Movement of solutes across the peritoneum allows for the use of peritoneal dialysis to treat kidney failure. However, there is a large inter-individual variability in the peritoneal solute transfer rate (PSTR). Here, we tested the hypothesis that common genetic variants are associated with variability in PSTR. Of the 3561 participants from 69 centers in six countries, 2850 with complete data were included in a genome-wide association study. PSTR was defined as the four-hour dialysate/plasma creatinine ratio from the first peritoneal equilibration test after starting PD. Heritability of PSTR wa

A 69-year-old woman with 26-year history of systemic lupus erythematosus and 4-year history of peritoneal dialysis was hospitalized for treatment of bacterial peritonitis. On admission, peritoneal dialysate was collected and subjected to bacterial culture. Cell count in the cloudy peritoneal dialysate was 4194/μL, and Gram-negative bacilli were detected. Vancomycin (1 g/day) and ceftazidime (1 g/day) were administered intraperitoneally, which resulted in rapid decrease in cell count in the peritoneal dialysate. However, on the 7th hospital day, peritonitis relapsed with abdominal pain and clou…

BACKGROUND: Heat sterilization of peritoneal dialysis (PD) dialysates leads to the generation of advanced glycation products (AGE), which can then deposit in the skin and be measured by skin autofluorescence (SAF). Newer biocompatible dual chamber dialysates contain less AGE. We wished to determine whether the use of these newer dialysates resulted in lower SAF.
METHODS: Skin autofluorescence was measured using the AGE reader, which directs ultraviolet light, intensity range 300-420 nm (peak 370 nm) in patients established on PD for >3 months using glucose containing dialysates.
RESULTS: We

Peritoneal carcinomatosis (PC) is a fatal condition arising in the gastrointestinal tract. PC patients administered drugs locally in the tumor region, such as in intraperitoneal chemotherapy (IPCh), suffer from low drug retention time and tumor penetration. Herein, we synthesized a lithocholic acid (LCA)-conjugated disulfide-linked polyethyleneimine (ssPEI) micelle (LAPMi) nanoconstruct by covalently conjugating ssPEI and LCA, thereby forming positive charged nanomicellar structures loaded with paclitaxel (PTX) (LAPMi-PTX) for IPCh. The incorporation of a positive surface charge aided in prolo

BACKGROUND: Complete cytoreductive surgery (CCRS) plus hyperthermic intraperitoneal chemotherapy (HIPEC) is on the verge of becoming the gold standard treatment for selected patients presenting peritoneal metastases (PM) of colorectal origin. PM is scored with the peritoneal cancer index (PCI), which is the main prognostic factor. However, small bowel (SB) involvement could exert an independent prognostic impact.
AIM: To define an adequate cut-off for the PCI and to appraise whether SB involvement exerts an impact on this cut-off.
PATIENTS AND METHODS: Patients (n = 139) treated with CCRS pl

One of the most important immunopathological consequence of intraperitoneal alveolar echinococcosis (AE) in the mouse is suppression of T cell-mediated immune responses. We investigated whether and how intraperitoneal macrophages (MØs) are, respectively, implicated as antigen-presenting cells (APCs). In a first step we showed that peritoneal MØs from infected mice (AE-MØs) exhibited a reduced ability to present a conventional antigen (chicken ovalbumin, C-Ova) to specific responder lymph node T cells. In a subsequent step, AE-MØs as well as naïve MØs (positive control) proved their ability to

Macrophages, isolated from dialysis fluid of three patients with continuous ambulatory peritoneal dialysis (CAPD) at different times during peritonitis were labelled with 14C-arachidonic acid and stimulated with the calcium ionophore A23187. The main metabolites formed by 5-lipoxygenase activity were leukotriene B4 (LTB4) and 5-hydroxy-6, 9, 11, 14-eicosatetraenoic acid (5-HETE). Smaller amounts of cyclooxygenase metabolites were present and also a major compound with an elution time between 6-keto-prostaglandin F1 alpha (6-keto-PGF1 alpha) and thromboxane B2 (TxB2). This substance was isolate…

Pancreatic ascites is an uncommon complication of pancreatic disorders resulting from persistent leakage of pancreatic juice into the peritoneal cavity, most commonly due to disruption of the main pancreatic duct or rupture of a pancreatic fluid collection. Bacterial peritonitis complicating pancreatic ascites is rarely reported. We report the case of a 40-year-old man who presented with massive pancreatic ascites associated with a marked elevation of serum lipase. Analysis of the ascitic fluid strongly supported its pancreatic origin, and conservative management with enteral nutrition and a s…

