Acquired heterotopic ossification (HO) is a painful and debilitating disease characterized by extraskeletal bone formation after injury. The exact pathogenesis of HO remains unknown. Here we show that TGF-β initiates and promotes HO in mice. We find that calcified cartilage and newly formed bone resorb osteoclasts after onset of HO, which leads to high levels of active TGF-β that recruit mesenchymal stromal/progenitor cells (MSPCs) in the HO microenvironment. Transgenic expression of active TGF-β in tendon induces spontaneous HO, whereas systemic injection of a TGF-β neutralizing antibody atte

PURPOSE: Injury patterns of occupants in motor vehicle crashes are changing, with upper extremity injury becoming more common in patients treated at trauma centres. Although not life threatening, upper extremity injuries may result in long-term disability, including chronic deformity, neurovascular compromise and degenerative arthritis. The purpose of this study was to compare upper extremity injury in drivers and passengers using the Crash Injury Research Engineering Network (CIREN) database.
METHODS: CIREN data were used to compare upper extremity skeletal injury patterns and sources for dr

INTRODUCTION: Few multicenter studies have analyzed the outcome of revision surgery of radial head arthroplasties (RHA) in the medium term follow up. The objective is twofold: to determine the factors associated with revision of RHAs and to analyze the results of revision with 2 surgical techniques: isolated removal of the RHA or revision with a new RHA (R-RHA).
HYPOTHESIS: There are associated factors of RHA revision and RHA revision results in satisfactory clinical and functional outcomes.
METHODS: Twenty-eight patients were included in this multicenter retrospective study, with all surgic

We have extensively used arm cycling to study the neural control of rhythmic movements such as arm swing during walking. Recently rhythmic movement of the arms has also been shown to enhance and shape muscle activity in the legs. However, restricted information is available concerning the conditions necessary to maximally alter lumbar spinal cord excitability. Knowledge on the neuromechanics of a task can assist in the determination of the type, level, and timing of neural signals, yet arm swing during walking and arm cycling have not received a detailed neuromechanical comparison. The purpose

The prevalence of psoriasis is 2% of the world's population (1). Inverse psoriasis is characterized by the development of erythematous shiny plaques at intertriginous areas of the body. The prevalence of only anogenital involvement appears to be low, but involvement of the anogenital area together with other areas is found in up to 45% of patients with psoriasis (2). A 21-year-old female student with a 3-month history of mild psoriasis (erythematosquamous plaque on the elbows and nail pitting on the nails of the hand) was referred to our Department. One month earlier, suddenly appearance of er

Supracondylar humerus fractures are the most common pediatric elbow fractures and may rarely be associated with vascular compromise. We present two pediatric cases of displaced left supracondylar humerus fractures complicated by arterial injury. A four-year-old boy developed absent radial and ulnar pulses following fracture fixation and underwent urgent thrombectomy with Fogarty catheterization using a Fogarty arterial embolectomy catheter (Edwards Lifesciences, Irvine, CA, USA), resulting in immediate restoration of distal perfusion. An 11-year-old girl presented with brachial artery entrapme

Mesenteric shear, injury due to sudden stretch of the mesentery, is a rare condition that remains poorly studied, especially in the pediatric population. Often resulting from trauma, its presentation can vary from nonspecific and vague abdominal symptoms to an acute abdomen and peritonitis requiring urgent surgical intervention. Unified management strategies are not yet in place. We describe the case of a 10-year-old boy who presented to the emergency department with diffuse abdominal pain of 1 day duration, 2 days after he sustained blunt abdominal trauma with an elbow during a soccer match.

A 74-year-old woman with rheumatoid arthritis had developed severe pain in her left elbow 7 years previously and undergone joint replacement. Recently, the joint replacement surgery had to be repeated, and 3 months later, she was hospitalized because of rapid deterioration of kidney function, blurred vision, and eye pain. Levels of serum albumin-adjusted calcium (10.6 mg/dL), serum 1,25-dihydroxyvitamin D3 (115 pg/mL), and lysozyme (27.5 μg/mL) were elevated, and kidney function was impaired (serum creatinine, 2.48 mg/dL). Biopsies of periarticular tissue and a left axillary lymph node reveale

INTRODUCTION: Engaging Hill-Sachs lesions in recurrent anterior shoulder dislocation have been managed with the remplissage procedure. Clinical and cadaveric studies have reported limitation of rotation after this procedure. We introduce the reverse McLaughlin procedure where the infraspinatus and the underlying capsule are detached and approximated into the Hill-Sachs defect with transosseous sutures. This is a preliminary report using this technique.
METHODS: Seventeen patients with recurrent anterior shoulder dislocations and an engaging Hill-Sachs lesion underwent a Bankart repair and rem

Ambiguous findings during external examination of a deceased in combination with dubious autopsy findings can raise doubts concerning the manner and cause of death. We report the case of a 35-year-old female deceased who had suffered from a borderline personality and depressive disorder with suicidal ideation. At the death scene, the body showed massive facial swelling accompanied by complete reddening of the skin of the face, with patchy skin abrasions on the forehead and neck, and purple bruise-like discolorations distributed symmetrically over both shoulders, elbows, hands, hips, knees, low

Some comminuted or segmental humeral fractures have been well-treated by functional bracing, but some also have been reported to have a bad outcome. If surgery is required, unlocked nailing has the disadvantage of unstable fixation, and plating has the disadvantage of extensive soft tissue injury. In the current study, closed locked nailing was used to treat 23 Orthopaedic Trauma Association Type C humeral fractures (C1, five; C2, three; C3, 15). The patients were 12 men and 11 women with a mean age of 42.4 years. There were 16 closed fractures and four Type I, two Type II, and one Type IIIA o

Upper trunk (C5-C6) injuries of the brachial plexus impair shoulder abduction and elbow flexion. For C5-C6 ruptures with preserved proximal stumps, nerve grafting (NG) remains the conventional approach. Nerve transfers (NT) represent the sole viable option in C5-C6 avulsions. The current global standard for NT involves triple transfer procedures: spinal accessory nerve (SAN) to suprascapular nerve(SSN), Oberlin procedure, and Leechavengvongs procedure. Recent clinical trends indicate a broadening of NT indications to include root rupture scenarios, driven by the rationale that distal NT signif

BACKGROUND: Coronoid process fractures, which comprise 10-15% of elbow injuries, are critical contributors to joint instability but remain poorly characterised in three dimensions (3D), particularly at the fragment level. This study addressed this gap by generating 3D fracture and heat maps of O'Driscoll Type 1 and Type 2 coronoid fractures to improve the understanding of coronoid process morphology, fracture patterns and inform device designs.
METHODS: Thirty-three unilateral CT scans underwent in-silico reduction using Quantitative 3D Computed Tomography (Q3DCT). Fracture lines were overlai…
