Psychogenic gait disorders.

Psychogenic disorders of posture and gait are common and are the major manifestation in 8 to 10% of patients with psychogenic movement disorders. The colorful history of these disorders is reviewed. Anxiety and depression are the commonest psychological accompaniments of functional gait disorder in contemporary practice. The particular case of the cautious gait and its flip side, "fear of falling," are considered in more detail. Common presentations for somatoform disorders and malingering are also described. It is often possible to make this diagnosis based on recognition features and gait ob…

Artificial intelligence in sleep medicine: Present and future.

Artificial intelligence (AI) has impacted many areas of healthcare. AI in healthcare uses machine learning, deep learning, and natural langu … ers such as insomnia, hypersomnia, parasomnia, narcolepsy, shift work sleep disorders, periodic leg movement disorders, etc. In this manuscript, we aim to address three critical issues about the use of AI in sleep medicine: (1) How can AI help in diagnosing and treating sleep disorders? (2) How can AI fill the gap in the care of sleep disorders? and (3) What are the …

Sleep in post-traumatic stress disorder and panic: convergence and divergence.

Disturbed sleep is a common clinical problem in anxiety disorders, particularly in patients with post-traumatic stress disorder (PTSD) and panic disorder (PD). Several studies have attempted to validate the subjective sleep complaints of these disorders using laboratory polysomnography. These attempts, typically focusing on PTSD or PD independently, have demonstrated inconsistent results. To our knowledge, no such studies have attempted to directly compare and contrast sleep disturbances in PTSD and PD together. Our review of the studies of subjective sleep disturbances, sleep architecture, an

[Prevalence of behavioral disorders in the Swiss horse population].

In the Swiss horse population, the prevalence of conspicuous behaviours (behavioural disorders and stable vices) was recorded with a written questionnaire in a representative survey among 1861 horse yards, and the occurrence of the stereotypic behavioural disorders crib-biting, weaving and boxwalking was analysed with emphasis on their association with horse specific (breed, age, sex) and environmental factors (e.g. housing system, nutrition, management and utilisation). 622 horse yards with a total number of 2536 horses answered to the questionnaire, and conspicuous behaviours were described

Posttraumatic stress disorder: neurocircuitry and implications for potential deep brain stimulation.

Posttraumatic stress disorder (PTSD) is a prevalent and highly disabling psychiatric disorder that is notoriously difficult to treat. At some point in their lifetimes, 5-8% of men, 10-14% of women, and up to a quarter of combat veterans carry this diagnosis. Despite pharmacological and behavioral therapies, up to 30% of patients are still symptomatic 10 years after initial diagnosis. Recent advances in imaging have implicated changes in the limbic and autonomic corticostriatopallidothalamocortical (CSPTC) circuitry in the pathogenesis of this disease. Deep brain stimulation modulates CSPTC cir

Diagnostic Decision Support and Automation in Vestibular Diagnosis.

This article examines the current and future role of decision-support systems and automation in vestibular diagnosis across emergency, primary care, and specialty settings. Vestibular disorders are common but often misdiagnosed because of overlapping symptoms, challenging bedside examinations, and limitations of neuroimaging. Structured diagnostic approaches improve accuracy when correctly applied. Emerging technologies offer objective assessment of eye movements and may enhance diagnostic reliability. Large language models provide valuable support for clinical reasoning, education, and inform

Seizures and Shaking in Newborns and Babies.

Babies and small children can make movements that look like seizures but are nonpileptic and call only for reassurance. Features more suggestive of seizure are vital sign changes, stereotyped, occur across different setting/situations, involve head/eye movement, cannot be stopped with repositioning, and are not stimulus induced/trigger. If a child's medical history is positive for risk factors for seizures, then further work up may be necessary. Risk factors include birth history concerning perinatal hypoxic-ischemic encephalopathy/prematurity/neonatal intensive care unit stay, genetic conditi

Paroxysmal Kinesigenic Dyskinesia: Genetics and Pathophysiological Mechanisms.

Paroxysmal kinesigenic dyskinesia (PKD), the most common type of paroxysmal movement disorder, is characterized by sudden and brief attacks of choreoathetosis or dystonia triggered by sudden voluntary movements. PKD is mainly caused by mutations in the PRRT2 or TMEM151A gene. The exact pathophysiological mechanisms of PKD remain unclear, although the function of PRRT2 protein has been well characterized in the last decade. Based on abnormal ion channels and disturbed synaptic transmission in the absence of PRRT2, PKD may be channelopathy or synaptopathy, or both. In addition, the cerebellum is

Tardive Dyskinesia: Spotlight on Current Approaches to Treatment.

