Key outcomes and addressing remaining challenges--perspectives from a final evaluation of the China GAVI project.

During the China GAVI project, implemented between 2002 and 2010, more than 25 million children received hepatitis B vaccine with the support of project, and the vaccine proved to be safe and effective. With careful consideration for project savings, China and GAVI continually adjusted the budget, additionally allowing the project to spend operational funds to support demonstration projects to improve timely birth dose (TBD), conduct training of EPI staff, and to monitor the project impact. Results from the final evaluation indicated the achievement of key outcomes. As a result of government c

The H1N1 influenza pandemic: need for solutions to ethical problems.

The rapid spread of the novel influenza virus of H1N1 swine origin led to widespread fear, panic and unrest among the public and healthcare personnel. The pandemic not only tested the world's health preparedness, but also brought up new ethical issues which need to be addressed as soon as possible. This article highlights these issues and suggests ethical answers to the same. The main areas that require attention are the distribution of scarce resources, prioritisation of antiviral drugs and vaccines, obligations of healthcare workers, and adequate dissemination and proper communication of inf

Evaluating HIV prevention efforts using semiparametric regression models: results from a large cohort of women participating in an HIV prevention trial from KwaZulu-Natal, South Africa.

BACKGROUND: As a group of more than 3.67 million people, the bone health of Chinese plasmapheresis donors, which the main population is also a risk group of osteoporosis (OP), has raised concerns. Therefore, this article investigates the relationship between bone mineral density (BMD), bone metabolism indicators, and plasmapheresis donation behavior among some high-risk plasmapheresis donors for OP in China, and further explores the mediating factors through reasonable statistical methods. METHODS: Recruiting long-term and highly active plasmapheresis donors and new donors to measure the tota

Fourteen years in resistance.

Resistance trends have changed greatly over the 14 years (1997-2011) whilst I was Director of the UK Antibiotic Resistance Monitoring and Reference Laboratory (ARMRL). Meticillin-resistant Staphylococcus aureus (MRSA) first rose, then fell with improved infection control, although with the decline of one major clone beginning before these improvements. Resistant pneumococci too have declined following conjugate vaccine deployment. If the situation against Gram-positive pathogens has improved, that against Gram-negatives has worsened, with the spread of (i) quinolone- and cephalosporin-resistan

Risks to young volunteers in international social projects.

BACKGROUND: The number of young volunteers in international social projects has increased significantly with governmental and non-governmental project support. This paper investigates the hypothesis that the preventative medical advice currently given prior to departure is inadequate because the risk profile of young persons (<30 years) differs from that of the general population. METHODS: A retrospective cross-sectional study was performed with participants of international social projects. A specific questionnaire was developed; inclusion criteria were age between 18 and 30 years at departu

South Carolina tuberculosis genotype cluster investigation: a tale of substance abuse and recurrent disease.

The South Carolina Tuberculosis (TB) Control Division ranked all the TB genotype clusters (two or more cases with matching genotypes) in the state based on the number of cases. The largest cluster, PCR00002, was investigated to determine if the cluster represented recent Mycobacterium tuberculosis transmission, and if so, to identify associated risk factors. The PCR0002 cluster, which included pediatric cases, clearly represented recent M. tuberculosis transmission. The two primary factors contributing to cluster growth were substance abuse and recurrent TB disease. Elimination of ongoing M. t

Long delays and missed opportunities in diagnosing smear-positive pulmonary tuberculosis in Kampala, Uganda: a cross-sectional study.

BACKGROUND: Early detection and treatment of tuberculosis cases are the hallmark of successful tuberculosis control. We conducted a cross-sectional study at public primary health facilities in Kampala city, Uganda to quantify diagnostic delay among pulmonary tuberculosis (PTB) patients, assess associated factors, and describe trajectories of patients' health care seeking. METHODOLOGY/PRINCIPAL FINDINGS: Semi-structured interviews with new smear-positive PTB patients (≥ 15 years) registered for treatment. Between April 2007 and April 2008, 253 patients were studied. The median total delay was

ESBL-producing Enterobacteriaceae: controlling the spread of infection.

Extended spectrum beta-lactamase (ESBL)-producing Enterobacteriaceae were first reported in Europe in the 1980s and have since become a worldwide problem. This has resulted in increased morbidity, mortality and cost in treating the infections they cause (Paterson et al, 2001; Bisson et al, 2002). In the UK the prevalence of ESBL rose from 5.4% in 1997 to 22.7% in 1999 (Goossens, 2000). ESBL are not the only beta-lactamases to present resistance to 1st, 2nd and 3rd generation cephalosporins but are the most important. New strains produce a particular type of ESBL, the CTX-M type, which is able

The Belgrade childhood diabetes study - comparison of children with type 1 diabetes with their siblings.

