Provider Preferences, Perceptions, and Barriers to Colorectal Cancer Screening in Federally Qualified Health Centers: A Cross-Sectional Survey.

BACKGROUND: Colorectal cancer (CRC) screening reduces mortality, yet uptake varies by screening modality, provider type, and clinical setting. Federally Qualified Health Centers (FQHCs) care for underserved populations and face unique challenges, including limited resources, diverse care needs, and competing clinical priorities. Despite the importance of screening, provider preferences for CRC screening modalities are not well understood. METHODS: We conducted a cross-sectional survey of primary care providers (physicians, nurse practitioners, physician assistants) at four FQHCs in Wisconsin.

Alternative Splicing Constitutes a Transcription-independent Regulatory Layer in Replicative Senescence of Human Umbilical Cord Mesenchymal Stem Cells.

Mesenchymal stem cells (MSCs) inevitably undergo replicative senescence during in vitro expansion, leading to progressive loss of therapeutic efficacy. However, the global landscape of alternative splicing (AS) during this process and its relationship with transcriptional regulation remain uncharacterized. Here, we performed splicing-aware transcriptomic profiling of human umbilical cord derived MSCs across four passage points (P2, P8, P10, P12; n = 11). Using stringent AND criteria (FDR < 0.05, |ΔPSI| ≥ 0.05), we identified 112 differential splicing events (DSEs) affecting 96 genes. Skipped e

Mapping the evidence on post-intensive care syndrome in paediatric populations: A scoping review protocol.

OBJECTIVE: This scoping review aims to comprehensively map the primary literature on post-intensive care syndrome in paediatric populations (PICS-p), across all recognised PICS-p domains, with a focus on diagnostic methodologies, preventive and therapeutic interventions, and reported post-ICU outcomes among children aged 1 month-18 years. INTRODUCTION: Post-intensive care syndrome (PICS) encompasses new or worsening physical, cognitive, psychological, or social impairments emerging after critical illness. While extensively studied in adults, its paediatric counterpart, PICS-p, remains under-r

Clinical characteristics and socioeconomic determinants of unintentional childhood injuries: An emergency department perspective.

BACKGROUND: Unintentional childhood injuries (UCIs) are a leading cause of morbidity and mortality among children globally, imposing significant clinical and economic burdens, particularly in low- and middle-income countries. Emergency Departments (EDs) serve not only as the first point of contact for such events but also as the initial entry point for non-fatal UCIs, which represent a hidden and more substantial burden on health services. The primary objective of this study is to comprehensively analyze the clinical and socioeconomic determinants and predictors of unintentional childhood inju

"Then you are left on your own": barriers to guideline-based speech and language therapy for aphasia along the patient journey in the German-speaking healthcare context.

BACKGROUND: Although evidence-based recommendations for aphasia rehabilitation exist, guideline-based speech and language therapy is still not consistently implemented in routine care. Previous studies have described barriers to therapy participation from the perspective of people with aphasia, but less is known about how these barriers develop along the patient journey, especially in outpatient care. This study explored systemic and individual barriers to the uptake of speech and language therapy in aphasia care and identified implications for improving services. METHODS: An exploratory qual

A Bidirectional C1q/TNF-Related Protein Signature Is Associated with Disease Activity in Behçet's Disease: A Cross-Sectional Study.

Background/Objectives: Distinguishing active from quiescent Behçet's disease (BD) is difficult because conventional acute-phase reactants perform inconsistently. C1q/TNF-related proteins (CTRPs) are adiponectin paralogues; several members have been reported to exert opposing immunometabolic actions in experimental systems, although the evidence is uneven across the family and no member has been examined in BD. Methods: In this cross-sectional study, eight CTRPs (CTRP1, CTRP4, CTRP6, CTRP9, CTRP12, CTRP13, CTRP14, CTRP15) were measured in serum using enzyme-linked immunosorbent assay in 30 pati

Fecal microbiota transplantation alleviates DSS-induced colitis: increased fecal butyrate, reduced colonic p65 phosphorylation, and altered Th17/Treg ratios in the spleen and mesenteric lymph nodes.

