Civilian popliteal artery trauma: an eleven year experience with 83 injuries.

Limb loss is the outcome in one third of previously reported popliteal artery injuries. This report summarizes 83 injuries with an amputation rate of 9.6%. Penetrating traumas accounted for 61 (73%) injuries and blunt traumas for 22 (27%). The incidence of amputation varied with injury type from none in seven stab wounds to three of 19 (15.8%) shotgun wounds. Distal ischemia or a pulse deficit highlights the presence of arterial trauma, and the external wound defines its site in most patients. Urgent operation is indicated by these findings. Equivocal findings suggest less compromised flow, an

Vascular injuries associated with fractures of the femur.

Over a five-year period, 21 patients were seen with vascular injuries associated with fractures of the femur. In 19, vascular repair was performed at the time of the acute injury. The average interval from injury to repair was 15 hours. Arterial injuries included transection, intimal flap, laceration, avulsion, and false aneurysm. End-to-end repair was done in 11 cases, and autogenous vein grafting in nine. One artery was ligated for false aneurysm. In the 19 acute cases, internal fixation was used in eight; this method of immobilization resulted in two major amputations and three cases of ant

Awake Sciatic Nerve Neurolysis after Injection Injury - An Innovative Technique and Our Experience.

BACKGROUND AND INTRODUCTION: Sciatic nerve injection injury is the most common type of injection nerve injury. Injection injury to nerve causes severe, intractable radicular pain in affected limb. Owing to chemical damage, nerve is damaged both mechanically and chemically. As a result, adhesions and nerve fibers damage is more than expected. Surgical neurolysis is recommended for pain relief. However it is done in general anesthesia and effect of neurolysis is checked only after reversal. OBJECTIVE: There are chances that incomplete neurolysis can cause persistence of pain. To overcome this l

Soccer and volleyball players do not land differently: implications for anterior cruciate ligament injury risk.

BACKGROUND: Non-contact anterior cruciate ligament injuries are common in soccer and volleyball, occurring during changes of direction and landings. This study aimed to investigate kinematic differences between soccer and volleyball players in single-planar and multiplanar landing tasks, simulating sport-specific injury mechanisms. Since the anterior cruciate ligament injury rate in soccer is higher than in volleyball, we hypothesized that volleyball players would adopt safer landing strategies, especially in single-planar landing tasks. METHODS: Twenty-two soccer and 19 volleyball players pe

Functional assessment and outcome following surgical treatment of displaced tibial plateau fractures: a retrospective analysis.

PURPOSE: Patients with tibial plateau fractures (TPF) are at risk of long-term hampered bipedal locomotion. A retrospective single-center study using patient-related outcome measures and a sophisticated assessment of walking abilities was conducted. METHODS: Adults receiving surgical treatment of an isolated TPF between January 2012 and December 2016 received the KOOS questionnaire together with the invitation for an extensive follow-up examination on the clinical outcome including standardized assessment of the walking abilities (loadsol® system). Outcome was assessed relative to the severit

A Case Series of Lymphatic Injuries After Suction Lipectomy in Women with Lipedema.

BACKGROUND Lipedema is a loose connective tissue disease characterized by disproportionate subcutaneous adipose tissue hypertrophy in the extremities. There is evidence of impaired lymphatic function in women with lipedema at all stages without signs of trophic skin changes associated with hereditary or acquired lymphedema. A modification of suction lipectomy is used to treat lipedema tissue and can reduce pain, limb size, and limb swelling and reduce the need for compression in women with lipedema. Studies have shown that modified liposuction can improve quality of life and mobility. There ar

Risk factors for major amputation after arterial vascular trauma of the lower extremity.

BACKGROUND & OBJECTIVE: Arterial vascular trauma bears a great risk of poor functional outcome or limb loss. The aim of this study was to analyze amputation rates of patients after vascular trauma and to perform a predictor analysis for the risk of major amputation. METHODS: In a single-center retrospective analysis of 119 extremities treated for arterial vascular injury between 1990 and 2018 amputation rates and factors associated with limb loss were assessed. All patients were treated for traumatic vascular injuries; iatrogenic injuries were not included in the study. RESULTS: During the s

Long-term paired associative stimulation can restore voluntary control over paralyzed muscles in incomplete chronic spinal cord injury patients.

