Predicting atrial fibrillation episodes with rapid ventricular rates associated with low levels of activity.

BACKGROUND: Rapid and irregular ventricular rates (RVR) are an important consequence of atrial fibrillation (AF). Raw accelerometry data in combination with electrocardiogram (ECG) data have the potential to distinguish inappropriate from appropriate tachycardia in AF. This can allow for the development of a just-in-time intervention for clinical treatments of AF events. The objective of this study is to develop a machine learning algorithm that can distinguish episodes of AF with RVR that are associated with low levels of activity. METHODS: This study involves 45 patients with persistent or

Diagnostic performance of classification trees and hematological functions in hematologic disorders: an application of multidimensional scaling and cluster analysis.

BACKGROUND: Several hematological indices have been already proposed to discriminate between iron deficiency anemia (IDA) and β-thalassemia trait (βTT). This study compared the diagnostic performance of different hematological discrimination indices with decision trees and support vector machines, so as to discriminate IDA from βTT using multidimensional scaling and cluster analysis. In addition, decision trees were used to determine the diagnostic classification scheme of patients. METHODS: Consisting of 1178 patients with hypochromic microcytic anemia (708 patients with βTT and 470 patients

Connected diagnostics to improve accurate diagnosis, treatment, and conditional payment of malaria services in Kenya.

BACKGROUND: In sub-Saharan Africa, the material and human capacity to diagnose patients reporting with fever to healthcare providers is largely insufficient. Febrile patients are typically treated presumptively with antimalarials and/or antibiotics. Such over-prescription can lead to drug resistance and involves unnecessary costs to the health system. International funding for malaria is currently not sufficient to control malaria. Transition to domestic funding is challenged by UHC efforts and recent COVID-19 outbreak. Herewith we present a digital approach to improve efficiencies in diagnosi

Two-Way MR-Forest Based Growing Path Classification for Malignancy Estimation of Pulmonary Nodules.

This paper proposes a two-way multi-ringed forest (TMR-Forest) to estimating the malignancy of the pulmonary nodules for false positive reduction (FPR). Based on our previous work of deep decision framework, named MR-Forest, we generate a growing path mode on predefined pseudo-timeline of L time slots to build pseudo-spatiotemporal features. It synchronously works with FPR based on MR-Forest to help predict the labels from a dynamic perspective. Concretely, Mask R-CNN is first used to recommend the bounding boxes of ROIs and classify their pathological features. Afterward, hierarchical attribu

A personalized decision aid for prostate cancer shared decision making.

BACKGROUND: A shared decision-making model is preferred for engaging prostate cancer patients in treatment decisions. However, the process of assessing an individual's preferences and values is challenging and not formalized. The purpose of this study is to develop an automated decision aid for patient-centric treatment decision-making using decision analysis, preference thresholds and value elicitations to maximize the compatibility between a patient's treatment expectations and outcome. METHODS: A template for patient-centric medical decision-making was constructed. The inputs included pros

Effective Clustering for Single Cell Sequencing Cancer Data.

Single cell sequencing (SCS) technologies provide a level of resolution that makes it indispensable for inferring from a sequenced tumor, evolutionary trees or phylogenies representing an accumulation of cancerous mutations. A drawback of SCS is elevated false negative and missing value rates, resulting in a large space of possible solutions, which in turn makes it difficult, sometimes infeasible using current approaches and tools. One possible solution is to reduce the size of an SCS instance - usually represented as a matrix of presence, absence, and uncertainty of the mutations found in the

Validation of Inertial Sensing-Based Wearable Device for Tremor and Bradykinesia Quantification.

Neurologists judge the severity of Parkinsonian motor symptoms according to clinical scales, and their judgments exist inconsistent because of differences in clinical experience. Correspondingly, inertial sensing-based wearable devices (ISWDs) produce objective and standardized quantifications. However, ISWDs indirectly quantify symptoms by parametric modeling of angular velocities and linear accelerations nd trained by the judgments of several neurologists through supervised learning algorithms. Hence, the ISWD outputs are biased along with the scores provided by neurologists. To investigate

Framework for Personalized Real-Time Control of Hidden Temperature Variables in Therapeutic Knee Cooling.

The paper formalizes, implements and evaluates a framework for personalized real-time control of inner knee temperature during cryotherapy after knee surgery. Studies have shown that the cryotherapy should be controlled depending on the individual patient's feedback on the cooling, which raises the need for smart personalized therapy. The framework is based on the feedback control loop that uses predicted instead of measured inner temperatures because measurements are not feasible or would introduce invasiveness into the system. It uses machine learning to construct a predictive model for esti

Fetal birthweight prediction with measured data by a temporal machine learning method.

BACKGROUND: Birthweight is an important indicator during the fetal development process to protect the maternal and infant safety. However, birthweight is difficult to be directly measured, and is usually roughly estimated by the empirical formulas according to the experience of the doctors in clinical practice. METHODS: This study attempts to combine multiple electronic medical records with the B-ultrasonic examination of pregnant women to construct a hybrid birth weight predicting classifier based on long short-term memory (LSTM) networks. The clinical data were collected from 5,759 Chinese

A Lightweight CNN Model for Detecting Respiratory Diseases From Lung Auscultation Sounds Using EMD-CWT-Based Hybrid Scalogram.

Listening to lung sounds through auscultation is vital in examining the respiratory system for abnormalities. Automated analysis of lung auscultation sounds can be beneficial to the health systems in low-resource settings where there is a lack of skilled physicians. In this work, we propose a lightweight convolutional neural network (CNN) architecture to classify respiratory diseases from individual breath cycles using hybrid scalogram-based features of lung sounds. The proposed feature-set utilizes the empirical mode decomposition (EMD) and the continuous wavelet transform (CWT). The performa

Automatic Acetowhite Lesion Segmentation via Specular Reflection Removal and Deep Attention Network.

Automatic acetowhite lesion segmentation in colposcopy images (cervigrams) is essential in assisting gynecologists for the diagnosis of cervical intraepithelial neoplasia grades and cervical cancer. It can also help gynecologists determine the correct lesion areas for further pathological examination. Existing computer-aided diagnosis algorithms show poor segmentation performance because of specular reflections, insufficient training data and the inability to focus on semantically meaningful lesion parts. In this paper, a novel computer-aided diagnosis algorithm is proposed to segment acetowhi

An OMOP-CDM based pharmacovigilance data-processing pipeline (PDP) providing active surveillance for ADR signal detection from real-world data sources.

BACKGROUND: Adverse drug reactions (ADRs) are regarded as a major cause of death and a major contributor to public health costs. For the active surveillance of drug safety, the use of real-world data and real-world evidence as part of the overall pharmacovigilance process is important. In this regard, many studies apply the data-driven approaches to support pharmacovigilance. We developed a pharmacovigilance data-processing pipeline (PDP) that utilized electronic health records (EHR) and spontaneous reporting system (SRS) data to explore pharmacovigilance signals. METHODS: To this end, we int
