Management for chordoma of the spine and sacrum with stereotactic radiosurgery.

OBJECTIVE: Resection for spinal and sacral chordoma is associated with significant morbidity and high rates of local recurrence. Stereotactic radiosurgery (SRS) offers radiobiological benefits over conventional radiation therapy and has emerged as a viable treatment option for these historically radioresistant tumors. There are relatively few studies that have reported on the treatment of spinal chordoma with SRS, given its rarity. Thus, the aim of this study was to identify long-term outcomes after SRS in patients with spinal and sacral chordoma. METHODS: A prospectively collected database a

Benign Breast Disease and Breast Cancer Risk in the Percutaneous Biopsy Era.

IMPORTANCE: Benign breast disease (BBD) comprises approximately 75% of breast biopsy diagnoses. Surgical biopsy specimens diagnosed as nonproliferative (NP), proliferative disease without atypia (PDWA), or atypical hyperplasia (AH) are associated with increasing breast cancer (BC) risk; however, knowledge is limited on risk associated with percutaneously diagnosed BBD. OBJECTIVES: To estimate BC risk associated with BBD in the percutaneous biopsy era irrespective of surgical biopsy. DESIGN, SETTING, AND PARTICIPANTS: In this retrospective cohort study, BBD biopsy specimens collected from Jan

Purulent Inflatable Penile Prostheses Can Be Safely Immediately Salvaged in Insulin-Dependent Diabetics.

INTRODUCTION: Historically, contra-indications for immediate salvage of an infected penile prosthesis have included purulence on the device, purulent infections in diabetic patients, and severe diabetes. These clinical principles are from the 1990s to early 2000s and based on limited retrospective series or expert opinion. Since the publication of these contra-indications there have been improvements in device design, surgical technique, corporal washout protocols, and perioperative antibiotic prophylaxis that allow us to re-evaluate whether we can broaden the criteria for immediate salvage of

Selective intradural dorsal rhizotomy for persistent radicular leg pain: a contemporary series.

BACKGROUND: Lumbar disc surgery for radicular leg pain is one of the most frequently performed spine procedures. In approximately 20% of patients poor outcome is achieved. The most complex cases have persistent leg pain without residual nerve root compression. Treatment for refractory cases is limited to medical pain management, spinal cord stimulation (SCS), and dorsal root ganglion stimulation. For the latter two, fair to good results are obtained in only 50% of patients and costs and complication rates are high. An alternative surgical procedure is selective intradural dorsal rhizotomy (SID

Prevention of everolimus-related stomatitis in women with hormone receptor-positive, HER2-negative metastatic breast cancer using dexamethasone mouthwash (SWISH): a single-arm, phase 2 trial.

BACKGROUND: Stomatitis is a class effect associated with the inhibition of mTOR and is associated with everolimus therapy for breast cancer. Topical steroids might reduce stomatitis incidence and severity, and the need for dose reductions and interruptions of everolimus. Anecdotal use of topical steroid oral prophylaxis has been reported in patients with breast cancer. We aimed to assess dexamethasone-based mouthwash for prevention of stomatitis in patients with breast cancer. METHODS: This US-based, multicentre, single-arm, phase 2 prevention study enrolled women aged 18 years and older with

[A cohort study on the characteristics of the recurrent epidemics on hand, foot and mouth disease, in Fujian province].

OBJECTIVE: To explore the recurrent epidemiological characteristics of hand, foot and mouth disease (HFMD) among children aged <4 years to provide evidence for HFMD prevention and control. METHODS: Principles on historical cohort study were followed when analyzing data related to HFMD surveillance in Fujian province. All the research objects were restricted to patients aged <4, with HFMD and who were permanent residents in Fujian province. Characteristics of the study objects were extracted as potential factors when the patients first showed symptoms of HFMD. These factors might cause the rec

Final report on the safety assessment of trilaurin, triarachidin, tribehenin, tricaprin, tricaprylin, trierucin, triheptanoin, triheptylundecanoin, triisononanoin, triisopalmitin, triisostearin, trilinolein, trimyristin, trioctanoin, triolein, tripalmitin, tripalmitolein, triricinolein, tristearin, triundecanoin, glyceryl triacetyl hydroxystearate, glyceryl triacetyl ricinoleate, and glyceryl stearate diacetate.

Triesters of glycerin and aliphatic acids, known generically as glyceryl triesters and specifically as Trilaurin, etc., are used in cosmetic products as occlusive skin-conditioning agents and/or nonaqueous viscosity-increasing agents. Hundreds of glyceryl triesters are used in a wide variety of cosmetic products at concentrations ranging from a few tenths of a percent to 46%. Glyceryl triesters are also known as triglycerides; ingested triglycerides are metabolized to monoglycerides, free fatty acids, and glycerol, all of which are absorbed in the intestinal mucosa and undergo further metaboli

Timing of radiotherapy and autologous stem cell transplantation in embryonal tumor with multilayered rosettes: a critical narrative review.

PURPOSE: Embryonal tumor with multilayered rosettes (ETMR) carries a median overall survival of approximately 10-12 months despite maximal-safe resection, high-dose chemotherapy (HDCT) with autologous stem cell transplantation (ASCT), and radiotherapy (RT). RT has historically been deferred or omitted in these predominantly infant-age patients owing to concerns over neurodevelopmental toxicity, with HDCT/ASCT serving as the principal consolidation strategy. More recent data have called this approach into question. This narrative review synthesizes the evidence on the timing of RT relative to H

Addressing racial inequality and its effects on vaccination rate: A trial comparing a pharmacist and peer educational program (MOTIVATE) in diverse older adults.