Tardive dyskinesia (TD) is a debilitating, iatrogenic, and potentially severe movement disorder characterized by involuntary, repetitive, purposeless movements that are present throughout the body. The authors present a review of studies of past, current, and possible future treatment approaches to the management of TD; consider the phenomenology, assessment, and putative pathophysiological mechanisms of TD, early pharmacological trials, a focus on the newer vesicular monoamine transporter 2 inhibitors, and other evidence-based approaches, such as clozapine; and present preliminary evidence fo

Not the Stereotypical Wilson Disease: A Case Report.

BACKGROUND: Wilson disease (WD), a potentially treatable genetic disorder with perturbations in copper metabolism, presents with hepatic and neuropsychiatric manifestations. Both hyper and hypokinetic movements predominate the latter spectrum. Motor stereotypies, however, are exceedingly rare. CASE REPORT: We present a case of a 12-year-old girl, with progressive behavioural alterations and cognitive impairment, with motor stereotypies involving the upper limbs, as the dominant movement semiology. She was diagnosed as WD with evidence of striatal involvement on brain imaging. Her motor sympto

Late adverse effects of the coadministration of valproate and lamotrigine.

Individuals treated with combined valproate-lamotrigine rarely present late adverse effects (unrelated to introduction and titration). We describe four patients in whom such effects occurred after continuous, long-term use of valproate-lamotrigine (at 9 months to 2 years after final antiepileptic drug adjustment). The patients presented heterogeneous disturbances, including ataxia, vertigo, and headache, and rare movement disorders, such as tics and abnormal eye movements. Although these effects are heterogeneous in their occurrence and timing, they can alert physicians to the possibility of l…

Natural history of Rett syndrome.

Rett syndrome is a unique neurodevelopmental disorder, with onset of hypotonia, autistic tendency, and abnormalities of fine finger movements and gross movements of the arms in early infancy. Clinical features include specific age-dependent symptoms. Studies of early and late signs correlated locomotive dysfunction to language disability and stereotypy to regression of higher cortical functions. Studies of sleep parameters revealed early hypofunction of brainstem aminergic neurons and late occurrence of hypofunction of dopaminergic neurons, followed by receptor supersensitivity. The syndrome's

Using ultrasound to investigate intrapersonal variability in durational aspects of tongue movement during swallowing.

The purpose of this study was to determine the extent of intrapersonal variability in durational aspects of tongue movement during swallowing. Using ultrasound, the entire duration of tongue movement and the duration of the oral transport stage during swallowing was measured in a group of patients with neurogenic swallowing disorder and a group of matched healthy control subjects. The results indicate considerable intrapersonal variability in both groups. The patient group performed significantly more variably in terms of the duration of the transport stage. However, when looking at individual

Periodic leg movements in sleep in essential hypertension.

The presence of periodic leg movements in sleep (PLMS) was assessed in 91 subjects diagnosed with essential hypertension. More than 18 per cent of the sample had PLMS, which is considerably higher than in normal controls. Also, the prevalence was significantly correlated with the severity of hypertension, as well as with age. Periodic leg movements in sleep were more frequent in the first few hours of the sleep period and during sleep stages 1 and 2. The arousing effect of PLMS was minimal, with only 17 per cent of all events related to an EEG arousal. Our results suggest that PLMS are common

Posthypoxic myoclonus (the Lance-Adams syndrome) in the intensive care unit.

The neurological assessment of patients admitted to the intensive care unit after successful resuscitation from cardiopulmonary arrest may be difficult. We describe the cases of two patients who developed myoclonus within 24 hours of hypoxic respiratory and cardiac arrest. Initially, the clonic movements were thought to be generalised convulsions and were treated as such, until it became evident that the patients were aware and distressed. Posthypoxic myoclonus is a rare complication of successful cardiopulmonary resusitation. Recognition depends on the awareness that the syndrome exists, and

Automated eye-tracking for Parkinson's disease diagnosis: a proof-of-concept cascade classifier study.

BACKGROUND: Parkinson's disease (PD) is a progressive neurodegenerative disorder of increasing prevalence, with diagnostic accuracy of approximately 26% in early symptomatic patients. There is a need for accurate, non-invasive biomarkers to aid in disease diagnosis. METHODS: This proof-of-concept study enrolled 90 participants (PD n = 30, other movement disorders [OM] n = 30, healthy controls [HC] n = 30) at a single institution. Participants completed two 10-min eye- tracking sessions using the SaccadeDX™ 250-Hz binocular system. A two-level cascade classifier was fitted using elastic-net fe…