BACKGROUND: Ulcerative colitis (UC) development and progression are associated with intestinal dysbiosis, altered short-chain fatty acid (SCFA) metabolism, and immune dysregulation. Although fecal microbiota transplantation (FMT) has therapeutic potential, its key effector metabolites and regulatory pathways remain unclear. METHODS: Male BALB/c mice with 2.5% dextran sulfate sodium (DSS)-induced colitis received FMT, 5-aminosalicylic acid (5-ASA), sodium butyrate (NaB), or pyrrolidine dithiocarbamate (PDTC). Colitis severity was evaluated using body weight, disease activity index, colon lengt

Effects of dynamic taping and moderate-intensity TheraBand training versus usual care on pain, disability, and well-being in adult National Cadet Corps with chronic heel pain: a pilot and feasibility trial.

INTRODUCTION: Identifying appropriate pre-exposure prophylaxis (PrEP) candidates is a challenge in planning for the safe and effective roll-out of this strategy. We explored the use of a validated HIV risk screening tool, HIV Incidence Risk Index for Men who have Sex with Men (HIRI-MSM), to identify "optimal" candidates among MSM testing at a busy sexual health clinic's community testing sites in Toronto, Canada. METHODS: Between November 2014 and April 2015, we surveyed MSM undergoing anonymous HIV testing at community testing sites in Toronto, Canada, to quantify "optimal" candidates for sc

Early alveolar molecular signatures after cardiopulmonary resuscitation: a bronchoalveolar lavage (BALF) proteomic study in swine.

BACKGROUND: Mechanical chest compressions during cardiopulmonary resuscitation (CPR) can produce pulmonary edema, described as cardiopulmonary resuscitation-associated lung edema (CRALE), which has been largely interpreted as a hydrostatic phenomenon. However, whether early molecular changes associated with epithelial injury, coagulation, and inflammation are also present during resuscitation remains uncertain. We hypothesized that bronchoalveolar lavage fluid proteomics would identify early protein signatures of lung injury after CPR. METHODS: Bronchoalveolar lavage fluid (BALF) was collecte

Digital Patient-Controlled Follow-Up in Palliative Cancer Care: Protocol for a Mixed Methods Feasibility Study.

BACKGROUND: Patient-centered care, emphasizing autonomy and shared decision-making, is essential in palliative cancer care. The increasing prevalence of cancer requires optimized health care use, and given patients' preference for home-based care, traditional time-based follow-up appointments may not adequately address their needs. We have developed a digital patient-controlled follow-up intervention at the acute palliative care unit in Norway. The digital app is integrated into the existing national health service platform, MyHealth, and facilitates symptom monitoring, self-management support

Abomasal infusion of docosahexaenoic acid to mid-lactation cows alters circulating leukocyte profiles and affects immune response to ex vivo bacterial stimulation.

n-3 Fatty acids have anti-inflammatory effects across species, and within the n-3 fatty acid family, different fatty acids have unique effects on immune responses. The objective of this study was to assess circulating leukocyte phenotypes and their ex vivo immune response to challenge following abomasal infusions of the n-3 fatty acid docosahexaenoic acid (DHA; C22:6). Eight mid-lactation Holstein cows (97 ± 37 DIM) were randomly assigned to treatment sequence in a 4 × 4 Latin square design. Treatments were 0, 2, 4, and 6 g/d DHA (supplied via an enriched oil containing 65% DHA) infused into t

Methylated CfDNA may distinguish between high- and intermediate-risk uveal melanoma: a pilot study.

BACKGROUND: Uveal melanoma (UM) is a highly aggressive malignancy with a metastatic risk that depends on the molecular subclass. This subclass can be determined through molecular characterization of tumor-derived tissue. With eye-sparing treatments, tumor tissue is rarely available for molecular testing. We hypothesized that minimal invasive biomarkers such as methylated cell-free DNA (cfDNA) or circulating tumor DNA (ctDNA) can be used for prognosis and monitoring of patients. METHODS: Plasma cfDNA was isolated from healthy blood donors (HBDs, N = 19) and UM patients (N = 22). Plasma was col

Genetic Predisposition to Low-Density Lipoprotein Cholesterol and Incident Type 2 Diabetes.

