INTRODUCTION: Lateral humeral condyle fractures typically require a longer period of internal fixation than other distal humeral fractures due to the increased risk of non-union. K-wires can be buried and left in situ until union or they can be left unburied and require removal after four weeks, with plaster immobilisation until union. There is no consensus as to whether wire burial is preferable or not. The aim of this study was to determine whether K-wire burial is associated with more complications than non-buried wires in treating lateral condyle fractures of the elbow.

INTRODUCTION: Rotator cuff injuries are commonly diagnosed orthopaedic conditions. The purpose of this study was to determine survivorship of primary open cuff repairs, with survivorship defined as a shoulder not requiring additional surgery.
MATERIALS AND METHODS: Between 1993 and 2004, open cuff repairs were performed in 254 patients (263 shoulders). All had a complete tear of the supraspinatus. In addition, 86 patients had tears of the infraspinatus and 50 had subscapularis tears. Survivors did not require further surgery. Survivorship was analyzed by use of the Kaplan-Meier method, and th

INTRODUCTION: Controversy continues around selecting the best strategy for managing nonunions of the humeral diaphysis. The objective of this retrospective study was to analyse the results of management of this complication using a uniform surgical technique.
HYPOTHESIS: The rate of union obtained in the present series is comparable to the results reported in the literature.
PATIENTS AND METHODS: Twenty-one patients were surgically treated at the Geneva University Hospital for nonunion of the humeral diaphysis between 1995 and 2005 with a mean follow up of 50 months. Open reduction and inter

BACKGROUND: Traumatic anterior-inferior shoulder joint dislocations are common injuries among the young athletic population. The aim of this study was to assess which factors, including concomitant injury (rotator cuff tears, superior labral anterior posterior [SLAP] lesions), patient age, and fixation methods, led to redislocation after arthroscopic stabilization.
HYPOTHESIS: There are several risk factors for the outcome after arthroscopic anterior-inferior glenohumeral stabilization.
STUDY DESIGN: Cohort Study; Level of evidence, 3.
METHODS: Between 1996 and 2000, 221 patients were treat

BACKGROUND: Displaced ipsilateral fractures of the clavicle and the glenoid neck are a complex injury pattern that is usually the result of high-energy trauma. The treatment of these injuries is controversial, as good results have been reported with both operative and nonoperative treatment.
METHODS: Nineteen patients who had sustained a displaced fracture of the glenoid neck with an ipsilateral clavicular fracture or acromioclavicular separation (floating shoulder) were retrospectively evaluated. The treatment was nonoperative in twelve patients and operative in seven. At the time of final f

INTRODUCTION: Distal biceps tendon repair is usually performed via a double-incision or single-incision bicortical drilling technique. However, these techniques are associated with specific complications and usually do not allow for anatomic footprint restoration. It was the aim of this study to report the clinical results of a double intracortical button anatomic footprint repair technique for distal biceps tendon tears. We hypothesized that this technique would result in supination strength comparable to the uninjured side with a low rerupture rate and minimal bony or neurologic complication

Lateral epicondylalgia (LE) is the most common chronic painful condition affecting the elbow in the general population. Although major advances have been accomplished in recent years in the understanding of LE, the underlying physiopathology is still a reason for debate. Differences in clinical presentation and evolution of the symptoms among patients, suggest the need for revisiting the current knowledge about subjacent mechanisms that attempt to explain pain and functional loss. Previous models have suggested that the condition is mainly a degenerative tendinopathy, associated with changes i

Arm wrestling places significant torque on the humeral shaft. A spiral distal humeral shaft fracture is an unusual but significant injury that can result. Of 93 patients who presented between 2009 and 2017 with closed humeral shaft fractures that were managed nonoperatively, 9 sustained the fractures while arm wrestling. Outcomes were compared with those of all other patients with nonoperatively managed humeral shaft fractures sustained through other mechanisms. The Student's t test was used to compare cohorts. All patients had spiral fractures that occurred in the distal one-third of the hume

BACKGROUND: Tendinopathies of the shoulder and elbow joint are a common problem. According to the current state of knowledge tendinopathies can be separated into acute and chronic tendinitis as well as degenerative tendinosis.
ORIGIN: The causes of tendinopathy can be intrinsic, extrinsic or a combination of both. A false straining or overuse with repetitive microtrauma is often the cause. Particularly affected are tendons of the rotator cuff, the long biceps tendons and lower arm extensors.
TREATMENT: Priority is given to conservative appproaches for these disease processes. Following appro

PURPOSE: To measure distances from anatomical landmarks to the median nerve, and estimate the length of the flexor-pronator/flexor carpi ulnaris (FCU) detachment necessary to expose the anteromedial facet of the ulnar coronoid process (UCP) using the Hotchkiss over-the-top approach.
METHODS: Dissections were made of 20 fresh-frozen cadaveric upper limbs. Measurements were made of the shortest distance from the medial epicondyle to the median nerve, the distance from the medial epicondyle to the median nerve in line with the flexor-pronator/FCU interval, the shortest distance from the apex of

BACKGROUND: Distal biceps brachii tendon repairs performed with a tension slide technique using a cortical button (CB) and interference screw are stronger than those based on suture fixation through bone tunnels (BTs) in biomechanical studies. However, clinical comparison of these 2 techniques is lacking in the literature.
PURPOSE: To perform a clinical comparison of the single-incision CB and double-incision BT techniques.
STUDY DESIGN: Cohort study; Level of evidence, 3.
METHODS: Distal biceps tendon ruptures repaired through either the single-incision CB or double-incision BT technique w

BACKGROUND AND PURPOSE: The pathomechanics of the Essex-Lopresti lesion are not fully understood. We used human cadavers and documented the genesis of the injury with high-speed cameras.
METHODS: 4 formalin-fixed cadaveric specimens of human upper extremities were tested in a prototype, custom-made, drop-weight test bench. An axial high-energy impulse was applied and the development of the lesion was documented with 3 high-speed cameras.
RESULTS: The high-speed images showed a transversal movement of the radius and ulna, which moved away from each other in the transversal plane during the im

OBJECTIVE: Physicians typically receive little continuing medical education (CME) about their role in workplace injury management as well as on workplace injuries and disease. Although new technologies may help educate physicians in these areas, careful evaluation is required, given the understudied nature of these interventions. The objective of this study is to evaluate two promising new technologies to deliver CME (online learning and videoconferencing) and to compare the effectiveness of these delivery methods to traditional CME interventions (large urban traditional conference lectures an

Although elbow dislocations are common orthopaedic lesions, vascular complications remain rare. We report the cases of three patients who presented with a rupture of the brachial artery after closed posterior dislocation, which is even more uncommon. Arteriograms were performed in all cases because of the persistent absence of pulses at the wrist after emergency reduction. In each patient, the treatment consisted of the insertion of reversed end-to-end saphenous bridges. None of them presented mid-term vascular complications (mean follow-up, 17 months). Brachial artery disruption can result fr

Long term results of children with supracondylar humeral fractures treated with manipulation and strapping and manipulation followed by pin fixation were evaluated. Forty patients were regarded as Gartland type II injuries. 33 of these were treated with closed reduction and collar and cuff immobilisation and 7 with closed reduction and percutaneous pinning. Two cases of cubitus varus were reported one from each treatment modality. Forty-four patients were included as Gartland type III injuries. Of these 14 were treated with closed reduction and collar and cuff immobilisation, 25 with closed re
