Applying State-of-the-Art Survival Extrapolation Techniques to the Evaluation of CAR-T Therapies: Evidence from a Systematic Literature Review.

IntroductionTraditional statistical techniques for extrapolating short-term survival data for anticancer therapies assume the same mortality rate for noncured and "cured" patients, which is appropriate for projecting survival of non-curative therapies but may lead to an underestimation of the treatment effectiveness for potentially curative therapies. Our objective was to ascertain research trends in survival extrapolation techniques used to project the survival benefits of chimeric antigen receptor T cell (CAR-T) therapies.MethodsA global systematic literature search produced a review of surv

Social Determinants and Military Veterans' Suicide Ideation and Attempt: a Cross-sectional Analysis of Electronic Health Record Data.

BACKGROUND: Health care systems struggle to identify risk factors for suicide. Adverse social determinants of health (SDH) are strong predictors of suicide risk, but most electronic health records (EHR) do not include SDH data. OBJECTIVE: To determine the prevalence of SDH documentation in the EHR and how SDH are associated with suicide ideation and attempt. DESIGN: This cross-sectional analysis included EHR data spanning October 1, 2015-September 30, 2016, from the Veterans Integrated Service Network Region 4. PARTICIPANTS: The study included all patients with at least one inpatient or out

Considerations of target surface area and the risk of radiosurgical toxicity.

OBJECTIVE: The goal of this study was to explore conceptual benefits of characterizing delineated target volumes based on surface area and to utilize the concept for assessing risk of therapeutic toxicity in radiosurgery. METHODS AND MATERIALS: Four computer-generated targets, a sphere, a cylinder, an ellipsoid and a box, were designed for two distinct scenarios. In the first scenario, all targets had identical volumes, and in the second one, all targets had identical surface areas. High quality stereotactic radiosurgery plans with at least 95% target coverage and selectivity were created for

Modeling a methylmalonic acid-derived change point for serum vitamin B-12 for adults in NHANES.

BACKGROUND: No consensus exists about which cutoff point should be applied for serum vitamin B-12 (SB-12) concentrations to define vitamin B-12 status in population-based research. OBJECTIVE: The study's aim was to identify whether a change point exists at which the relation between plasma methylmalonic acid (MMA) and SB-12 changes slope to differentiate between inadequate and adequate vitamin B-12 status by using various statistical models. DESIGN: We used data on adults (≥19 y; n = 12,683) from NHANES 1999-2004-a nationally representative, cross-sectional survey. We evaluated 6 piece-wise

Confirmatory factory analysis of the Neck Disability Index in a general problematic neck population indicates a one-factor model.

BACKGROUND CONTEXT: The Neck Disability Index frequently is used to measure outcomes of the neck. The statistical rigor of the Neck Disability Index has been assessed with conflicting outcomes. To date, Confirmatory Factor Analysis of the Neck Disability Index has not been reported for a suitably large population study. Because the Neck Disability Index is not a condition-specific measure of neck function, initial Confirmatory Factor Analysis should consider problematic neck patients as a homogenous group. PURPOSE: We sought to analyze the factor structure of the Neck Disability Index through

Fool's gold: Why imperfect reference tests are undermining the evaluation of novel diagnostics: a reevaluation of 5 diagnostic tests for leptospirosis.

BACKGROUND: We observed that some patients with clinical leptospirosis supported by positive results of rapid tests were negative for leptospirosis on the basis of our diagnostic gold standard, which involves isolation of Leptospira species from blood culture and/or a positive result of a microscopic agglutination test (MAT). We hypothesized that our reference standard was imperfect and used statistical modeling to investigate this hypothesis. METHODS: Data for 1652 patients with suspected leptospirosis recruited during three observational studies and one randomized control trial that describ

Near-Infrared Hyperspectral Classification of Waste Textiles Using Complementary Spectral Representations and a Convolution-Enhanced Patch Transformer.

High-value recycling of waste textiles requires accurate fiber-composition identification, yet similar blends often exhibit overlapping near-infrared absorption bands and substantial within-class variation. Using 291 waste-textile swatches, this study develops ConvPatchTST, a convolution-enhanced Patch Transformer that integrates Raw and SNV (SG) spectra as complementary input channels. Overlapping convolutional patch embedding captures local continuous absorption features, while a Transformer encoder models cross-band dependencies. Spectral bands spanning 958.5-1646.6 nm were selected for mod

A conceptual framework for core stabilization training based on integrated ultrasonographic and functional evidence.

