Ambulatory blood pressure and adherence monitoring: diagnosing pseudoresistant hypertension.

A small proportion of the treated hypertensive population consistently has a blood pressure greater than 140/90 mm Hg despite a triple therapy including a diuretic, a calcium channel blocker, and a blocker of the renin-angiotensin system. According to guidelines, these patients have so-called resistant hypertension. The prevalence of this clinical condition is higher in tertiary than primary care centers and often is associated with chronic kidney disease, diabetes, obesity, and sleep apnea syndrome. Exclusion of pseudoresistant hypertension using ambulatory or home blood pressure monitoring i

[Acute coronary syndrom as a cardiac manifestation of granulomatosis with polyangiitis].

HISTORY AND ADMISSION FINDINGS: A 49-year-old patient was admitted to our ward because of a troponin elevation (non ST-elevation myocardial infarction) following a rhinoscopy in an external hospital. The patient complained of typical angina, chronic rhinitis and epistaxis. Analysis of the nasal biopsy had shown the histological finding of granulomatosis with polyangiitis (Wegener's granulomatosis). INVESTIGATION: The consecutively performed catheterization showed a coronary one-vessel disease without significant stenosis. Echocardiography showed diastolic dysfunction as well as hemodynamicall

Pocketed microneedles for rapid delivery of a liquid-state botulinum toxin A formulation into human skin.

Botulinum toxin A (BT) is used therapeutically for the treatment of primary focal hyperhidrosis, a chronic debilitating condition characterised by over-activity of the eccrine sweat glands. Systemic toxicity concerns require BT to be administered by local injection, which in the case of hyperhidrosis means multiple painful intradermal injections by a skilled clinician at 6-monthly intervals. This study investigates the potential of a liquid-loaded pocketed microneedle device to deliver botulinum toxin A into the human dermis with the aim of reducing patient pain, improving therapeutic targetin

Isolated conjunctival lichen planus: a diagnostic challenge.

BACKGROUND: Lichen planus is a common inflammatory autoimmune condition of unknown etiology that commonly affects the skin and mucous membranes. Isolated ocular lichen planus is an extremely rare presentation that most commonly involves the eyelids, conjunctiva, and cornea, leading to severe scarring, and is clinically indistinguishable from other causes of cicatricial conjunctivitis. OBSERVATIONS: A 79-year-old man complained of a chronic keratoconjunctivitis refractory to multiple topical treatments. Slit-lamp examination revealed diffuse bilateral conjunctival hyperemia, subepithelial fibr

Scleroderma.

The prototypic autoimmune diseases involving skin (lupus, dermatomyositis) typically result in epithelial injury and autoantibodies to characteristic cellular antigens. Disease-specific autoantibodies are also found in scleroderma, but scleroderma is different from other cutaneous autoimmune diseases because epithelial injury does not occur. Multiple factors and combinations of factors (immune system, vascular and extracellular matrix abnormalities) are the most likely triggers in an individual with a genetic predisposition to scleroderma. These lead to increased synthesis of normal collagen i

Adverse respiratory reactions to aspirin and nonsteroidal anti-inflammatory drugs.

Aspirin-exacerbated respiratory disease (AERD) is an adult-onset condition that manifests as asthma, rhinosinusitis/nasal polyps, and sensitivity to aspirin and other cyclooxygenase-1 (COX-1)-inhibitor nonsteroidal anti-inflammatory drugs (NSAIDs). There is no cross-sensitivity to highly selective COX-2 inhibitors. AERD is chronic and does not improve with avoidance of COX-1 inhibitors. The diagnosis of AERD is made through provocative challenge testing. Following a positive aspirin challenge, patients can be desensitized to aspirin and NSAIDs. The desensitized state can be maintained indefini

Increasing use of reduced intensity conditioning transplants: report of the 2001 EBMT activity survey.

Since 1990, the EBMT has annually collected numbers of HSCT by disease indication, donor type and stem cell source. The 2001 survey concentrates on the use of reduced intensity conditioning (RIC) transplants and its dissemination in Europe. In 2001, there were 19576 HSCT for new patients, 6413 with allogeneic HSCT (33%), 13163 with autologous HSCT (67%) and 3256 additional re- or multiple transplants, 868 (667/201) allogeneic and 2658 (537/2121) autologous, collected from 596 centers in 35 European countries. The main indications in 2001 were leukemias (32%; 73% of them allogeneic); lymphomas

CsA-associated neurotoxicity and ineffective prophylaxis with clonazepam in patients transplanted for thalassemia major: analysis of risk factors.

Cyclosporin A (CsA) has been shown to be useful in the prophylaxis of acute graft-versus-host-disease (GVHD). However, this immunosuppressive agent produces multiple side-effects including nephrotoxicity, hypertension, hypertricosis, gum hyperplasia, infections, and neurotoxicity. We report a retrospective analysis of neurotoxicity in 625 recipients transplanted for thalassemia and given CsA as part of GVHD prophylaxis. Neurotoxicity consisted in mental status changes, tremor, headache (grade 1), visual disturbance and cortical blindness (grade 2) and seizures and coma (grade 3). The overall t

Somatostatin analogue (SMS 201-995) decreases plasma levels of corticotropin (ACTH) and corticotropin-releasing hormone in a patient with ectopic ACTH-producing tumors.

