Use of intraoperative dynamic infrared imaging with detection wavelength of 7-14 microm in the surgical obliteration of spinal arteriovenous fistula: case report and technical considerations.

BACKGROUND AND OBJECTIVE: Although improvements of spatial and temporal resolution in infrared (IR) imaging have enabled intraoperative real-time acquisition of physiological and pathological information on living organs, the imaging qualities of anatomical delineation of blood vessels and functional delineation of blood flow were insufficient to serve as visual monitoring. The main reason was partly due to the lack of an appropriate IR detection IR band (formerly 3 - 5 micro m), and the broad dynamic range in previous modalities. METHODS: To make a good contrast between blood vessels and sur

Microsurgical removal of petroclival meningiomas: a report of 33 patients.

This is a report of 33 consecutive cases of petroclival meningioma treated surgically at our institution over the last 10 years; there were 21 women and 12 men between the ages of 27 and 68 (mean age, 52). All patients were assessed by computed tomographic scans including coronal sections and bone algorithm studies; in most cases, digital subtraction angiography and magnetic resonance imaging were also done. The largest tumor diameter was between 2 and 3.5 cm in 14 cases, 3.5 to 6 cm in 15 cases, and over 6 cm in 4 cases. Dural attachment predominantly involved the clivus and apical petrous bo

First experience with endovascular recanalization of chronic internal carotid artery occlusion.

BACKGROUND: According to WHO, the number of people suffering from atherosclerotic lesions of the brachiocephalic arteries in different age groups is 20-50%. The development of ischemic stroke with existing chronic occlusion of the internal carotid artery (ICA) occurs with a frequency of 6-15%, and the annual risk of developing acute ischemic disorders in the ipsilateral basin, despite adequate drug treatment, ranges from 6 to 20%. The aim is to evaluate the efficacy and safety of endovascular recanalization in chronic ICA occlusion. MATERIAL AND METHODS: The analysis of the first results of i

Percutaneous Endovascular Therapeutic Options in Treating Posttransplant Hepatic Artery Thrombosis With the Aim of Salvaging Liver Allografts: Our Experience.

OBJECTIVES: Hepatic artery thrombosis remains a major complication after orthoptic liver transplant. Treatment of hepatic artery thrombosis is complex and requires a multidisciplinary approach. Retransplant is the procedure of choice. In nonsurgical candidates, endovascular options are evolving. MATERIALS AND METHODS: Based on our experience at a busy transplant center, we discuss 4 representative cases to explain the potential role of endovascular treatment beyond just attempts at recanalization. From our experience, as well as a review of the literature, we propose a clinical practice algor

A patient with acute kidney pain and high blood pressure.

This case presented challenging diagnostic and management issues in a young healthy man who presented with abdominal pain and new-onset hypertension. The differential diagnosis evolved over the course of the clinical presentation. The patient had severe vascular involvement of his renal and basal cerebral arteries that initially was assumed to be due to a vasculitic process or hypercoagulable state. Finally it became apparent that the patient did not have a systemic illness but rather a localized vascular disease most likely due to segmental arterial mediolysis, a rare, under-recognized condit

Successful fibroid embolization of pelvic and inferior mesenteric artery collaterals after previous uterine artery embolization.

A 47-year-old woman with a history of myomectomies and uterine artery embolization 15 years previously presented with increasing menorrhagia and dysmenorrhea. Magnetic resonance imaging (MRI) demonstrated multiple enhancing fibroids, extensive uterine supply from what appeared to be patent uterine arteries, and significant supply from what appeared to be the left ovarian artery. Aortography demonstrated no ovarian supply, but extensive collateral supply from distal branches of the inferior mesenteric artery (IMA), with further collateral supply from the anterior division of both internal iliac

Spinal dural arteriovenous fistulas: outcome and prognostic factors.

The aim of this study is to review the clinical outcome of patients treated for spinal dural arteriovenous malformations and investigate the presence of pretreatment indicators of outcome after short- and midterm follow-up. The authors retrospectively reviewed the records of 65 consecutive patients treated either surgically or endovascularly in 3 neurosurgery departments between 1989 and 2009. After treatment, 80% of patients reported improvement of at least 1 symptom. Motor symptoms improved more than sensory disorders, pain, or sphincter impairment. Spinal dural arteriovenous fistulas at the

Percutaneous treatment of liver failure and acute mesenteric ischaemia.

INTRODUCTION: Synchronous embolism to the superior mesenteric artery (SMA) and coeliac axis (CA) is a rare disease. REPORT: A 67-year-old man with atrial fibrillation developed acute liver failure due to an embolic occlusion of the CA and SMA, with a severe coagulation disorder. He was successfully managed with percutaneous stent placement and an exploratory laparotomy was not needed. He remains symptom-free 1 year after the procedure, and duplex follow-up showed stent patency. CONCLUSION: Endovascular techniques in patients with liver failure, no signs of peritonism, early diagnosis and hig

Delayed permanent paraplegia after endovascular repair of abdominal aortic aneurysm.

Spinal cord ischemia is a rare complication after abdominal aortic surgery and has been attributed to surgical devascularization of the spinal cord, atheroembolization of the cord circulation, or hypoperfusion of cord structures secondary to hypotension or cord edema. We present a diabetic, hypertensive 75-year-old male with endstage renal disease who presented with a 5.5 cm asymptomatic infrarenal abdominal aortic aneurysm, and concomitant 3.5 cm right common iliac artery aneurysm. After undergoing successful endovascular repair with an aorto-uni-iliac device, unilateral hypogastric artery em

[The Klippel-Trenaunay syndrom associated with multiple visceral arteries aneurysms].

The Klippel-Trenaunay-syndrom (KTS) is a congenital angiodysplasia of venous vessels characterized by three main symptoms: cutanous vascular naevi, hyperthropy of a limb and varicosis or venous malformations. The coincidence of KTS and arterial malformations such as renal artery aneurysm has been described twice in the literature. We report the case study of a 40-years-old male patient with KTS and aneurysms of a renal artery, the splenic artery, the superior mesenteric artery and of a popliteal artery and popliteal vein. After documenting the diagnostic and therapeutic course we describe this