This manuscript presents a conceptual and integrative framework for understanding method-specific structural and functional adaptations resulting from different core stabilization training approaches in healthy adults. The synthesis is based on harmonized outcome measures derived from three prospective 4-week intervention studies conducted within a single research program. Identical ultrasonographic measurement protocols and standardized trunk mobility assessments were applied across all interventions, enabling a structured examination of adaptation patterns across training models. No pooled r

Real-world clinical validation of brainstem-based ocular biomarkers for ADHD classification in children and adults.

Current ADHD diagnostic practices rely on subjective rating scales and continuous performance tests with limited specificity. We propose an objective deep-learning approach classifying ADHD via task-evoked pupil diameter and binocular eye-movement synchrony during a visual cueing task in 439 participants across 14 clinical centers. We implemented two independent models: a multiple instance learning (MIL) framework for pupil dynamics and conventional classifiers for eye-movement synchrony. The outputs of these models were fused to derive two novel indices, a diagnostic score and an impulsivity

aiDIVA - hybrid AI for rare disease diagnostics using evidence-based, machine learning and language models.

Genome sequencing enables accurate detection of genetic variants and is transforming rare disease diagnostics. While data generation is scalable, prioritization and clinical interpretation remain challenging, often requiring expert manual classification. AI-driven decision support systems are therefore needed to assist in causal variant identification or to fully automate large-scale re-analysis of unsolved cases. Existing tools often estimate variant impact on protein function, but few integrate genomic, phenotypic, and clinical annotation data for diagnosis. We present aiDIVA, an ensemble-AI

Impact of Vitamin D Status on Speed, Power, and Sport-Specific Skills in Young Soccer Players Living Above 55° North Latitude.

BACKGROUND: Vitamin D is crucial for calcium metabolism, protein and hormone synthesis, and immune response. Deficiency may impair muscle function and athletic performance, but evidence on sport‑specific skills and power remains inconsistent. METHODS: We studied 170 soccer players aged 7-18 from a region above 55°N. In February 2024, serum 25‑hydroxy-vitamin D (25[OH]D) was measured by mass spectrometry, and physical performance tests (sprint, jumps, change‑of‑direction) were conducted. Players were compared by vitamin D status (normal, insufficient, deficient) and by 25th vs. 75th percentile

Ethnicity modifies the association between microRNA single nucleotide polymorphisms and pediatric acute lymphoblastic leukemia risk: a meta-analysis.

INTRODUCTION: MicroRNA (miRNA) single nucleotide polymorphisms (miRNA-SNPs) have been associated with pediatric acute lymphoblastic leukemia (ALL). However, since the results of these individual studies have been inconsistent, we performed a meta-analysis to help establish a statistical significance for the association between miRNA-SNPs and pediatric ALL risk. We also analyzed whether they confer susceptibility across country-specific studies by using different genetic models. METHODS: Articles published from 2001 to 2023 were collected from PubMed and Google Scholar databases. Through MetaG

Tumoral Skin Invasion Is an Independent Predictor of Rapid Recurrence in Head and Neck Cancer.

BACKGROUND: Head and neck squamous cell carcinoma (HNSCC) is an aggressive malignancy, with 50% of patients recurring. A subset of patients experience rapid recurrence (RR) postoperatively but prior to adjuvant therapy. This study identifies factors associated with RR and additional recurrence intervals: short-interval recurrence (SIR) and standard recurrence (SR). METHODS: Retrospective 10-year review of 246 HNSCC patients undergoing surgery with adjuvant therapy. Recurrence was categorized as RR (prior to initiation of adjuvant therapy), SIR (≤ 6 months post-adjuvant therapy), and SR (> 6 m

Machine learning in Alzheimer's disease genetics.

Traditional statistical approaches have advanced our understanding of the genetics of complex diseases, yet are limited to linear additive models. Here we applied machine learning (ML) to genome-wide data from 41,686 individuals in the largest European consortium on Alzheimer's disease (AD) to investigate the effectiveness of various ML algorithms in replicating known findings, discovering novel loci, and predicting individuals at risk. We utilised Gradient Boosting Machines (GBMs), biological pathway-informed Neural Networks (NNs), and Model-based Multifactor Dimensionality Reduction (MB-MDR)

Prognostic differences between pre-existing atrial fibrillation in chronic kidney disease and new-onset atrial fibrillation at hemodialysis initiation: a retrospective single-center cohort study.

Atrial fibrillation (AF) can develop in patients with chronic kidney disease. However, the impact of new-onset AF in patients who are initiated on hemodialysis remains unclear. We categorized 254 patients who were started on hemodialysis into three groups: those with pre-existing AF, those with new-onset AF, and those without AF. Statistical analyses were performed to evaluate the associations between patient characteristics and survival outcomes. AF was observed in 42 patients (16.5%), of whom 19 (7.5%) had pre-existing AF and 23 (9.1%) developed new-onset AF at the initiation of hemodialysis