A case-control study was conducted in Belgrade (about 320,000 inhabitants 0-16 years old) during the period 1994-97, comprising 68 diabetic children (cases) and 68 controls chosen from the siblings of the cases. Analysis using multivariable logistic regression analysis indicated the following independent risk factors for Type 1 diabetes: higher birth order, infections during the 6 months preceding the onset of the disease and stressful events. Out of individual stressful and psychological factors, 'other' stressful events (severe accident or hospitalisation or death of a close friend, conflict

Cost-effectiveness of different ways of controlling intestinal schistosomiasis: a case study.

Intestinal schistosomiasis is highly prevalent among the labourers and their families in many of the miners' villages in Maniema, Zaire. Following treatment, a quick re-infection can be observed if no measures are taken to reduce transmission. It is shown that the rate of re-infection is reduced when the water bodies around an endemic village are properly molluscicided. When, in addition to chemotherapy, a strategy of focal mollusciciding was adopted beneficial effects were visible 12 months after treatment but no longer at 20 months. In view of the comparatively small size of the human popula

Evaluation of the sentinel surveillance system for influenza-like illnesses in the Greater Accra region, Ghana, 2018.

BACKGROUND: Influenza-like Illness (ILI) is a medical diagnosis of possible influenza or another respiratory illness with a common set of symptoms. The deaths of four schoolchildren, during a pandemic influenza outbreak in December 2017 in Ghana, raised doubts about the ILI surveillance system's performance. We evaluated the ILI surveillance system in the Greater Accra region, Ghana, to assess the system's attributes and its performance on set objectives. METHODS: CDC guidelines were used to evaluate the data of the ILI surveillance system between 2013 and 2017. We interviewed the surveillanc

Horse owners' biosecurity practices following the first equine influenza outbreak in Australia.

A cross-sectional study was conducted involving 759 Australian horse owners to determine their biosecurity practices and perceptions one year after the 2007 equine influenza outbreak and to investigate the factors influencing these perceptions and practices. A web link to an online questionnaire was sent to 1224 horse owners as a follow-up to a previous study to obtain information about biosecurity perceptions and practices, impacts of the 2007 EI outbreak, demographic information and information about horse industry involvement. Ordinal logistic regression analyses were conducted to determine

Human T-lymphotrophic virus type 1-associated infective dermatitis: a comprehensive review.

Infective dermatitis (ID) is a chronic, relapsing dermatitis associated with human T-lymphotrophic virus (HTLV)-1 which was initially described in Jamaican children. Although most cases have been reported in Jamaica, ID may be seen in other HTLV-1 endemic areas, such as Brazil, Japan, sub-Saharan Africa, and Trinidad and Tobago. Since HTLV-1 infection has been implicated in the development of adult T-cell leukemia/lymphoma, an aggressive hematologic malignancy, and HTLV-1-associated myelopathy/tropical spastic paraparesis, a neurodegenerative disease, ID may serve as an early clinical marker f

Integrated Care Models for HIV, Diabetes and Hypertension in Sub-Saharan Africa: A Systematic Review of Effectiveness, Implementation and Real-World Applicability.

BACKGROUND: Sub-Saharan Africa faces a syndemic of HIV and non-communicable diseases (NCDs), particularly diabetes and hypertension. Integrated care models have been promoted to improve service efficiency and patient outcomes; however, evidence on their effectiveness, implementation and real-world applicability remains fragmented. OBJECTIVE: This systematic review aimed to assess the clinical and service delivery effectiveness of integrated HIV-diabetes-hypertension care models in sub-Saharan Africa and to identify key implementation barriers and facilitators, and sustainability consideration

Relationships between prolonged physical and social isolation during the COVID-19 pandemic, reduced physical activity and disability in activities of daily living among people with advanced respiratory disease.

In people with advanced respiratory disease, we examined (i) the impact of COVID-19-related physical and social isolation on physical activity and (ii) relationships between time spent in isolation and disability in activities of daily living. Cross-sectional analysis was conducted in adults with advanced non-small cell lung cancer, chronic obstructive lung disease or interstitial lung disease. Measures included change in physical activity since physically and socially isolating (Likert scale) and disability (Barthel Index and Lawton-Brody IADL scale) or difficulty (World Health Organisation D
