Clinical antiarrhythmic efficacy of mexiletine: a review.

This paper consists of a review of some of the most important papers which have recently been devoted to mexiletine. The electrophysiological effects of the drug in normal subjects and in patients with abnormal impulse formation or conduction are described. The results of open studies related to the clinical effects of mexiletine administered intravenously or orally are briefly reported. The efficacy of the drug on ventricular rhythm disorders is then discussed on the basis of data provided by several controlled double-blind studies. The possible role of mexiletine as a protection against sudd

Wolff-Parkinson-White syndrome in the neonate.

Of 16 infants who presented with paroxysmal supraventricular tachycardia in the neonatal period, 50 percent had an electrocardiographic pattern consistent with Wolff-Parkinson-White conduction, type A. It is suggested that infants have bypass pathways similar to or identical with a Kent pathway as part of normal maturation. Infants with paroxysmal supraventricular tachycardia have electrically active bypass tracts but these are documented in only about one half of the patients because of the short duration of recordings or because of concealment (the bypass tract conducts only in retrograde fa

Sensor detection and pacing for impending reflex syncope.

AIMS: Reflex syncope occurs infrequently but may benefit from pacing intervention. However, reflex syncope poses special problems for successful cardiac pacing. Between episodes, monitoring by the implanted device is required to provide warning of an impending syncope and initiate pacing therapy if appropriate. This article aims to review available devices and their function in different forms of reflex syncope. METHODS AND RESULTS: The first device dedicated to address detection of VVS was the Rate Drop Response (RDR) (Medtronic Inc., MN, USA). It used a modified rate hysteresis approach to

SGLT2 inhibitors in type 2 diabetes: a comprehensive review of their effects on the autonomic nervous system and cardiovascular health.

Sodium-glucose cotransporter 2 inhibitors (SGLT2i) are antidiabetic drugs that help lower high blood sugar levels by blocking the reabsorption of glucose in the kidneys. Although their primary function is to control blood sugar in type 2 diabetes mellitus (T2DM), growing evidence suggests they may also have additional benefits, particularly in reducing neurological and cardiovascular complications related to T2DM. This study explores the neuroprotective effects of SGLT2i, which appear to improve symptoms of peripheral neuropathy by enhancing nerve conduction for both sensory and motor function

Wolff-Parkinson-White syndrome: a masquerading clinical condition in an 8-year-old Nigerian girl.

BACKGROUND: Wolff-Parkinson-White (WPW) syndrome is a congenital abnormality of the cardiac conduction system characterized by the presence of an accessory pathway, which can predispose affected individuals to supraventricular tachycardia (SVT), atrial fibrillation, ventricular fibrillation, and sudden cardiac death. Despite its clinical significance, WPW syndrome is often underdiagnosed, particularly in resource-limited settings where cardiac arrhythmias may be misattributed to other conditions. CASE PRESENTATION: We report an eight-year-old Nigerian girl with WPW syndrome who was repeatedly

AV-optimized conduction system pacing for treatment of AV dromotropathy: A randomized, cross-over study.

BACKGROUND: Severe first-degree atrioventricular (AV) block may produce symptoms similar to heart failure due to AV dyssynchrony, a syndrome termed AV dromotropathy. According to guidelines, it should be considered for permanent pacemaker implantation, yet evidence supporting this treatment is scarce. OBJECTIVES: This study aimed to determine the impact of AV-optimized conduction system pacing (CSP) in patients with symptomatic severe first-degree AV block and echocardiographic signs of AV dyssynchrony. METHODS: Patients with symptomatic first-degree AV block (PR > 250 ms), preserved left ve

Distinctive Deposition Patterns of Sporadic Transthyretin-Derived Amyloidosis in the Atria: A Forensic Autopsy-Based Study.