IMPORTANCE: Treatment to lower high levels of low-density lipoprotein cholesterol (LDL-C) reduces incident coronary artery disease (CAD) risk but modestly increases the risk for incident type 2 diabetes (T2D). The extent to which genetic factors across the cholesterol spectrum are associated with incident T2D is not well understood. OBJECTIVE: To investigate the association of genetic predisposition to increased LDL-C levels with incident T2D risk. DESIGN, SETTING, AND PARTICIPANTS: In this large prospective, population-based cohort study, UK Biobank participants who underwent whole-exome se

An Adapted Cardioprotective Diet with or Without Phytosterol and/or Krill Oil Supplementation in Familial Hypercholesterolemia: Results of a Pilot Randomized Clinical Trial.

Background/Objectives: Familial hypercholesterolemia (FH) is an increasingly common inherited disorder that increases cardiovascular risk. Despite the importance of lifestyle interventions, adherence to a healthy diet among individuals with FH remains suboptimal. This pilot, multicenter, double-blind, placebo-controlled randomized trial aimed to evaluate the feasibility and preliminary effects of a culturally adapted cardioprotective diet (DICA-FH), alone or in combination with phytosterol and/or krill oil supplementation, on lipid parameters in Brazilian adults with probable or definitive FH.

[Construction and identification of a sizeable naive human Fab phage display antibody library].

Objective: To construct a sizeable naive human Fab phage display antibody library to screen high-affinity specific antibodies in vitro. Methods: Total RNA was extracted from peripheral blood mononuclear cells (PBMCs) of 126 healthy individuals, subsequently reverse-transcribed into cDNA, and used as a template. PCR amplification was performed to obtain the VH from IgG, IgM and light chain κ, λ, separately, with the initial PCR products serving as templates for a second round of PCR. Overlap extension PCR was employed to generate fragments of the κ and λ light chains. These fragments were ligat

Whole-Brain Analysis of Increased Brain Iron in Patients with β-Thalassemia.

RATIONALE AND OBJECTIVES: β-thalassemia (β-TM) is an inherited hematological disorder characterized by impaired hemoglobin synthesis, leading to chronic anemia and systemic iron overload. While numerous studies have documented widespread iron deposition caused by repeated blood transfusions and disrupted iron metabolism, the impact of iron overload on the brain remains poorly understood. This study aimed to investigate whole-brain iron accumulation in patients with β-TM using quantitative susceptibility mapping (QSM). METHODS: A total of 88 patients with transfusion-dependent β-TM and 57 age

Examining the impact of gestational diabetes genetic susceptibility variants on maternal glucose levels during and post pregnancy.

AIM: Genetic variants associated with gestational diabetes mellitus (GDM, n=14 SNPs) were recently classified into two groups: type 2 diabetes predominant effects (Class-T, three SNPs) and GDM-predominant effects (Class-G, eight SNPs; three SNPs unclassified). We aimed to compare the effects of GDM-associated variants on glucose levels (fasting glucose and 2-hour post-OGTT) measured during versus post pregnancy. RESEARCH DESIGN AND METHODS: We calculated genetic scores (GS) by class (T_GS and G_GS) and overall (All_GS) in 10 225 pregnant women and 4763 women post pregnancy (mean 10.5 years po

Using a Hand-Held Icterometer to Screen for Neonatal Jaundice: Validation, Feasibility, and Acceptability of the Bili-RulerTM in Kumasi, Ghana.

BACKGROUND: Neonatal jaundice (NNJ) remains a leading cause of newborn mortality in much of sub-Saharan Africa. We sought to examine the validity of using a hand-held icterometer as a screening tool to determine which newborns need further assessment. Additionally, we sought to assess the feasibility of its use among mothers. METHODS: We recruited and trained healthcare workers at one large district hospital in Ghana to use a hand-held icterometer known as the Bili-RulerTM. We recruited mothers of 341 newborns aged 0 to 2 weeks at the same hospital. Mothers watched a standardized training vid
