Acute Subcutaneous Gastric Perforation at a Previous Percutaneous Endoscopic Gastrostomy (PEG) Site.

Percutaneous endoscopic gastrostomy (PEG) is commonly used to provide long-term enteral nutrition and is generally associated with a low rate of major complications. Persistent gastrocutaneous fistula after PEG removal occurs in approximately 0.5-3.9% of cases. We report an unusual case of delayed subcutaneous gastric perforation into the subcutaneous space presenting as abdominal wall sepsis six years after PEG removal without prior history of PEG-related complications. A 24-year-old man with a history of repaired congenital tracheoesophageal fistula and long-term PEG dependence presented wit

Orthodontic, regenerative, and implant management of a mandibular anterior defect: 5-year outcome.

BACKGROUND: Severe alveolar ridge defects in the mandibular anterior region, often associated with congenital tooth absence and subsequent loss of retained primary teeth, present significant challenges for implant rehabilitation. Limited space, horizontal ridge deficiency, and complex site anatomy often require a multidisciplinary approach to achieve stable hard- and soft-tissue outcomes. METHODS: A 30-year-old woman presented with absence of teeth #23 and #25, distal rotation of tooth #24, and insufficient mesiodistal space for implant placement. After 15 months of orthodontic space creation

[Application of sample-efficient neural network in predicting postoperative facial profiles in skeletal class Ⅱ orthognathic patients].

OBJECTIVE: Soft tissue prediction is of great importance for the surgical design and patients ' satisfaction in orthognathic surgery. Previous studies either rely on elastic mechanical models, such as finite element which involves time-consuming computations, or using artificial intelligence method which require insatiable large patient data. Therefore, in this study, we proposed a more efficient and lightweight method for facial morphology prediction and validated its accuracy. METHODS: A total of 85 skeletal class Ⅱ patients who underwent double jaw surgery performed by the same senior surg

Development, validation, and biomechanical evaluation of a high-fidelity finite element model of the forearm musculoskeletal system.

BACKGROUND: The human forearm, a dynamically coordinated system of bones, muscles, and connective tissues, is central to upper limb function but poses significant challenges for computational modeling due to its anatomical complexity. Existing finite element (FE) models of the forearm often oversimplify soft tissues or lack rigorous validation, limiting their clinical utility. This study aimed to develop and validate a high-fidelity 3D FE model of the forearm musculoskeletal system to replicate in vivo stress distribution under axial loading. METHODS: The model was constructed using CT and MRI

Diagnostic Imaging Pathways in Peripheral Artery Disease: A Comparative Review of Duplex Ultrasound Through Computed Tomography Angiography.

Peripheral artery disease (PAD) is a major global health challenge, affecting more than 200 million people worldwide and an estimated 8 to 12 million in the United States. Its clinical spectrum ranges from intermittent claudication to chronic limb-threatening ischemia and acute limb ischemia. Accurate vascular imaging is essential for diagnosis, risk stratification, treatment planning, and postprocedural surveillance, guiding management from conservative therapy to endovascular or surgical revascularization. In this review, we aim to summarize the contemporary imaging pathways in PAD, focusing

In-silico prediction of multi‑target mechanisms of Pinellia ternata phytochemicals in lung cancer: Evidence from a graph‑attention‑guided virtual screening and multi‑scale simulations.

Pinellia ternate has long been used to treat respiratory diseases, possessing potential anti-tumor activity and exhibiting multi-component, multi-target characteristics. This study prioritized lung cancer-related targets using the HERBGAT framework based on graph attention networks (GAT). High-quality PDB structures were retrieved, and diffusion-generative docking was performed to construct complex conformations and assess their confidence levels. Molecular dynamics simulations of representative complexes were conducted over 200 ns, and binding free energies were estimated using the MM/PBSA me

Computed Tomographic Angiography (CTA)-guided precision reconstruction in complex lower leg wounds.