BACKGROUND: The mortality, morbidity, health care utilization, and cost attributable to vaccine-preventable diseases are substantial for those aged 50 years and older. Although vaccination is the most cost-effective strategy to prevent common infectious diseases in older adults, vaccination rates remain below US Centers for Disease Control and Prevention benchmarks, especially among racial minorities. Historical mistrust, structural racism within the US medical system, and misinformation contributed to lower immunization rates among minorities, especially Black Americans. To address the critic

Immunogenicity, safety, and efficacy of the HPV vaccines among people living with HIV: A systematic review and meta-analysis.

BACKGROUND: Vaccines have been demonstrated to protect against high-risk human papillomavirus infection (HPV), including HPV-16/18, and cervical lesions among HIV negative women. However, their efficacy remains uncertain for people living with HIV (PLHIV).We systematically reviewed available evidence on HPV vaccine on immunological, virological, or other biological outcomes in PLHIV. METHODS: We searched five electronic databases (PubMed, Medline and Embase, clinicaltrials.gov and the WHO clinical trial database) for longitudinal prospective studies reporting immunogenicity, virological, cyto

Walk in our shoes: older homeless women's perceptions of the challenges of navigating streets and shelter life.

INTRODUCTION: Older women experiencing homelessness (OWEH) have been a hidden or invisible population; however, with rising numbers globally, they are gaining attention. Our study examines what it means to be a woman in her 50s who is struggling financially and precariously housed in the United States. Although in their 50s, these women often experience accelerated aging and exhibit chronic health conditions comparable to those of housed women in their 70s and 80s. Lacking access to both public old-age benefits and family support, they often fall between the cracks of the nation's safety net s

On Temporal Patterns and Circulation of Influenza Virus Strains in Taiwan, 2008-2014: Implications of 2009 pH1N1 Pandemic.

BACKGROUND: It has been observed that, historically, strains of pandemic influenza led to succeeding seasonal waves, albeit with decidedly different patterns. Recent studies suggest that the 2009 A(H1N1)pdm09 pandemic has had an impact on the circulation patterns of seasonal influenza strains in the post-pandemic years. In this work we aim to investigate this issue and also to compare the relative transmissibility of these waves of differing strains using Taiwan influenza surveillance data before, during and after the pandemic. METHODS: We make use of the Taiwan Center for Disease Control and

Chapter 2. Fasciola, lymnaeids and human fascioliasis, with a global overview on disease transmission, epidemiology, evolutionary genetics, molecular epidemiology and control.

Fascioliasis, caused by liver fluke species of the genus Fasciola, has always been well recognized because of its high veterinary impact but it has been among the most neglected diseases for decades with regard to human infection. However, the increasing importance of human fascioliasis worldwide has re-launched interest in fascioliasis. From the 1990s, many new concepts have been developed regarding human fascioliasis and these have furnished a new baseline for the human disease that is very different to a simple extrapolation from fascioliasis in livestock. Studies have shown that human fasc

Impact of climate warming on Oncomelania hupensis in China: multi-scale evidence.

BACKGROUND: Climate warming is increasingly reshaping the population dynamics of Oncomelania hupensis, the sole intermediate host of Schistosoma japonicum in China. Investigating the response of O. hupensis to climate warming is crucial for guiding targeted and precise interventions against these snails, thereby advancing the progress towards schistosomiasis elimination and sustaining the achievement of elimination in China. This study aims to quantify the multi-scale responses of O. hupensis to climate warming. METHOD: We conducted a 45-day temperature-controlled laboratory experiment with a

Neutralising antibody responses to MPXV clades Ia, Ib, and IIb after infection or vaccination: a multicountry observational study.

BACKGROUND: The 2022 global outbreak of monkeypox virus (MPXV) clade IIb prompted widespread use of the modified vaccinia Ankara-Bavarian Nordic (MVA-BN) vaccine which conferred partial protection and contributed to outbreak control. In 2024, the emergence and regional expansion of MPXV clade Ib in central Africa raised new public health concerns and prompted questions about the extent to which immunity elicited by previous clade IIb infection or vaccination confers cross-neutralising immunity against clade Ib. We aimed to assess neutralising and binding antibody responses to MPXV clades Ia, I

Vaccines for preventing influenza in the elderly.

BACKGROUND: The consequences of influenza in the elderly (those age 65 years or older) are complications, hospitalisations, and death. The primary goal of influenza vaccination in the elderly is to reduce the risk of death among people who are most vulnerable. This is an update of a review published in 2010. Future updates of this review will be made only when new trials or vaccines become available. Observational data included in previous versions of the review have been retained for historical reasons but have not been updated because of their lack of influence on the review conclusions. OB

Pharmacodynamic substance discovery and mechanism of action of Tibetan medicine Ba-Wei-Chen-Xiang San in spontaneously hypertensive rats.

BACKGROUND: Ba-Wei-Chen-Xiang San (BWCX), a classic formula in Traditional Tibetan Medicine (TTM), has been historically employed for preventing and treating "Chalong" disorders, which correspond to cardiovascular diseases in modern medicine. Previous literature mainly describes BWCX as a cardioprotective agent that can improve myocardial hypertrophy by regulating cardiac energy metabolism pathways. But the specific pharmacological mechanism by which it regulates the systemic metabolic drivers of essential hypertension still needs to be studied. AIM OF THE STUDY: This study aimed to systemati
