A large cartilage lesion of the tibial plateau with a deficient meniscus in a young patient is a challenging pathology for the orthopaedic surgeon due to the limited options available. While hemiarthroplasty procedures can be an option, the risk of revision in a young patient is high, and therefore a reconstructive procedure is advocated. The purpose of this Technical Note is to describe our technique for tibial plateau resurfacing, including the preparation and implantation of a medial tibial plateau and medial meniscal allograft, in a patient with a post-traumatic KD-4 knee dislocation with

Secondary injury following initial spinal cord trauma is uncommon and frequently attributed to mismanagement of an unprotected cord in the acute time period after injury. Subacute posttraumatic ascending myelopathy (SPAM) is a rare occurrence in the days to weeks following an initial spinal cord injury that is unrelated to manipulation of an unprotected cord and involves 4 or more vertebral levels above the original injury. The authors present a case of SPAM occurring in a 15-year-old boy who sustained a T3-4 fracture-dislocation resulting in a complete spinal cord injury, and they highlight t

Blunt injury of the abdominal aorta is highly fatal. We present an unusual case of an osteophyte impaling the abdominal aorta treated by endovascular repair. A 77-year-old man sustained a thoracolumbar fracture-dislocation with posterior aortic rupture between his celiac and superior mesenteric artery origins. His aortic injury was treated with a stent graft, excluding the celiac origin. He was dismissed on postoperative day 6. At 6 months, he had returned to most preinjury activities, and at 2-year follow-up, he continues to have good functional outcome. Endovascular repair may be successfull

The occurrence of tetraplegic spinal cord injuries in two patients who had preexisting cervical spine fusions highlights the vulnerable nature of such patients. One patient sustained a C5-6 fracture-dislocation after a motor vehicle accident, despite a spontaneously fused cervical spine as a consequence of his juvenile rheumatoid arthritis. A second patient sustained a C3-4 distraction injury in a sporting injury 2 years after he had undergone a posterior cervical spine fusion because of an aneurysmal bone cyst. Prior to their spinal cord injuries, both patients had no apparent neurologic defi

We performed total elbow replacement, using the Souter-Strathclyde prosthesis in 19 elbows of 18 patients with rheumatoid arthritis and followed the patients for 5 (1-11) years. Pain relief was achieved initially in all patients. The average range of flexion-extension was increased by 12 degrees and pronation-supination by 40 degrees. There were no infections. 1 patient sustained an intraoperative fracture of the medial epicondyle, 3 patients developed neuropathies and 1 patient had an immediate postoperative dislocation of the joint. At follow-up, 6 prostheses had radiographic loosening, with

Fourteen cases of traumatic basal subarachnoid hemorrhages, examined in the years 1980-1988, were presented. Bleeding was connected with the fracture of the transverse process of the atlas in eleven cases and with atlanto-occipital dislocation in three. The source of hemorrhage--vertebral artery rupture--was determined only four times. In nine cases (63%) atlases showed various developmental malformations (posterior ponticle and foramen arcuale, epitransverse process, incomplete ossification of the transverse process). The results were related to the literature of the subject. Special attentio

Posttraumatic malunion usually results from incorrect treatment of Colles or Smith fracture. Chronic pains, limited motion and impaired power can be avoided and cosmetic appearance of the hand improved by restoration of the functional anatomy of the wrist. The malalignment is corrected by open wedge osteotomy of the distal end of the radius. A small iliae cortico-cancellus bone graft is inserted at the osteotomy site. By using this technique it is usually possible to avoid additional resection of the distal ulna. The surgical approach dorsal or volar is determined by the type of dislocation. T

BACKGROUND: Stability is regarded as an important factor for proper healing and avoiding secondary dislocation in osteoporotic fractures of the proximal humerus. Several surgical techniques have been proposed for treatment, including intramedullary nail osteosynthesis and locking plate fixation. This study introduces a novel approach that combines both techniques and compares its primary stability with conventional methods.
METHODS: The study involved 25 osteoporotic humeri with two-part fracture models, which were randomly assigned to locking-plate fixation, intramedullary nailing, or a comb

We present a minimally invasive direct posterior, gluteal muscle splitting approach (PMS) as an alternative to the traditional Kocher-Langenbeck (KL) approach for posterior access to acetabular fractures. We believe it offers significant advantages and provides improved access while maximizing the range of fracture patterns that can be addressed through a posterior approach. One hundred and eighty-four consecutive patients treated with this approach by the senior author (RMS) between 2001 and 2018 were reviewed. The most common individual fracture pattern addressed was a posterior wall (66/36%

