Both cadavers and living patients were studied regarding a method to resolve large skin defects with bone exposure in the leg, with long-distance thrombosis of the anterior tibial vessels or posterior tibial vessels resulting from traumatic lesions. Popliteal vascular injury remains one of the most difficult diagnostic and therapeutic challenges for trauma surgeons. Only with strict attention to rapid diagnosis; early surgical treatment with meticulous technical skill; and aggressive use of various adjunctive measures, such as completion arteriography, anticoagulation, fasciotomy, and proper prioritization of management of multiple injuries, can limb salvage be optimized.

In 31 patients after a craniocerebral trauma combined with a trauma of locomotor system (CCCT), and in 28 patients with only a limb fractures a time course (1, 3, 7, and 14 days) of biochemical stress reaction inductors (cortisol, serotonin, histamine, lipid peroxidation products) in various tactics of surgical treatment was studied. A case of bacteraemia in a 54-year-old previously healthy man caused by the gram-negative rod Capnocytophaga canimorsus is presented. The patient developed severe ischaemia in both feet, thrombocytopenic purpura and renal failure. The bacteraemia was apparently caused by saliva from the patient's dog who had licked ulcers on the patient's legs. The patient regained full health but had a minor inferior myocardial infarction seven weeks after start of the infection.

Extremity amputation is a devastating injury. Forty-two patients who sustained traumatic limb amputation were contacted 3-57 months (mean, 25 months; median, 24 months) after injury to evaluate residual disability and to determine what factors were associated with a good recovery. There were 35 men and 7 women with ages ranging from 5 to 73 years (mean, 34 years). Amputation was the result of a motor vehicle crash in 18, work-related injury in 11, motorcycle crash in 9, and other causes in 4. Reconstructive plastic procedures in primary care and in postprimary/secondary treatment following below knee and above knee amputation are discussed. Short amputation stumps and stumps suffering from insufficient soft tissue coverage should be primarily avoided employing salvage replantation techniques. Secondarily conventional flaps and grafts, tissue expansion and/or microsurgical free tissue transfer all serve for achieving stable skin conditions and preserve adequate stump lengths. Interdisciplinary treatment racionale will lead to improvement of functional and social rehabilitation.

In surgical outpatients who required immobilization with a plaster cast because of injury to the lower limb a deep vein thrombosis occurred in 14%. General thromboprophylaxis seems advisable for surgical outpatients and depends on the severity of trauma and thrombosis risk factors. In a study of 97 patients who received a daily subcutaneous injection of heparin fragment calcium throughout their cast period, only one patient with a deep vein thrombosis was seen.

Mycobacterium smegmatis is an uncommon pathogen in humans. Fourteen cases of skin or soft-tissue infection due to M. smegmatis have been previously reported. We report two cases of posttraumatic M. smegmatis infection of the lower extremity. M. smegmatis infection produces chronic cellulitis with fistula formation that is most commonly a result of direct traumatic inoculation of contaminated material. Extensive surgical debridement followed by skin grafting has been necessary for cure in the majority of cases.

The paper refers to an experience of skin reconstruction of a widely damaged lower limb as result of a hot water burn injury in a young girl. Evidence is given to adopted reconstruction times, and to the decision not to recur to flap transposition by skin innests, but to the tissue expansion technique, also to avoid new aesthetic damages. The peculiarity of the experience is in the employment of a multiple number of tissues expander, to meet characteristic problems of the limbs related to this reconstructive technique, particularly in presence of heavy burn scars. We are often confronted with heavy destructions of the lower extremities, where there is no possibility to conserve the limb, because of the general or/and local conditions. In this situation it's wise to preserve vital rest structures that can be used for primary or secondary reconstructions of the injured limb. There are different possibilities, some of them are presented in the cases described in this article.

Prompt surgical decompression is the only means of preventing the late sequelae of ischaemic contracture in post-traumatic compartment syndromes.
