[Organ defects due to cancer treatment in infancy and childhood].

In infancy and childhood, as in adults, organ defect is the most direct adverse effect of cancer treatment. Anatomical defects due to surgical treatment and functional defects due to radiotherapy are the two main types. In this paper, the relationship between the radicality of surgical treatment and preservation of organ form and function are discussed with regard to the thyroid, chest wall, lung, stomach, intestine, liver, pancreas, kidney and intra-pelvic organs. The more radical the treatment, the less preservation of organ form and function. The role of surgery has been changing in accorda

The surgical treatment of well-differentiated carcinoma of the thyroid.

Ninety-five surgically treated patients with well-differentiated carcinoma of the thyroid with an average of 11.2 years of follow-up were reviewed. Overall complications included 13% recurrence, 3% deaths due to thyroid cancer, 3% recurrent laryngeal nerve damage, and 2% hypoparathyroidism. Statistically significant prognostic findings included lower rates of recurrence and/or death among patients who were female, without lymph node involvement, or younger than 45. In comparison with total thyroidectomy, subtotal thyroidectomy was associated with markedly higher rates of recurrence and death a

Complications following surgery for cancer of the larynx and hypopharynx.

The hospital and office records of patients undergoing major surgery for cancer of the larynx and hypopharynx at the Washington University Medical Center, St. Louis from 1955 to 1972 were reviewed. Study parameters were correlated with the incidence of major complications and statistically analyzed to elucidate what factors contribute to increased complication rates. Abnormal margins of surgical resection were found to be significant in determining subsequent complication. Age, sex, race, site and stage of the primary tumor, and the presence of pre-treatment cervical lymph node fixation did no

[Effectiveness of simplified predictive intubation difficulty score and thyromental height in head and neck surgeries: an observational study].

BACKGROUND AND OBJECTIVES: In this study, we aimed to investigate the predictive value of different airway assessment tools, including parts of the Simplified Predictive Intubation Difficulty Score (SPIDS), the SPIDS itself and the Thyromental Height Test (TMHT), in intubations defined as difficult by the Intubation Difficulty Score (IDS) in a group of patients who have head and neck pathologies. METHODS: One hundred fifty-three patients who underwent head and neck surgeries were included in the study. The Modified Mallampati Test (MMT) result, Thyromental Distance (TMD), Ratio of the Height/

Anatomical Control of Adenoma Technique: An Accurate Surgical Approach to Thulium Laser Enucleation of the Prostate.

OBJECTIVE: To present our 2-lobe technique of thulium laser transurethral enucleation of the prostate. Transurethral resection of prostate and open prostatectomy have been traditionally considered as the gold standard for benign prostatic enlargement surgical treatment. Laser has been recently made available for benign prostatic enlargement surgery to minimize morbidity of traditional surgery.1 In 2009, Bach et al introduced the use of thulium laser,2 describing the 3-lobe technique, which overlaps open prostatectomy but with a greater control of hemostasis.3 Two techniques have been recently

Spinal Accessory Nerve Mononeuropathy Following Trapezius Lipoma Resection: A Case Report.

The spinal accessory nerve (SAN) is crucial for the motor function of the trapezius and sternocleidomastoid muscles, playing a significant role in scapular stability and upper limb mobility. While SAN injuries are commonly associated with iatrogenic causes such as lymph node biopsies, neck dissections, and posterior cervical trauma, injury following trapezius lipoma resection has not been well-documented. Given the SAN's superficial location and proximity to surgical fields in the posterior cervical triangle, it is vulnerable to inadvertent injury during tumor excision. To the best of our know…

Lymphoscintigraphy in pectoralis major myocutaneous flaps.

Myocutaneous flaps are used widely in the surgical treatment of advanced cancers, which in the past had been thought to be inoperable. Tumor recurrences are expected more frequently in these patients. Recurrent tumors may spread locally and to the regional areas via lymphatics and vessels. However, the lymphatic spread may differ in cases where myocutaneous flaps are used for reconstruction. This study is based on five patients with head and neck cancer who underwent reconstruction with myocutaneous flaps. Technetium Tc99m-labeled dextran was used to demonstrate the lymphatic flow, and technet

Development and internal validation of a nomogram for predicting lymph node metastasis posterior to the right recurrent laryngeal nerve in papillary thyroid carcinoma: a single-center retrospective study.

BACKGROUND: The lymph nodes posterior to the right recurrent laryngeal nerve (LN-prRLN) lie deep in the right central compartment. Clearing them is technically demanding and adds to the risk of injury to the nerve and parathyroid glands, whereas occult metastases left behind may cause persistent or recurrent disease. Knowing in advance which patients are likely to have LN-prRLN metastasis would help tailor the extent of central neck dissection. METHODS: We retrospectively reviewed 320 patients with papillary thyroid carcinoma (PTC) treated at a single tertiary center between August 2021 and F…

An updated systematic review and meta-analysis of results of transcervical carotid artery stenting with flow reversal.

OBJECTIVE: Transcervical carotid artery stenting (CAS) has emerged as an alternative to transfemoral CAS. An earlier systematic review from our group (n = 12 studies; 739 transcervical CAS procedures [489/739 with flow reversal]) demonstrated that transcervical CAS is a safe procedure associated with a low incidence of stroke and complications. Since then, new studies have been published adding nearly 1600 patients to the literature. We aimed to update our early systematic review and also to perform a meta-analysis to investigate outcomes specifically after transcervical CAS with flow reversal

Anatomical triangles defining routes to anterior communicating artery aneurysms: the junctional and precommunicating triangles and the role of dome projection.

OBJECTIVE: Anterior communicating artery (ACoA) aneurysms are common intracranial aneurysms. Despite advances in endovascular therapy, microsurgical clipping remains an important treatment for aneurysms with broad necks, large size, intraluminal thrombus, complex branches, or previous coiling. Anatomical triangles identify safe corridors for aneurysm access. The authors introduce the A1-A2 junctional triangle and the A1-A1 precommunicating triangle and examine relationships between dome projection, triangular corridors of access, and surgical outcomes. METHODS: Preoperative catheter and CT an
