A Sedation Protocol for Endoscopic Baton-Plate Adjustment in Neonates With Pierre Robin Sequence.

BACKGROUND: Robin sequence (RS) is characterized by upper airway obstruction leading to life-threatening desaturations in neonates. Procedural sedation in this population presents unique challenges due to anatomical airway anomalies, young patient age, and associated comorbidities. The pre-epiglottic baton-plate (PEBP) procedure is a non-invasive, endoscopically guided intervention that alleviates obstruction, facilitates oral feeding, and promotes mandibular growth. Sedation for PEBP adjustment requires the maintenance of spontaneous respiration while ensuring patient comfort and procedural t

Nursing home research in Ireland before and after the pandemic.

BACKGROUND: Nursing home residents in Ireland experienced a disproportionate burden of illness and death during the COVID-19 pandemic, highlighting longstanding systemic deficiencies in governance, staffing, and research engagement. Prior to the pandemic, this vulnerable population, characterised by high levels of multimorbidity and disability, received limited research attention. A subgroup from the National Clinical Programme for Older People recommended the development of a research agenda in nursing homes, including resident involvement. This study aimed to characterise the extent and natu

Defining Roles in Pediatric Palliative Care: Perspectives From Oncology and Palliative Care Teams.

BACKGROUND: Early integration of pediatric palliative care (PPC) is associated with improved symptom management, quality of life, and healthcare utilization for children with cancer. Despite this, variation persists in how PPC is understood, operationalized, and integrated within pediatric oncology programs. In particular, ambiguity surrounding the roles of oncology teams providing generalist palliative care and specialist PPC teams may contribute to delayed or inconsistent involvement. METHODS: We conducted a qualitative study using a grounded theory approach as part of a larger multi-site p

Integration of a complexity billing tool to evaluate unrealized pharmacist revenue and productivity in an academic primary care network.

BACKGROUND: Pharmacists are not recognized as health care providers under the Social Security Act. This limitation often confines pharmacists to "incident-to" billing for clinical services, which is commonly used for the simplest level of care, equating to the lowest reimbursement from payers. OBJECTIVE: Primary objectives of this study were to (1) compare the actual and potential unrealized revenue for pharmacist-provided services using a complexity billing tool adjusted for the level of care provided and (2) compare the actual and simulated work relative value unit (wRVU) for pharmacist-pro

Advance in the Kawasaki disease related coronary artery aneurysms: knowledge mapping, trends, and research frontiers.

BACKGROUND: Kawasaki disease (KD) is the leading cause of acquired heart disease in children. In untreated cases, coronary artery aneurysms (CAAs) may develop in up to 25% of patients. As the most significant long-term sequelae of KD, Kawasaki disease-related coronary artery aneurysms (KD-CAAs) contribute substantially to long-term cardiac morbidity and mortality. To date, few bibliometric study has specifically focused on this area. METHODS: We conducted a search in the Web of Science Core Collection (WOSCC) for papers related to KD-CAAs published between 2005 and 2024, and performed visual

Learnings from Patient Mortality after Delandistrogene Moxeparvovec Administration: A Report of Two Cases and Expert Committee Considerations for Future Mitigation and Management.

Acute liver injury (ALI) is a recognized adverse event with adeno-associated virus (AAV)-based gene therapies, including delandistrogene moxeparvovec, an AAVrh74-based gene therapy for Duchenne muscular dystrophy. Progression of ALI to acute liver failure (ALF) is rare. In clinical trials, ALF was defined as an international normalized ratio ≥1.5 with encephalopathy and/or ascites, occurring <26 weeks since product/treatment exposure, and in the absence of identified preexisting liver disease as assessed by the treating investigator. The two cases presented here represent the only known instan

Bridging tradition and innovation: electroacupuncture's impact on premature ovarian insufficiency.

Premature ovarian insufficiency (POI) refers to the decline in ovarian function in women before the age of 40, which can lead to premature ovarian failure and ultimately lead to infertility. In recent years, the incidence of POI has continued to rise, posing a serious threat to women's reproductive health and mental well-being. Although hormone replacement therapy (HRT) is currently the most widely used Western medical treatment method, its long-term use may carry risks such as thrombosis and breast cancer, and it is not yet an ideal treatment option. Electroacupuncture(EA), as an important in

Impact of Respiratory Muscle Strength Training Program on Hospital Readmissions in Patients With Chronic Obstructive Pulmonary Disease: A Retrospective Pilot Study.