Emerging therapeutic strategies for spinal cord injury aim at sparing or restoring at least part of the corticospinal tract at the acute stage. Hence, approaches that strengthen the weak connections that are spared or restored are crucial. Transient plastic changes in the human corticospinal tract can be induced through paired associative stimulation, a noninvasive technique in which transcranial magnetic brain stimulation is synchronized with electrical peripheral nerve stimulation. A single paired associative stimulation session can induce transient plasticity in spinal cord injury patients.

DNA Microarray Analysis in Screening Features of Genes Involved in Spinal Cord Injury.

BACKGROUND Spinal cord injury (SCI) is the most critical complication of spinal injury. We aimed to identify differentially expressed genes (DEGs) and to find associated pathways that may function as targets for SCI prognosis and therapy. MATERIAL AND METHODS Seven gene microarray expression profiles, downloaded from the GEO database (ID: GSE33886), were used to screen the DEGs of leg tissue and to compare these between SCI patients and corresponding normal specimens. Then, GO enrichment analysis was performed on these selected DEGs. Afterwards, interactions among these DEGs were analyzed by S

The absence of brachial plexus injury in stroke.

Brachial plexus injury and proximal mononeuropathy have been reported as a potential complications in the hemiplegic shoulder after a stroke. The diagnosis of brachial plexus injury and proximal mononeuropathy in the hemiplegic extremity is complicated by the upper motor neuron findings on physical examination and by the diffusely abnormal electrodiagnostic test results frequently seen in hemiplegic limbs. This study investigated the incidence of brachial plexus injury and proximal mononeuropathy after a thromboembolic stroke. Hemiplegic patients (n = 50) underwent physical examination, needle

Symmetrical "mirror-image" injuries and the "chessboard" pattern: useful markers of self-mutilation.

The evaluation of injuries due to self-mutilation may be complicated by atypical lesions and fabricated histories of the causative events. Four cases are described where one of the most striking findings was the presence of "mirror-image" injuries and/or a "chessboard" pattern of intersecting parallel lines from self-inflicted incised wounds. Case 1: A 32-year-old man was dead in his burnt-out car, with symmetrical, "mirror-image", circular burns from the car cigarette lighter over his forehead, cheeks, ears, upper and lower arms, the dorsal and palmar aspects of the hands, the anterior chest

Three-dimensional analysis of ankle instability after tibiofibular syndesmosis injuries: a biomechanical experimental study.

BACKGROUND: Rupture of the distal tibiofibular syndesmosis commonly occurs with extreme external rotation. Most studies of syndesmosis injuries have concentrated only on external rotation instability of the ankle joint and have not examined other defects. HYPOTHESIS: Syndesmosis injuries cause multidirectional ankle instability. STUDY DESIGN: Controlled laboratory study. METHODS: Ankle instability caused by distal tibiofibular syndesmosis injuries was examined using 7 normal fresh-frozen cadaveric legs. The anterior tibiofibular ligament, interosseous membrane, and posterior tibiofibular li

Neurocognitive Challenges During Drop Vertical Jumps Increase Sensitivity to Differentiate Atypical Landing Mechanics and Jump Height in Individuals With Anterior Cruciate Ligament Reconstruction.

BACKGROUND: Athletes with anterior cruciate ligament reconstruction (ACLR) have high rates of secondary injury. Insufficient return-to-sport screening may be due to standard functional tests not resembling chaotic sporting environments where injuries occur. Neurocognitive deficits among individuals with ACLR indicate that cognitive challenges during screening tests may better reveal atypical movement mechanics. HYPOTHESIS: Adding secondary cognitive tasks to drop vertical jumps (DVJs) would increase between-group differences in landing mechanics and jump height compared with the standard DVJ.

Management of Femoral Shaft Fractures: The Significance of Traction or Operative Position.

Background and objective Intramedullary femoral nailing (IMN) is the gold standard for managing femoral shaft fractures (FSFs). Though good clinical outcomes and union rates have been reported following this procedure, it has also been commonly associated with perioperative complications. Positioning the patient in lateral decubitus, avoiding a fracture table, or using manual traction have been touted as possible techniques to reduce perioperative complications in IMN. However, given the scarce availability of comparative research, the decision to employ any of the techniques mentioned above i