BACKGROUND: Reconstruction of complex lower leg wounds presents significant challenges due to extensive soft tissue damage, vascular variations, and potential neurovascular injuries. While flap transfer, particularly free flaps, remains the preferred method for reconstruction, the lack of precise preoperative vascular information often complicates surgical planning. This study introduces a Computed Tomographic Angiography (CTA)-guided approach for precision reconstruction to address these challenges. PURPOSE: This study aims to elucidate the critical role of precise preoperative assessment of

Artifact suppression for breast specimen imaging in micro CBCT using deep learning.

BACKGROUND: Cone-beam computed tomography (CBCT) has been introduced for breast-specimen imaging to identify a free resection margin of abnormal tissues in breast conservation. As well-known, typical micro CT consumes long acquisition and computation times. One simple solution to reduce the acquisition scan time is to decrease of the number of projections, but this method generates streak artifacts on breast specimen images. Furthermore, the presence of a metallic-needle marker on a breast specimen causes metal artifacts that are prominently visible in the images. In this work, we propose a de

Up-to-Date Imaging for Parathyroid Tumor Localization in MEN1 Patients with Primary Hyperparathyroidism: When and Which Ones (A Narrative Pictorial Review).

Patients diagnosed with multiple endocrine neoplasia type-1 (MEN1) often initially present with primary hyperparathyroidism (pHPT), and typically undergo surgical intervention. While laboratory tests are fundamental for diagnosis, imaging is crucial for localizing pathological parathyroids to aid in precise surgical planning. In this pictorial review, we will begin by comprehensively examining key imaging techniques and their established protocols, evaluating their effectiveness in detecting abnormal parathyroid glands. This analysis will emphasize both the advantages and potential limitations

A review on the added value of whole-body MRI in metastatic lobular breast cancer.

Invasive lobular breast carcinomas (ILC) account for approximately 15% of breast cancer diagnoses. They can be difficult to diagnose both clinically and radiologically, due to their infiltrative growth pattern. The pattern of metastasis of ILC is unusual, with spread to the serosal surfaces (pleura and peritoneum), retroperitoneum and gastrointestinal (GI)/genitourinary (GU) tracts and a higher rate of leptomeningeal spread than IDC. Routine staging and response assessment with computed tomography (CT) can be undertaken quickly and measurements can be reproduced easily, but this is challenging

The impact of autologous concentrated growth factors on the alveolar ridge preservation after posterior tooth extraction: A prospective, randomized controlled clinical trial.

BACKGROUND: Alveolar ridge preservation can effectively decrease alveolar ridge resorption following tooth extraction, but it can be limited by reducing new bone formation and residual bone graft material. Efforts to develop more efficacious approaches are thus an area of active research. PURPOSE: To assess the impact of autologous concentrated growth factors (CGF) on alveolar ridge absorption and osteogenesis following posterior tooth extraction. MATERIALS AND METHODS: Fifty patients were randomly assigned to have extraction sockets treated with CGF or no treatment. At 10 days, 1 month, and

Simulations of inorganic-bioorganic interfaces to discover new materials: insights, comparisons to experiment, challenges, and opportunities.

Natural and man-made materials often rely on functional interfaces between inorganic and organic compounds. Examples include skeletal tissues and biominerals, drug delivery systems, catalysts, sensors, separation media, energy conversion devices, and polymer nanocomposites. Current laboratory techniques are limited to monitor and manipulate assembly on the 1 to 100 nm scale, time-consuming, and costly. Computational methods have become increasingly reliable to understand materials assembly and performance. This review explores the merit of simulations in comparison to experiment at the 1 to 10

Closed reduction and percutaneous Kirschner wire fixation in the treatment of simple zygoma fractures: orbital volumetric outcomes.

INTRODUCTION: The primary goal of treatment of fractures of the zygoma is the perfect restoration of function and aesthetic appearance. An adequate surgical treatment must combine adequate fracture reduction with the lowest possible morbidity of the surrounding soft tissues. This is the principle that guides the use of closed method for the treatment of simple fractures of the zygomatic bone. For a long-term evaluation, it is necessary to develop a method of volumetric orbitometry using tomography and use it for the evaluation of recent and late outcomes of patients treated with the closed tec