AIM OF THE STUDY: Primary anastomosis (PA) is now recommended for most cases of diverticular peritonitis. However, recent studies have shown that Hartmann's Procedure (HP) is still included in the majority of practice patterns. The aim of this study was to analyze situations wherein HP was carried out and to define its remaining place among current treatment options for generalized peritonitis.
METHODS: We performed a retrospective cohort study of patients who underwent sigmoidectomy for diverticular peritonitis at Rouen University Hospital between 2004 and 2020. Logistic regression was compu…

The global availability of GS-441524 for treating feline infectious peritonitis (FIP) has increased in recent years, yet little data is available about how primary care veterinarians diagnose the disease. This study aimed to evaluate the diagnostic approach to FIP in primary care practices and compare findings between effusive and non-effusive forms. Retrospective analysis was conducted using a dataset of 243 cats treated with unlicensed GS-441524 in Western Switzerland, obtained via a social media platform, all of which completed a minimum of 12-weeks treatment. Demographics were consistent w…

ObjectivesFeline infectious peritonitis (FIP) can be treated with antiviral agents such as remdesivir and GS-441524, with a commonly used treatment duration of 12 weeks. Although serum amyloid A (SAA) and alpha-1 acid glycoprotein (α1AG) have been proposed as markers of disease improvement, the criteria for treatment cessation before this duration remain unclear. This study evaluated whether SAA and α1AG could guide treatment cessation in cats with FIP and whether a high-dose induction protocol could allow treatment duration to be shortened.MethodsA total of 30 cats with FIP, excluding those p…

Background: Extended-spectrum β-lactamase (ESBL)-producing Escherichia coli represent an increasing therapeutic challenge. While ertapenem (ERP) is commonly used as first-line therapy, amoxicillin-clavulanic acid (AMC) achieves therapeutic concentrations in serum and ascitic fluid and may offer a narrower-spectrum alternative. This exploratory preclinical study evaluated whether AMC produces effects comparable to ERP in a rat model of ESBL E. coli peritonitis. Methods: Thirty-three male rats were allocated to four groups: untreated E. coli, AMC, ERP, and sham controls. Peritonitis was induced …

BACKGROUND: Generalized peritonitis is a severe intra-abdominal infection associated with high morbidity and mortality in patients undergoing major emergency abdominal surgery. While short-term outcomes are well documented, long-term survival and prognostic factors remain less explored. This study investigated four-year survival and conditional survival to assess the impact of generalized peritonitis on long-term outcomes.
METHODS: This was a retrospective study based on prospectively collected data from 2017 to 2020, including patients aged ≥ 18 years who underwent major emergency abdominal …

We report a case of generalized acute peritonitis following an unsafe abortion complicated by uterine perforation. The patient was a 32-year-old woman admitted for abdominopelvic pain evolving in a febrile context, with an 8-week history of amenorrhea. On admission, clinical examination revealed a picture of generalized acute peritonitis. Abdominopelvic ultrasound and plain abdominal X-ray revealed an intra-abdominal foreign body (a full pencil). Surgical exploration confirmed generalized acute peritonitis secondary to uterine perforation, with approximately 200 ml of purulent fluid and a comp…

BACKGROUND: COVID-19 infection has caused a global health crisis with over 6,956,900 deaths. Extra-pulmonary forms of the infection are common, including digestive symptoms. Pregnant women are at greater risk of developing severe forms of the infection. They are also affected by extra-pulmonary symptoms. CASE PRESENTATION: We report the case of a 28week + 4 pregnant woman with paralytic ileus caused by inflammatory peritonitis related to COVID-19 leading to premature birth. Pulmonary deterioration was secondary to abdominal symptoms. CONCLUSIONS: Therefore, the presence of gastrointestinal (GI…

BACKGROUND: Albumin is recommended in the management of patients with spontaneous bacterial peritonitis (SBP) to reduce rates of renal injury and mortality. Current guideline recommendations suggest an intravenous albumin dosing regimen of 1.5 g/kg on day 1 and 1 g/kg on day 3, although a maximum dosing strategy is not well-defined.
OBJECTIVE: The purpose of this study was to evaluate differences in renal injury for patients with SBP treated with a capped dose less than or equal to 100 g vs a weight-based dose greater than 100 g.
METHODS: A retrospective analysis was conducted at a single ac…

BACKGROUND: Perforation peritonitis remains a significant cause of morbidity and mortality in emergency surgical patients. Conventionally, open laparotomy has been the preferred approach. However, with advancements in minimally invasive techniques, laparoscopic management has emerged as a potential alternative. The study aims to assess the feasibility and effectiveness of laparoscopic surgery compared to open laparotomy in managing perforation peritonitis.
PATIENTS AND METHODS: A prospective observational study was conducted from January 2022 to May 2023 for patients diagnosed with perforatio…
