Orbital septum attachment sites on the levator aponeurosis in Asians and whites.

PURPOSE: To characterize the attachment site of the orbital septum on the levator aponeurosis and the anatomic level of the inferior end of the preaponeurotic fat pad, both in Asians and whites. MATERIALS AND METHODS: Full-thickness sagittal sections of upper eyelids, fixed in 10% buffered formalin, were microscopically examined in 18 postmortem upper eyelids of 10 Asian cadavers (mean death age, 78 years; age range, 68-89 years) and 11 postmortem upper eyelids of 7 whites (mean death age, 88 years; age range, 78-101 years). We measured the distance from the superior tarsal border to the atta…

Diffuse large B-cell lymphoma masquerading as orbital cellulitis.

Proptosis is commonly encountered in clinical practice. We report on a patient with acute proptosis, eyelid swelling, and chemosis, which was initially treated as cellulitis. After radiological and pathological assessments, a diagnosis of large B-cell lymphoma was made. The patient died within 2 months of presentation. Not all patients with proptosis have cellulitis. Proptosis, especially unilateral proptosis, should always lead to a radiological scan in case of malignancy.

Effect of smoking on orbital fat and muscle volume in Graves' orbitopathy.

BACKGROUND: Smoking adversely affects the course and severity of Graves' orbitopathy (GO). Cigarette smoke enhances adipogenesis in cultured human orbital fibroblasts. The present study tested our hypothesis that smoking is associated with increased orbital fat in GO patients. METHODS: This was an observational case series study. In 95 consecutive patients with untreated GO, the ratios of fat volume/orbital volume (FV/OV) and muscle volume/OV (MV/OV) were calculated with validated software. The most affected orbit of each patient was chosen for analysis. Patients were divided into two groups …

Uncommon presentation of an intradiploic orbital epidermoid tumor: case report.

PURPOSE: Epidermoid tumors are uncommon entities that usually occur intradurally in the cerebellopontine angle and parasellar cisterns. The few cases that have been reported to occur in the intraorbital location involve patients in the first four decades of life. To our knowledge, an elderly person presenting with an orbital epidermoid tumor has never been reported. METHODS: A 67-year-old man presented with left-sided headache and eye pain. He was found to have a hemorrhagic lesion eroding bone of the greater and lesser wings of the sphenoid that was compressing the frontal and temporal lobes…

[Orbital tumors in children: clinical examination, imaging, specific progression].

"Orbital tumors (OT) are neoplasms of the bony orbit and contents except for the eyeball." Given this definition, we exclude the retinoblastomas, which are the most frequent tumors of this anatomical area in the pediatric population. Although these tumors are rare, there is a great variety of etiologies. Among them, the most frequent OTs in childhood are rhabdomyosarcomas and metastatic tumors: metastatic neuroblastomas and leukemias (chloromas). Moreover, adult and pediatric varieties of OT are made up of two clusters of distinctive histological entities. Examination must be careful: measurem…

Corneal graft dehiscence during swinging eyelid orbital decompression.

A 68-year-old man with Graves orbitopathy underwent a bilateral swinging eyelid orbital decompression for disfiguring proptosis. A penetrating keratoplasty for keratoconus of the left eye had been performed 13 years earlier. Prior to orbital decompression, biomicroscopy of the left eye showed a partially decompensated sutureless corneal graft with good wound apposition. The corneal stroma appeared edematous, and some epithelial bullae were seen. During orbital wall removal, the corneal wound dehisced for 270 degrees. Swinging eyelid orbital decompression is the technique of choice in the Nethe…

Orbital and periorbital vascular anomalies--an approach to diagnosis and therapeutic concepts.

CONCLUSION: A correct and universally accepted terminology based on the biologic behavior of orbital vascular anomalies is essential to avoid misleading diagnoses, to choose an adequate therapy regimen, and to compare different therapeutic approaches. The management of these diseases is multi-modal and an interdisciplinary challenge. BACKGROUND: There is persisting terminological and clinical confusion about vascular anomalies involving the orbit. Review of the literature reveals a diverse or misleading terminology and a lack of consensus for the treatment of vascular malformations or vascula…

A yellowish subconjunctival lesion in an infant-congenital orbital fat herniation.

A very rare case of a congenital orbital fat herniation is demonstrated. Clinically, the child presented a progressive, small yellowish and mobile mass on the temporal orbital wall of her right eye. Magnetic resonance imaging showed a fat isointensive structure in T1-weighing without contrast enhancement. The possible aetiology of the herniation could be a congenital weakness of the Tenon's capsule or the conjunctival fornix.

[A case of inadequate surgery tactics in the use of an unstandard orbital implant].

By analyzing a clinical case with inadequate surgery tactics in the use of an unstandard orbital implant, the authors discuss possible errors in surgical techniques and in the choice of an implant. Furthermore, by analyzing problems in the use of various implants, including those unstandard ones, they discuss possible causes of complications, consider the optimal time for resurgeries, and propose the ways of solving the problems associated with the occurrence of complications, including treatment policy.

Fabrication of a surgical template for orbital implant placement: a case report.

Implant placement in orbital sites is challenging because of limited bone volume and poor bone quality. Preoperative optimal implant planning and intraoperative guides are important to ensure success. This article describes the use of computer simulation for implant insertion and fabrication of a surgical template as a drilling guide for orbital implant placement. A 29-year-old woman with a right orbital defect underwent computerized tomographic scanning, and the data were processed with interactive software to simulate orbital implant placement. A surgical template that served as a drilling g…

Age- and sex-related changes in the soft tissues of the orbital region.

The orbital region plays a predominant role in the evaluation of the craniofacial complex. In the current study information about normal sex-related dimensions of the orbital region, and growth, development and aging, were provided. The three-dimensional coordinates of several soft-tissue landmarks on the orbits and face were obtained by a non-invasive, computerized electromagnetic digitizer in 531 male and 357 female healthy subjects aged 4-73 years. From the landmarks, biocular and intercanthal widths, paired height and inclination of the orbit relative to both the true horizontal (head in n…

Idiopathic sclerosing orbital inflammation: two cases presenting with paresthesia.

The authors report 2 patients with idiopathic sclerosing inflammation of the orbit who presented with periorbital paresthesia in the trigeminal nerve distribution. The diagnosis in both cases was confirmed with biopsy and both patients responded to corticosteroid treatment. Although periorbital paresthesia is usually a sign of malignancy, these cases illustrate that it may also occur in patients with sclerosing orbital inflammation.

Adult orbital langerhans cell histiocytosis with frontal bone involvement.

A 28-year-old woman presented with a 2-week history of right upper eyelid swelling and intermittent frontal headaches. CT demonstrated an ill-defined superior right orbital mass with adjacent right frontal bone erosion and undeveloped frontal sinuses. The orbital biopsy revealed tissue strongly positive for CD1a and S100, diagnostic of Langerhans cell histiocytosis. The systemic workup was negative for multifocal lesions and for diabetes insipidus. In addition to subtotal resection, the patient was treated with a 6-month course of oral prednisone and intravenous vinblastine.