Left-to-right differences in the histopathologic patterns of transthyretin-derived amyloid (ATTR) deposition in the atria of older adults have not yet been investigated. Hence, this study evaluated heart specimens from 325 serial autopsy subjects. The amount of ATTR deposits in the seven cardiac regions, including both sides of atria and atrial appendages, was evaluated semiquantitatively. Using digital pathology, we quantitatively evaluated the immunohistochemical deposition burden of ATTR in the myocardium. We identified 20 sporadic ATTR cardiac amyloidosis cases (nine males). All patients h

KCNH2 mutation c.3099_3112del causes congenital long QT syndrome type 2 with gender differences.

INTRODUCTION: Long QT Syndrome (LQTS) is an inherited disease with an abnormal electrical conduction system in the heart that can cause sudden death as a result of QT prolongation. LQT2 is the second most common subtype of LQTS caused by loss of function mutations in the potassium voltage-gated channel subfamily H member 2 (KCNH2) gene. Although more than 900 mutations are associated with the LQTS, many of these mutations are not validated or characterized. METHODS AND RESULTS: Sequencing analyses of genomic DNA of a family with LQT2 identified a putative mutation. i.e., KCNH2(NM_000238.3): c

Intracellular to Interorgan Mitochondrial Communication in Striated Muscle in Health and Disease.

Mitochondria sense both biochemical and energetic input in addition to communicating signals regarding the energetic state of the cell. Increasingly, these signaling organelles are recognized as key for regulating different cell functions. This review summarizes recent advances in mitochondrial communication in striated muscle, with specific focus on the processes by which mitochondria communicate with each other, other organelles, and across distant organ systems. Intermitochondrial communication in striated muscle is mediated via conduction of the mitochondrial membrane potential to adjacent

Assessment of palpitation complaints in patients with benign paroxysmal positional vertigo.

OBJECTIVE: The detection of cardiac arrhythmias during vertigo attacks along with the resolution of vertigo by treating the arrhythmias is defined as cardiac vertigo. The cause of palpitations that develop following attacks of non-cardiac vertigo is unknown. We aimed to investigate the effect of benign paroxysmal positional vertigo (BPPV) on the cardiac conduction system and evaluate patients' palpitation complaints using 24-hour ambulatory ECG (Holter) monitoring. PATIENTS AND METHODS: Sixty-five patients with BPPV and forty-eight healthy controls were included in the study. ECG was performe

Etiology of tricuspid valve disease is a predictor of bradyarrhythmia after tricuspid valve surgery.

AIMS: The tricuspid valve is situated in close proximity to cardiac conduction tissue and damage to this tissue can affect postoperative rhythm. The aim of this study was to quantify the incidence of pacemaker requirement after tricuspid valve surgery and investigate predictors. METHODS: Data were collected via our operative data collection system and patient files. All patients who underwent surgical procedures of the tricuspid valve from 2004 until 2017 and lacked a pacemaker preoperatively were included in the study. RESULTS: In our cohort of 505 patients 54 required a pacemaker in the fi

Potential New Agents for the Management of Hyperkalemia.

Hyperkalemia is a common electrolyte disturbance with multiple potential etiologies. It is usually observed in the setting of reduced renal function. Mild to moderate hyperkalemia is usually asymptomatic, but is associated with poor prognosis. When severe, hyperkalemia may cause serious acute cardiac arrhythmias and conduction abnormalities, and may result in sudden death. The rising prevalence of conditions associated with hyperkalemia (heart failure, chronic kidney disease, and diabetes) and broad use of renin-angiotensin-aldosterone system (RAAS) inhibitors and mineralocorticoid receptor an

[Present trends and controversies in the use of beta-blockers in cardiovascular diseases].

In this paper we summarize present trends and controversies in the use of beta-blockers in cardiovascular diseases. Beta-blockers are catecolamine competitive inhibitors and act through alpha and beta adrenergic receptors blockade. Different agents have a dose-dependent affinity for different beta adrenergic receptors (beta 1, beta 2, beta 3) which is less with higher doses. The most important therapeutic effects of beta-blockers are on cardiovascular system, where they act as negative chronotropic and inotropic agents, lowering cardiac work and improving oxygen demand /supply ratio. Clinical
