman's syndrome) in a 33-year-old patient is described. Both the lower arms and the lower legs were affected. The diagnosis was established by biopsy which revealed cellular infiltration of the deep fascia with thickening. In addition to typical symptoms there was hepatosplenomegaly. Gastrointestinal or haematological disease was excluded. Under systemic corticosteroid administration all symptoms of fasciitis quickly disappeared.