Effects of long-acting somatostain analogue (SMS 201-995) on plasma corticotropin (ACTH) and corticotropin-releasing hormone (CRH) levels were studied in a patient (63-year-old woman) with ectopic ACTH-producing tumors associated with type I multiple endocrine neoplasia (MEN-I). The patient had undergone bilateral adrenalectomy. Plasma CRH, as well as plasma ACTH, beta-endorphin and alpha-MSH, increased. The hormone levels were dramatically decreased by acute administration of SMS 201-995. Moderately higher doses of dexamethasone (0.05 or 0.1 mg/kg a day) did not decrease plasma CRH or ACTH. A

Chronic inflammatory and lymphoproliferative lesions of the equine small intestine.

A retrospective study was made of 20 horses with severe and extensive chronic disease of the small intestine. Many of the animals had clinical evidence of malabsorption, with progressive loss of weight, hypoalbuminaemia and sometimes anaemia. All but two of the horses were Thoroughbreds. The pathology was diverse. Nine of the cases were alimentary lymphomas (Platt, 1986) and five had lymphocytic and eosinophilic infiltrations in the bowel wall which were considered to be probable reactions to parasitic invasion. One had acute thrombosis associated with partial occlusion of the anterior mesente

Development and validation of a predictive model for chronic or persistent immune thrombocytopenia in children incorporating anti-glycoprotein IIb antibody: a retrospective cohort study utilizing LASSO regression and bootstrap stability analysis.

BACKGROUND: Pediatric immune thrombocytopenia (ITP) is an autoimmune condition involving platelet destruction. While most cases resolve spontaneously, 20%-30% progress to chronic disease (cITP), necessitating early risk prediction. Current prognostic models for childhood ITP lack platelet-specific immunological markers. This study aimed to develop and validate a predictive model for chronic/persistent ITP(c/pITP) integrating clinical factors and anti-platelet glycoprotein antibodies. METHODS: We retrospectively analyzed 381 children with newly diagnosed ITP (2022-2024). Patients were randomly

Brucella suis S2 strain inhibits IRE1/caspase-12/caspase-3 pathway-mediated apoptosis of microglia HMC3 by affecting the ubiquitination of CALR.

Neurobrucellosis represents a severe complication of brucellosis, posing a considerable risk to human health and quality of life. This condition arises from an increased susceptibility to chronic Brucella infection, a significant clinical challenge. One key factor contributing to chronic neurobrucellosis is the regulation of microglial apoptosis by Brucella; however, the exact molecular mechanisms remain largely unresolved. In this study, human microglial clone 3 (HMC3) cells were infected with Brucella suis vaccine strain S2 (B. suis S2) at varying multiplicity of infection (MOI) and duration

The Impact of the Minimum Legal Drinking Age on Alcohol-Related Chronic Disease Mortality.

BACKGROUND: The minimum legal drinking age (MLDA) of 21 has been associated with a number of benefits compared to lower MLDAs, including long-term effects, such as reduced risk for alcoholism in adulthood. However, no studies have examined whether MLDA during young adulthood is associated with mortality later in life. We examined whether individuals exposed to permissive MLDA (<21) had higher risk of death from alcohol-related chronic disease compared to those exposed to the 21 MLDA. Because prior work suggests that MLDA affects college students differently, we also conducted conditional analy

Beyond Physical Limitations: Depressive Mood and Self-Rated Health Among Adults with Severe Disabilities.

Background/Objectives: Self-rated health (SRH) is a widely used summary indicator of health and a predictor of subsequent morbidity and mortality. Among adults with severe disabilities, SRH may reflect not only chronic conditions and functional limitations but also psychological factors, particularly depressive mood. This study examined the incremental contribution of depressive mood beyond physical and functional factors to SRH among adults with severe disabilities. Methods: We analyzed data from a survey of adults with severe disabilities in Seoul, South Korea (N = 1519). SRH (higher scores

Bridging organizational boundaries in complex care pathways: lessons from a Lean and Safety Management approach.

BACKGROUND: Complex care processes that span hospital and community settings represent a growing organizational challenge for healthcare systems, particularly in the management of chronic conditions. These pathways involve interdependent activities distributed across multiple facilities, professional groups, and levels of care, increasing the risk of fragmentation, inefficiency, and patient harm. While Lean Healthcare (LH) has been widely adopted to improve healthcare performance, its application has largely remained confined to single hospital units, with limited attention to care integration

Multimodal Machine Learning to Personalize Transcutaneous Spinal Cord Stimulation for Stroke Rehabilitation.

INTRODUCTION: Several studies have identified transcutaneous spinal cord stimulation (tSCS) as a noninvasive neuromodulation technique for improving motor function in individuals with neurological disorders, including stroke. Despite a plethora of preliminary findings, there remains no standardized protocol regarding the optimal tSCS parameters tailored to patients with stroke. The objective of this study was to employ a supervised machine learning (ML) approach to determine the optimal tSCS frequency and intensity parameters for patients with chronic stroke by leveraging data from single-day
