Communicating bronchopulmonary foregut malformation associated with esophageal atresia and tracheo-esophageal fistula.

We report on two neonates who had esophageal atresia with a distal tracheo-esophageal fistula (type III) and a second fistula between the distal esophagus and an intralobular sequestrated lung segment. In both patients the bronchopulmonary foregut malformation became evident only when the entire esophagus was in continuity following primary repair and a contrast swallow was performed to evaluate the anastomotic site.

A gene for hypospadias in a child with presumed tetrasomy 18p.

The proband and his elder brother had intrauterine growth retardation, hypospadias, cryptorchism, a high palate, distally placed axial triradii, and a functional and maturational CNS defect that improved with age and included the inability to suck, severe swallowing difficulties, and frequent vomiting. Their hypospadias is due to an autosomal dominant gene. The proband also had a small extra metacentric chromosome presumed to be an isochromosome 18p.

Brainstem and vagal dysfunction in pharyngoesophageal and esophageal dysphagia.

Dysphagia affects approximately 16% of the US population and is classified as pharyngoesophageal and esophageal, the former being more clinically urgent because loss of airway protection may cause pulmonary aspiration and death. Safe swallowing depends on a brainstem central pattern generator, modulated by cortical and subcortical input and executed through the cranial nerves, above all the vagus. The latter supplies the pharynx, larynx, upper esophageal sphincter, esophagus and lower esophageal sphincter. Because afferent and efferent pathways converge in the medulla, small lesions in medulla

Oral Environment and Aspiration Pneumonia Risk in Parkinson's Disease: A Narrative Review and Conceptual Framework.

BACKGROUND: Aspiration pneumonia is a major clinical concern in Parkinson's disease (PD) and is associated with poor prognosis. Dysphagia is a well-recognized risk factor for aspiration pneumonia, but the oral environment may also contribute to this risk. OBJECTIVE: This narrative review aims to synthesize knowledge on the relationship between the oral environment and dysphagia in PD and to propose a conceptual framework that incorporates oral environment management into dysphagia management. METHODS: We searched PubMed, Scopus and Google Scholar for publications from January 2000 to Decembe

Low Dose Botulinum Toxin Injection in Retrograde Cricopharyngeal Dysfunction.

Retrograde cricopharyngeal dysfunction or R-CPD has a variety of symptoms including the inability to belch. Often, physical examination and additional testing reveal no abnormalities. Botulinum toxin injections into the cricopharyngeal muscle is the most described treatment in patients with R-CPD. Dosages range from 10 to 200 units, injected in one or several sites of the cricopharyngeal muscle. This study is a retrospective case series study of 3 consecutive patients referred for botulinum toxin injection in the cricopharyngeal muscle after diagnostic work-up for inability to belch. The dose

Effect of ozone-induced aleurone-derived dietary fiber modification on the structure properties of aleurone-rice-gel.

This study aims to develop high-quality whole-grain rice products for diverse swallowing needs. Accordingly, the effects and mechanisms of aleurone-derived dietary fiber modification on the gel structure, starch crystal structure and rheological properties of aleurone-rice-gel (ARG) under varying ozone treatment times (0-120 min) were investigated. The results suggest that ozone enhances gel stability by strengthening hydrogen bonding and improving the solubility of dietary fiber. Specifically, ozone oxidation significantly increased both the soluble dietary fiber content (6.05 ± 0.12 g/100 g)

Sigmoid colon perforation caused by ingested removable dentures: A case report and literature review.

RATIONALE: Ingestion of removable dentures leading to gastrointestinal perforation is a rare but potentially life-threatening emergency, particularly in the elderly population due to diminished oral sensation and swallowing reflex. This article reports the management of a patient who developed sigmoid colon necrosis and perforation. PATIENT CONCERNS: A 76-year-old female presented to the emergency department with a 2-day history of nausea and left lower abdominal pain. Medical history collection confirmed that the patient had accidentally swallowed her removable denture 3 days ago. DIAGNOSES

Fusion of the Pharyngeal Wall to the Soft Palate Around the Laryngeal Inlet Isolates the Airway in Odontocete Cetaceans.

In odontocete cetaceans (toothed whales, dolphins, and porpoises), a palatopharyngeal sphincter permanently anchors the laryngeal inlet in the nasopharynx and isolates the airway from the gut tube. Using an MRI data set of a pacific white-sided dolphin (Aethalodelphis obliquidens), together with dissections of the dolphin and two neonatal harbour porpoises (Phocoena phocoena), we show that the sphincter is formed by fusion of part of the superior constrictor of the pharyngeal wall to the soft palate. The part of the superior constrictor that participates in forming the sphincter is brought clo

Vocal fold augmentation with autologous fat following transoral laryngeal surgery.

BACKGROUND: Transoral laryngeal surgery (TOLS) may lead to glottic insufficiency and scarring which often causes pronounced hoarseness and sometimes swallowing impairment. Injection laryngoplasty with autologous fat is an established method to restore glottic competence and thus improve voice and swallowing function. We hereby aim to demonstrate how laryngeal fat injection can be applied following TOLS. METHOD: Autologous fat is harvested via liposuction and then centrifuged for three minutes at 3000 rpm (rpm) to separate fat cells from liquid fat and debris. Fat cells are then transferred to

Longitudinal association between oral function and underweight onset among community-dwelling older adults: Role of regular self-weighing.

AIMS: Regular self-weighing is effective in weight management and may help to mitigate the risk of underweight among older adults. We examined which factors of oral function are associated with a risk for underweight among community-dwelling older adults and whether regular self-weighing can mitigate that risk. METHODS: This was a cohort study. Self-administered questionnaires were mailed to 7665 older adults in March 2019 (response rate 74.3%) and 7591 in March 2020 (response rate 74.7%). Among those who responded to both surveys, 3594 older adults who were not underweight in March 2019 were

Evaluation of the Safety of Percutaneous Sensory Nerve Stimulation in Patients with Head and Neck Cancer Receiving Chemoradiotherapy.

Chemoradiotherapy (CRT) is the standard treatment for locally advanced head and neck cancer; however, CRT may cause post-treatment dysphagia. Transcutaneous electrical sensory stimulation (TESS), developed in recent years for swallowing rehabilitation, is used at many medical facilities. Although TESS has been used for dysphagia in several fields, its safety and efficacy in patients with head and neck cancer remain to be clarified. Therefore, this study evaluated the safety of TESS in ten patients with head and neck cancers undergoing CRT. Swallowing rehabilitation intervention and TESS implem
