Anti-inflammatory cytokine TSG-6 inhibits hypertrophic scar formation in a rabbit ear model.

Hypertrophic scars are characterized by excessive fibrosis and extracellular matrix (ECM) deposition and can be functionally and cosmetically problematic; however, there are few satisfactory treatments for controlling hypertrophic scars. The inflammatory cells and cytokines involved in excessive inflammation during wound healing facilitate fibroblast proliferation and collagen deposition, leading to pathologic scar formation. TSG-6 exhibits anti-inflammatory activity. This study examined the effect of recombinant TSG-6 on inflammation in hypertrophic scars using a rabbit ear model. Six 7-mm, f…

Effects of ear-canal static pressure on pure-tone thresholds and wideband acoustic immittance.

BACKGROUND: Wideband acoustic immittance (WAI) measures provide information about middle-ear function across the traditional audiometric frequency range from 0.25 to 8.0 kHz. Recent studies have found that WAI is effective in predicting the presence of conductive hearing loss (CHL). It is not known whether WAI can accurately estimate the degree of threshold shift caused by CHL. PURPOSE: The purpose of the present study was to evaluate the relationship between changes in pure-tone threshold and changes in wideband absorbance and acoustic conductance levels induced by positive and negative ear-…

Regeneration of human-ear-shaped cartilage by co-culturing human microtia chondrocytes with BMSCs.

Previously, we had addressed the issues of shape control/maintenance of in vitro engineered human-ear-shaped cartilage. Thus, lack of applicable cell source had become a major concern that blocks clinical translation of this technology. Autologous microtia chondrocytes (MCs) and bone marrow stromal cells (BMSCs) were both promising chondrogenic cells that did not involve obvious donor site morbidity. However, limited cell availability of MCs and ectopic ossification of chondrogenically induced BMSCs in subcutaneous environment greatly restricted their applications in external ear reconstructio…

Cerebellar infarction presenting as inner ear decompression sickness following scuba diving: a case report.

INTRODUCTION: Inner ear decompression sickness following scuba diving is not uncommon and the characteristic features of this disorder are acute peripheral vestibular syndrome, sometimes associated with cochlear signs, requiring urgent hyperbaric oxygen therapy. Cerebellar infarction can also mimic isolated peripheral vestibulopathy. CASE REPORT: The authors report the case of a 47-year-old man in good general health admitted with acute left vestibular dysfunction suggestive of inner ear decompression sickness 6 hours after scuba diving. Normal videonystagmography and delayed onset of occipit…

A painful stiff neck following an ear, nose, and throat surgical procedure: case report.

Grisel syndrome is a rare, nontraumatic atlantoaxial subluxation, typical of developmental ages and characterized by head flexion/rotation and painful fixation. Neurological symptoms may occur. It is secondary to head/neck infections and ear, nose, and throat surgery (adenoidectomy, tonsillectomy, and mastoidectomy). Here, we report the case of a child who presented a painful stiff neck following an adenotonsillectomy, with imaging evidencing an atlantoaxial subluxation. The child showed improvement in his condition following a conservative treatment with antibiotics, anti-inflammatory, and an…

First branchial cleft fistula associated with external auditory canal stenosis and middle ear cholesteatoma.

INTRODUCTION: First branchial cleft anomalies manifest with duplication of the external auditory canal. CASE REPORT: This report features a rare case of microtia and congenital middle ear and canal cholesteatoma with first branchial fistula. External auditory canal stenosis was complicated by middle ear and external canal cholesteatoma, but branchial fistula, opening in the zygomatic root and a sinus in the helical root, may explain this feature. A canal wall down mastoidectomy with canaloplasty and wide meatoplasty was performed. The branchial cleft was excised through parotidectomy and faci…

Estimation of bone conduction skull transmission by hearing thresholds and ear-canal sound pressure.

Bone conduction sound transmission in the human skull and the occlusion effect were estimated from hearing thresholds and ear-canal sound pressure (ECSP) measured by a probe tube microphone when stimulation was at three positions on the skull (ipsilateral mastoid, contralateral mastoid, and forehead). The measurements were done with the ear-canal open as well as occluded by an ear-plug. Depending on the estimation method, transcranial transmission at frequencies below 1 kHz was between -8 and 5 dB, around 0 dB at 1 kHz that decreased with frequency to between -17 and -7 dB at 8 kHz. The forehe…

Antibody in middle ear fluid of children originates predominantly from sera and nasopharyngeal secretions.

