Pelvic body composition measurements by quantitative computed tomography: association with recent hip fracture.

INTRODUCTION: Loss of subcutaneous fat, decreased muscle cross-sectional area (CSA) and increased muscle adiposity are related to declining physical function and disability in the elderly, but there is little information about the relationship of these tissue changes to hip fracture. Thus we have compared body composition measures in women with hip fractures to age-matched controls, using quantitative computed tomography (QCT) imaging of the hip to characterize total adiposity, muscle CSA and muscle attenuation coefficient, a measure of adiposity. MATERIALS AND METHODS: 45 Chinese women (mean

A partial response to atezolizumab in alveolar soft part sarcoma with near-complete resolution of extensive pulmonary metastases: a case report.

Alveolar soft part sarcoma (ASPS) is a rare soft tissue sarcoma with high metastatic potential and limited response to systemic therapy. Biomarkers that predict which patients may benefit from emerging immune checkpoint inhibitors (ICIs) continue to surface. We report a case of stage IV ASPS with extensive pulmonary metastases and high PD-L1 expression that exhibited a marked response to atezolizumab, a PD-L1 antagonist. A 35-year-old man presented to an outside institution in October 2023 with a painless, progressively enlarging left thigh mass. Magnetic resonance imaging (MRI) demonstrated a

Impact of a novel multilayer imager on metal artifacts in MV-CBCT.

Objective. Megavoltage cone-beam computed tomography (MV-CBCT) imaging offers several advantages including reduced metal artifacts and accurate electron density mapping for adaptive or emergent situations. However, MV-CBCT imaging is limited by the poor efficiency of current detectors. Here we examine a new MV imager and compare CBCT reconstructions under clinically relevant scenarios.Approach. A multilayer imager (MLI), consisting of four vertically stacked standard flat-panel imagers, was mounted to a clinical linear accelerator. A custom anthropomorphic pelvis phantom with replaceable femor

Novel Covert-Inferior Pelvic Approach with a Subpubic Plate for Anterior Pelvic Ring Fractures: Preliminary Results.

BACKGROUND: Anterior pelvic ring injuries can be treated via Pfannenstiel, modified Stoppa, or ilioinguinal approaches, but these require exposing the abdominal soft tissues and may damage pelvic organs. The scar on the abdominal wall is also unacceptable for some patients. The minimally invasive anterior pelvic ring internal fixator (INFIX) is not ideal for thin patients with easily irritated skin, and it is associated with complications such as femoral nerve palsy, vascular occlusion, and lateral femoral cutaneous nerve injury. In this study, we designed a new external pelvic approach for th

Technique of C2 Nerve Root Preservation in the Presence of Anomalous Vertebral Artery in Atlantoaxial Operations.

While accessing the C1-C2 joint during posterior atlantoaxial fixation, the C2 nerve root along with its perineural venous plexus remains an obstacle for a panoramic visualization of the entry point of the C1 lateral mass and joint preparation. Therefore, many surgeons frequently advocate its intentional sectioning during this approach, with no related major complications.1,2 However, this sectioning has at times been associated with symptoms such as hypoesthesia, numbness, dysesthesia, and neuropathic ulcers.3 Thus C2 nerve root preservation during posterior approach for atlantoaxial dislocat

Subtotal Exenteration of the Orbit for Benign Orbital Disease.

PURPOSE: Total exenteration of the orbit with removal of the eye globe and surrounding tissues is most frequently indicated for malignant tumors. The indications for exenteration of the orbit for benign orbital lesion are rare. Not adequately treated infection of the orbit by systemic antibiotics can lead to destructive changes of soft tissues in the region of the orbit and partial exenteration with eyelid sparing technique is necessary. DESIGN: Retrospective case series. METHODS: Data of all patients between 2010 and 2018 who underwent exenteration of the orbit for periocular lesions infilt

Medial sural artery as a salvage recipient vessel for complex post traumatic microvascular lower limb reconstruction.

BACKGROUND: Complex lower extremity trauma still poses a formidable challenge for micro vascular reconstruction. The extensive surrounding zone of trauma can make it difficult to find a suitable recipient pedicle for anastomosis. A need was felt for exploration of newer recipient vessels in lower extremity which could be harnessed for reliable and tension free micro anastomosis. The purpose of the present report is to highlight the possibility of using medial sural artery for safe micro vascular anastomosis in selected scenarios like vessel depleted extremity. METHODS: Between 2008 and 2013,

Enhancing dermal and bone regeneration in calvarial defect surgery.

INTRODUCTION: To optimize the functional and esthetic result of cranioplasty, it is necessary to choose appropriate materials and take steps to preserve and support tissue vitality. As far as materials are concerned, custom-made porous hydroxyapatite implants are biomimetic, and therefore, provide good biological interaction and biointegration. However, before it is fully integrated, this material has relatively low mechanical resistance. Therefore, to reduce the risk of postoperative implant fracture, it would be desirable to accelerate regeneration of the tissues around and within the graft.

CT of orbital trauma.

In a patient with acute orbital trauma, visual acuity and extraocular muscle motility are the two most important ophthalmologic functions to be evaluated emergently. The assessment of these capabilities may sometimes be difficult due to the severity of the head injury, the extent of periorbital soft tissue edema, inadequate cooperation in alert patients, and a reduced level of consciousness in obtunded individuals. Consequently, computed tomography (CT) has come to play a major role in the orbital examination of acute trauma patients. In this study, in conjunction with clinical evaluation, we

Pregnancy-related transverse arch deformation: a subject-specific finite element analysis of the contributions of body weight and tissue stiffness.

BACKGROUND: Pregnancy is accompanied by physiological changes that alter foot structure and loading conditions, which may contribute to foot pain, instability, and an increased risk of falls. Gestational weight gain and pregnancy-related changes in passive soft-tissue compliance occur simultaneously, making it difficult for clinical studies to distinguish their individual contributions to transverse arch deformation. The purpose of this study was to independently quantify the effects of increased body weight and reduced soft tissue stiffness on transverse foot arch geometry during pregnancy an