Traumatic events may lead to lumbar spine injuries ranging from low severity bony fracture to complex fracture dislocation. Injury pathomechanisms as well as the influence of loading rate and ligament mechanical properties were not yet fully elucidated. The objective was to quantify the influence of impact velocity and ligament properties variability on the lumbar spine response in traumatic flexion-shear conditions. An L1-L3 finite element spinal segment was submitted to a posterior-anterior impact at three velocities (2.7, 5, or 10 m/s) and for 27 sets of ligament properties. Spinal injury p…

Displaced acetabular fractures should be treated surgically. Over the past decade, surgical approaches to the acetabulum and the surgical technique for repair of common fracture patterns have advanced. Excellent outcomes after repair of these injuries can be achieved. The aim of this study was to assess the medium-term results of reconstruction of acetabular fractures by using shape-memory alloy designed by the authors. This is a retrospective review conducted at a level 1 trauma centre. From October 1999 to July 2009, 19 patients with acetabular fractures were treated with our patented Ni-Ti

This article describes a case of a 23-year-old man who sustained a far medial fracture dislocation of the medial clavicle. We performed an open reduction and internal fixation of the fragment back to the remaining medial fragment using a small locking plate. This fracture can be problematic and usually requires reduction due to the potential vascular or airway compromise. Because the fracture often occurs at the medial physis, it is difficult to secure in place to prevent displacement. Historically, fixation of this fracture has been problematic with sutures and K-wires. Failure of fixation an…

BACKGROUND: In some studies, the direct anterior approach (DAA) for elective total hip arthroplasty (THA) is associated with decreased dislocation and greater functional gains compared to the posterior approach (PA), as well as higher functional outcomes compared to the direct lateral approach (LA) at 2 weeks postoperatively. Given the paucity of literature on femoral neck fracture (FNF), we aspired to determine the association between the surgical approach used in THA and outcomes.
METHODS: We conducted a retrospective review of patients undergoing THA for FNF at 9 institutions from 2010 to

OBJECT: The authors report on a series of 145 consecutive patients with different types of spine lesions surgically treated via an anterior approach (AA) at the thoracic and lumbar levels during the past 10 years. Indications, techniques, and surgical results are described.
METHODS: This series included 92 patients with fractures, 30 with neoplasms, 13 with thoracic disc hernias, and 10 with spinal infections. Based on the lesion to be addressed, the AA was used for lesion excision, corpectomy, vertebral body reconstruction with cages, realignment, and/or plating or screwing. The approach was

RATIONALE: Central collapsed fracture blocks traditionally require either an anteromedial or anterolateral approach for reduction. However, existing techniques face challenges such as soft tissue damage and compromised tibial strength, especially in pilon fractures with central articular surface collapse and an intact anterior cortex, as classified under 43B2.3 in the 2018 Orthopaedic Trauma Association/Association for the Study of Internal Fixation Fracture and Dislocation Classification Compendium.
PATIENT CONCERNS: We address the management of pilon fractures with central articular surface

BACKGROUND: This study aimed to evaluate the injury mechanism of medial epicondylar fractures in children and adolescents and its association with increased carrying angle (CA) as a predisposing factor.
MATERIALS AND METHODS: We evaluated 37 patients with medial epicondylar fractures who were surgically treated at our institution. Medical records and plain radiographs were reviewed to determine the mechanism of injury and the humerus-elbow-wrist angle (HEWA) and CA of the uninjured arm. To evaluate the effect of coronal alignment on specific fracture type, we compared the CA and HEWA of the 2

Medial humeral epicondyle fractures account for 10% to 20% of elbow injuries in children. We hypothesized that in the fixation of medial humeral epicondyle fractures, safety and efficiency of bioabsorbable poly(l-lactide-co-glycolic acid) implant are comparable to traditional metallic and other novel approaches. A retrospective cohort study was performed between 2016 and 2019, analyzing 24 children who had medial humeral epicondyle fractures. Every fracture was stabilized with biodegradable poly(l-lactide-co-glycolic acid) implants (Bioretec® ActivaPin®) and tension band polydioxanone sutures.

BACKGROUND: The care of distal periprosthetic femoral fractures (PFF) is becoming a major interdisciplinary challenge due to demographic developments. The operative treatment is often performed (depending on the type of fracture) by means of locking plate fixation (LPF), although little data on the clinical outcome exist by now. The aim of the study is to identify risk factors for a poor outcome and increased mortality METHODS: In this retrospective study, 36 cases with distal PFF were examined. Exclusively treatment with LPF were included. Relevant previous illnesses (ASA score, Charlson inde