The human middle ear is devoid of any immunocompetent cells in normal mucosa. We sought to determine the source of antibody present in the middle ear of children. Total IgG, IgA, and secretory IgA antibodies were determined by enzyme-linked immunosorbent assay from the nasopharyngeal, middle ear, and serum samples of children with acute otitis media. The two-dimensional gel electrophoresis pattern of the entire array of IgA antibodies in the nasal wash (NW) and middle ear fluid (MEF) was compared from the MEF and NW samples using isoelectric focusing and Western blotting. The total IgG and IgA…

Systematic analysis of ear-nose-throat malpractice complaints may be beneficial for patient safety.

INTRODUCTION: The analysis of malpractice complaints can provide valuable information on patient safety. This study offers a detailed examination of the backgrounds concerning reasons and outcomes of ear, nose and throat (ENT) malpractice complaints handled by the National Board of Patients' Complaints (NBPC), 1998-2008. MATERIAL AND METHODS: All NBPC decisions related to the ENT specialty from 1998-2008 in Denmark were analysed. RESULTS: Deficient medical recording was criticized even though the complainant did not mention this in the complaint. The juridical theme of medical error was the …

Air-bone gap component of inner-ear origin in audiograms of cochlear implant candidates.

BACKGROUND: Experimental studies have shown that creating a window in the bony cover of the cochlea and vestibular parts of the inner ear, with preservation of membranous and middle-ear functions, induces an air-bone gap (ABG). This study sought to determine if a similar mechanism explains the ABG frequently observed in audiograms of cochlear implant candidates. METHOD: The study group included 47 candidates for a cochlear implant (94 ears) attending a university-affiliated tertiary medical center who had an ABG component in the audiogram in the absence of external or middle-ear abnormalities…

Behavioral hearing thresholds between 0.125 and 20 kHz using depth-compensated ear simulator calibration.

OBJECTIVES: The purpose of this study was to obtain behavioral hearing thresholds for frequencies between 0.125 and 20 kHz from a large population between 10 and 65 yr old using a clinically feasible calibration method expected to compensate well for variations in the distance between the eardrum and an insert-type sound source. Previous reports of hearing thresholds in the extended high frequencies (>8 kHz) have either used calibration techniques known to be inaccurate or specialized equipment not suitable for clinical use. DESIGN: Hearing thresholds were measured from 352 human subjects bet…

Gustatory changes in patients with chronic otitis media, before and after middle-ear surgery.

OBJECTIVE: We investigated gustatory changes in patients with chronic otitis media, before and after middle-ear surgery. METHODS: This prospective study included 38 patients with unilateral chronic otitis media. We used taste testing solutions to evaluate each patient's taste function. Intra-operative assessments of the chorda tympani nerve were also compared and analysed. RESULTS: Patients with chronic otitis media had significantly worse ipsilateral perception of sour, bitter and salty tastes. In patients with good intra-operative preservation of the chorda tympani nerve, there was signifi…

Creating a neoconchal complex using the adjustable conchal sliding technique in prominent ear correction.

An oversized aural concha plays a significant role in prominent ear deformities and should be taken into consideration during preoperative examination. In the current study, we present a procedure known as the conchal sliding technique as an alternative to more disruptive methods. Twenty-four patients (9 females and 15 males; 47 ears in total) underwent a conchal sliding procedure between 2006 and 2010. During the surgery, a wide subperichondrial dissection is performed after a posterior elliptical incision. After the concha is exposed as a hemisphere, it is split along its long axis to reveal…

Effective scoring of scapha prevents helix irregularity in prominent ear correction - a biomechanical study.

BACKGROUND: As the antihelix is created in the operation for prominent ear, the helix often presents irregularities. This biomechanical study aims to elucidate effective techniques to prevent these irregularities. METHODS: Finite element models were produced simulating 10 prominent ears. The scaphas of the 10 models were thinned to simulate scoring or abrasion of the cartilage. The thinning was conducted in four fashions. In the first group, no thinning was conducted (Non-Scoring Models); in the second group, the upper half of the scapha was thinned (Upper-Scoring Models); in the third group,…
