Expeditious femoral nailing prior to vascular repair in fractures associated with vascular injury: A series of four cases.

CASE: Femoral shaft fractures with concomitant vascular injury requiring limb revascularization, although rare, are a limb-threatening condition. Historically, emergent external fixation of the femur fracture followed by vascular repair has been considered the standard of care. We discuss four cases of femoral fracture with an associated vascular injury amenable to nail fixation stabilized by expeditious intramedullary nailing (IMN), followed by limb revascularization. We discuss the timeline and duration of the procedure for this technique. CONCLUSION: Expeditious femoral IMN prior to limb r

Mechanical study of the safe distance between humerus shaft fracture and distal locking screws in antegrade nailing.

BACKGROUND: Optimal positioning of distal locking screws in intramedullary humeral nailing remains uncertain, particularly the influence of the distance between the fracture plane and the proximal distal locking screw on construct stability. This study aims to evaluate the mechanical stability of humeral nailing under different fracture-to-screw distances and numbers of distal locking screws using finite element analysis and mechanical testing on bone models. METHODS: A finite element model and mechanical testing on six sawbones models were performed under traction (500 N), compression (500 N

Combination of radiofrequency ablation and intramedullary nailing for the treatment of femoral metastases: single-center, retrospective observational study.

BACKGROUND: Radiofrequency ablation (RFA) has gained attention as a palliative treatment for bone metastases, providing pain relief and local tumor control. While its use for axial lesions is well documented, its application in long bones remains limited due to concerns about post-ablation fractures. These risks may be mitigated through the combination of RFA with prophylactic intramedullary nailing (IMN). METHODS: Five consecutive patients with femoral metastases who underwent combined RFA and IMN, performed either as a single-stage procedure under intraoperative fluoroscopic guidance or as

There is no difference in prevalence of rotation malalignment following intramedullary nailing of open versus closed tibial shaft fractures.

INTRODUCTION: Tibial shaft fractures are the most prevalent long-bone fracture and often associated with high energy mechanisms. As a result, 12-15% are seen to been open fractures making up 45% of open fractures. Open fractures are commonly associated with higher complication rates such as infection, and non-union. Intramedullary nails (IMN) are an accepted treatment modality open tibial shaft fractures; however, IMNs are known to be associated with potential rotational malalignment. We hypothesis that an open tibial shaft fracture may in fact be protective against post operative rotational m…

Management of Impending and/or Pathologic Fractures of the Proximal Radius in the Setting of Metastatic Bone Disease With an Intramedullary Photodynamic Nail.

Metastatic bone disease of the radius is rare, but the functional loss of an upper extremity can profoundly impact patient quality of life and hinder independent living. When the mechanical stability of the radius is compromised, surgical intervention such as intramedullary (IM) nailing or plating may be considered in combination with radiation therapy (RT) to alleviate pain, control local tumor progression, and provide immediate stability to the bone. There are several limitations to traditional metallic IM nails, such as limited use in adults, complicated postoperative CT planning for RT, th…

Retrograde Intramedullary Nailing for Distal Femur Fractures: A Prospective Study of Functional Outcomes, Complications, and Union Rates.

Background Distal third femur fractures, especially those with intra-articular extension, are challenging to manage and often lead to complications. Retrograde intramedullary nailing (RIN) has emerged as a minimally invasive alternative to traditional plating, offering advantages such as reduced soft tissue trauma, lower infection rates, and faster healing. This study aimed to evaluate the efficacy of RIN in managing distal femur fractures with associated soft tissue damage. Methods A prospective analysis was conducted involving 39 patients with distal femur fractures treated with retrograde n

Intramedullary Fibular Nail Fixation in an Elderly Patient with a Comminuted Weber C Ankle Fracture: A Case Report.

INTRODUCTION: Ankle fractures are among the most common lower extremity injuries, accounting for approximately 9% of all fractures. Various methods exist for the internal fixation of fibular fractures, with open reduction and internal fixation using plates and screws currently regarded as the gold standard. However, plate and screw fixation is highly invasive and may lead to significant post-operative complications. Intramedullary (IM) nails, offering a less invasive alternative, have conventionally been indicated for non-comminuted Weber B and C fractures. To our knowledge, this is one of the…

A case of concurrent Alport syndrome and Nail-patella syndrome posing diagnostic challenge without genetic testing.

Hereditary glomerular basement membrane disease is a group of conditions caused by genetic mutations in the development and maintenance of the glomerular basement membrane. Alport syndrome is a classic example caused by variants in the genes COL4A3, COL4A4, or COL4A5. Less common example includes nail-patella syndrome (LMX1B-associated nephropathy), which is caused by variants in the LMX1B gene. The manifestations of LMX1B-associated nephropathy and Alport syndrome can overlap because they share abnormalities in type IV collagen, and this can sometimes cause diagnostic challenges. We describe

Surgical treatment of distal tibia non-unions with a retrograde intramedullary nail as a therapy option to prevent amputation.

Compared to other long bones, non-unions of the tibia after fractures are more likely. Due to surgical debridement, the residual bone stock in the distal part of the bone may be insufficient for adequate fixation of an implant. Thus, treatment with a retrograde nail with the additional performance of an arthrodesis of the hindfoot may be an alternative. The aim of this study was to examine the consolidation and reinfection rates of non-unions of the distal tibia treated with a retrograde arthrodesis nail. A total of 27 patients between 2010 and 2018 were included according to our inclusion cri

Fixation of Pubic and Acetabular Pathologic Fracture with Curvafix Nail and Concurrent Total Hip Arthroplasty: A Case Report.

INTRODUCTION: The main way that the skin produces Vitamin D, which is necessary for calcium metabolism and skeletal health, is through exposure to ultraviolet B rays. Long-term skin disorders such as ichthyosis can prevent the skin from producing enough Vitamin D, leading to deficiencies and issues with bone health. This report highlights the need to take Vitamin D insufficiency and its sequelae into account in clinical care by presenting a case of genu valgum in a patient with lamellar ichthyosis. Similar correlations have also been shown in earlier research, underscoring the necessity of con

Cell-free DNA from nail clippings as source of normal control for genomic studies in hematologic malignancies.

Comprehensive genomic sequencing is becoming a critical component in the assessment of hematologic malignancies, with broad implications for … ated with a concurrent/evolving clonal myeloid neoplasm. Donor DNA was identified in 22% (11/50) of nails collected after allogeneic stem-cell transplantation. In this cohort, an association with a recent history of graft-versus-host disease was identified. These findings should be considered as a potential limitation to the use of nails as a source of …

Nail Telocytes: Identification, Potential Physiological Function, and Role in Pathology: A Reappraisal of the So-Called Onychofibroblasts/Onychodermis.

Some authors have suggested that the fibroblasts of the nail mesenchyme (onychofibroblasts) can be distinguished from skin fibroblasts by their high expression of CD10. My 2015 study documented the presence of a relatively sparse CD34 + /CD10 + dendritic subpopulation in the dermis and hypodermis of the matrix. For some time now, my hypothesis has been that these interstitial dendritic mesenchymal cells of the matrix correspond to telocytes. Telocytes have been described as peculiar interstitial dendritic cells present in the mesenchymal tissue of numerous organs, including the skin, but their