PURPOSE: Chronic obstructive pulmonary disease (COPD) is associated with high 30-day hospital readmission rates and significant health care costs. Respiratory muscle weakness and dysphagia are common in this population and may contribute to adverse outcomes. Respiratory muscle strength training (RMST) has demonstrated benefits in outpatient settings; however, evidence supporting its initiation during acute hospitalization is limited. The purpose of this study was to evaluate the impact and feasibility of a speech-language pathologist (SLP)-led RMST program initiated during acute hospitalizatio

Development and implementation of a severe accidental hypothermia resuscitation pathway in an urban Canadian emergency department: a quality improvement initiative.

OBJECTIVES: Severe accidental hypothermia (core temperature < 28 °C) is an infrequent but resource‑intensive emergency department (ED) presentation that requires altered Advanced Cardiac Life Support (ACLS) management, aggressive rewarming, and timely consideration of veno-arterial extracorporeal membrane oxygenation (VA-ECMO). Prior to this project, our inner‑city academic ED lacked a standardized approach, contributing to variability in care. We designed, implemented, and achieved ≥ 95% real‑time utilization of a bedside pathway for patients with severe accidental hypothermia. METHODS: In r

Association between autoimmune diseases and atrial fibrillation: a real-world analysis from German outpatient data.

BACKGROUND: Atrial fibrillation (AF) is the most common sustained arrhythmia in clinical practice and is associated with considerable morbidity and mortality. Chronic systemic inflammation is increasingly recognized as a contributing factor in AF pathogenesis. Autoimmune diseases (AIDs), with their prototypical inflammatory conditions, may therefore be associated with AF. However, evidence from routine outpatient care regarding these potential associations across multiple AIDs remains limited. OBJECTIVE: We aimed to examine whether autoimmune diseases are associated with increased odds of atri

Prevalence of Bodily Distress Syndrome and Prediction of Patient Outcomes: Cohort Study of 3762 Individuals With Persistent Pain.

BACKGROUND: Persistent physical symptoms are common and often result in disability and high healthcare use. To capture how such symptoms co-occur, Bodily Distress Syndrome (BDS, also called Functional Somatic Disorders) was developed as an empirically derived construct, in contrast to consensus-based syndromes such as fibromyalgia and chronic fatigue syndrome/myalgic encephalomyelitis (CFS/ME). BDS is distinct from ICD-11 Bodily Distress Disorder and reflects the multisystem symptom pattern described in the Functional Somatic Disorder framework. However, its prevalence, symptom structure and p

Chronic pain and type 2 diabetes: associations between neuropathic and nociplastic pain conditions, health behaviors, and mental health in veterans.

Pain is common in T2D; however, the differential impact of specific types of chronic pain (i.e., diabetic peripheral neuropathy; DPN and chronic overlapping pain conditions; COPCs) on T2D outcomes is unclear. Understanding these differences may provide insight into shared mechanisms and treatment targets. Aims were to (1) Evaluate the association of COPCs with T2D in Veterans, and (2) evaluate if chronic pain conditions (DPN and COPCs) are associated with worse health behavior and mental health outcomes in Veterans with T2D. This was a retrospective observational cohort study with Million Vete

Incidence and risk factors for cardiac arrest following upper cervical spine fractures in the elderly after minor trauma.

INTRODUCTION: Cardiac etiologies are one of the main causes for out of hospital cardiopulmonary arrest (OHCA). Other etiologies might be easily overlooked, leading to delayed causal treatment. One such representative is neurogenic shock following upper cervical spine fractures after minor impact trauma in the elderly with little data on prevalence and risk factors, a drawback that we address in the present study. MATERIAL AND METHODS: Between 01/2013 and 12/2023, all patients ≥ 65 years with a fracture of the first and/or second vertebra after minor impact trauma were retrospectively reviewed

Subjective Values in Decision-Making Under Uncertainty: A Systematic Review.

Decision-making under uncertainty, risk, and ambiguity is a central topic in behavioral sciences, yet the cognitive and neural mechanisms through which individuals construct subjective value remain fragmented across disciplines. Neuroeconomics has emerged as an interdisciplinary field integrating psychology, economics, and neuroscience to explain how subjective value guides judgment and choice under uncertain conditions. However, the conceptual foundations of subjective value and its role across different forms of uncertainty remain insufficiently integrated. This study provides the first comp

Integrating Ambient In-Home Sensor Data and Electronic Health Record Data for the Prediction of Outcomes in Amyotrophic Lateral Sclerosis: Protocol for an Exploratory Feasibility Study.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) leads to rapid physiological and functional decline before causing untimely death. Current best-practice approaches to interdisciplinary care are unable to provide adequate monitoring of patients' health. Passive in-home sensor systems enable 24×7 health monitoring. Combining sensor data with outcomes extracted from the electronic health record (EHR) through a supervised machine learning algorithm may enable health care providers to predict and ultimately slow decline among people living with ALS. OBJECTIVE: This study aims to describe a federat
