Orbital cellulitis and endogenous endophthalmitis secondary to Proteus mirabilis cholecystitis.

We report the first case of orbital cellulitis and endogenous endophthalmitis secondary to Proteus mirabilis bacteremia that resulted from a calculus cholecystitis. Despite resolution of the gallbladder infection with antimicrobial therapy, the patient required evisceration of the affected eye. The pathogenesis of hematogenous endophthalmitis due to Gram-negative bacilli is discussed.

Reconstruction of the orbital floor with sheets of autogenous iliac cancellous bone.

PURPOSE: Various autografts and synthetic materials such as titanium plates have been used for orbital floor reconstruction; however, no consensus on reconstruction methods has been reached. We have used iliac cancellous bone for this reconstruction with good results. PATIENTS AND METHODS: Patients (n = 101) enrolled in this study underwent orbital floor reconstruction of pure blowout fracture with an approximate 1 mm thinned and trimmed iliac cancellous bone without fixation of the graft. The results were assessed by CT; diplopia exceeding an angle of 45 degrees was evaluated as a poor outco…

[Emphasis on pathology and hemodynamics based classification of orbital vascular lesions].

Orbital vascular lesions, mainly including hemangioma and vascular malformation, are the most common disorders among orbital diseases. Vascular lesions of the orbit have clinically complicated presentations and are very hard to deal with. Selection of therapeutic modality on orbital vascular lesions requires an appropriate classification of them. An improved classification of these lesions has been proposed by the Orbital Society based on the pathological types and hemodynamic features and has been extensively used in developed countries. However, orbital vascular lesions are still being class…

Endovascular therapy of hypoglossal canal AVFs presenting with orbital symptoms.

OBJECTIVE: Dural arteriovenous fistulae (DAVF) of the hypoglossal canal region are rare lesions. We describe three cases of DAVF of the hypoglossal canal presenting with ocular symptoms and discuss the endovascular management options. METHODS: Three consecutive patients with DAVF of the hypoglossal canal region presented with proptosis, chemosis and disturbances of extra-ocular mobility. Each patient was treated using a different endovascular approach, based on variations of the vascular access. RESULTS: The cases and treatments are reviewed, with a literature review on the subject. Endovasc

3D visualization and simulation in surgical planning system of orbital hypertelorism.

Simulation and three-dimensional visualization of object motion is a prerequisite for any surgical planning system. Orbital hypertelorism is a disease, which is most commonly associated with craniofacial malformations. We have developed a surgical planning system for planning and evaluation of orbital hypertelorism surgery. In our system CT-based virtual surface models fitted by oriented bounding boxes (OBB) are manipulated. Three-dimensional motion as well as a correction surgery can be simulated. Both are controlled by collision detection. The computer-based interactive surgery simulation sy…

Orbital cyst associated with ocular pit in an adult without microphthalmos.

We describe an adult without microphthalmos who developed an acquired compressive optic neuropathy secondary to an orbital cyst which showed direct communication with an optic disc pit. Such pits may be revealed by late complications such as orbital compressive cyst of the optic nerve, even in previously normal adult patients.

Medial orbital wall reconstruction through subciliary approach: revisited.

The aim of this study is to determine the safety and complication of subciliary approach through the retrospective review of our experiences. From 2005 through 2008, the subciliary skin-muscle flap methods were used in 30 patients undergoing medial orbital wall reconstruction. Preoperative and postoperative ophthalmic findings including diplopia, Hertel exophthalmometry, and occurrence of complications were checked. Resorbable polylactic acid sheet or porous polyethylene sheet was trimmed and molded in L shape, vertical portion to cover the medial wall defect and horizontal portion for stabili…

Evidence for visual compromise in preverbal children with orbital vascular birthmarks.

PURPOSE: To learn whether electrophysiological changes indicating amblyopia occur even in the absence of clinically recognizable amblyopia. DESIGN: Prospective study. METHODS: Four consecutive infants between 7 and 19 months of age with unilateral periocular vascular lesions that intermittently obstructed vision in the affected eye and no clinical evidence of amblyopia were evaluated. No child had anisometropia greater than 0.50 diopter in the greatest meridian or strabismus. Sweep visual evoked potential vernier acuity was measured under monocular viewing conditions with the fellow eye test

Orbital lymphoma presenting as an unstable ocular prosthesis: a salutary lesson.

We report the unusual presentation of an extranodal marginal zone B-cell lymphoma presenting as instability of an ocular prosthesis. More commonly, the stability of an ocular prosthesis can be affected by orbital implant migration, sunken superior sulcus, eyelid malformations, shallow inferior fornix, and contracted sockets (Charlton & Weinstein, 1995). Although rare, this case highlights the importance of meticulous examination of the socket, including careful palpation for any potential masses. To our knowledge, instability of an ocular prosthesis in association with orbital lymphoma has not…

Orbital meningioma after cranial radiation for acute lymphocytic leukemia.

PURPOSE: To describe the ophthalmic findings of cranial radiation-induced orbital meningioma after acute lymphocytic leukemia therapy. METHODS: This is a prospective clinical report. RESULTS: We describe two patients recently evaluated with orbital meningiomas many years after cranial radiation for acute lymphocytic leukemia. CONCLUSIONS: There will probably be more of these cases in the next several years. Unlike primary meningiomas, these neoplasms have a tendency to be more diffuse, multiple, and may undergo malignant degeneration.

Inclination of the superomedial orbital rim in relation to that of the nasolacrimal drainage system.

The influence of the height of the superomedial orbital rim on the ease of nasolacrimal intubation has not been investigated. The authors measured the inclination of the height of the superomedial orbital rim and the nasolacrimal drainage system in 46 sides of Japanese cadavers. The inclinations to the coronal plane of the lacrimal sac and the nasolacrimal duct were measured. A straight probe was inserted into the lacrimal sac and allowed to rest on the superomedial orbital rim. The angle made by this probe and the coronal plane, defined as the inclination of the height of the superomedial orb…

Natural killer/T-cell lymphoma masquerading as orbital cellulitis.

We describe 3 cases of natural killer/T-cell lymphoma that presented by masquerading as orbital cellulitis. All of the patients were examined for pain, fever, proptosis, and motility restriction. Computed tomography of the orbits and sinuses revealed orbital soft-tissue swelling without focal abscess in all 3 cases. Bacterial and fungal cultures were negative in each case and all of the patients had initial improvement on systemic antibiotics, only to relapse several days later. Diagnosis of natural killer/T-cell lymphoma was then made based on biopsy of the orbit or sinus. Natural killer/T-ce…

Orbital cellulitis with abscess formation following surgical treatment of canaliculitis.

The authors report a case of orbital cellulitis with abscess formation that developed after surgical management of canaliculitis. The patient developed pain, erythema, proptosis, and limited eye movements 3 days after canalicular debridement. She required urgent drainage of an orbital abscess and was treated with intravenous antibiotics for 2 weeks. Complications from surgical management of canaliculitis are unusual. Clinicians should be aware of the rare but serious risk of spread of the infection to the orbit.

Superior oblique myositis as an early feature of orbital Wegener's granulomatosis.

A 66-year-old female presented with a 6-month history of increasing right-sided facial pain and diplopia. Right ocular movements were restricted in adduction and downgaze. Superior oblique (SO) enlargement was seen on an initial MRI scan, and subsequent diffuse infiltration was seen on a repeat scan. An elevated cANCA and biopsy of SO confirmed Wegener's granulomatosis (WG). We report a case of SO myositis as a predominant early feature of WG granulomatosis and review the literature regarding extraocular muscle involvement in WG. To our knowledge, early SO involvement has not been reported.
