Implementing Istanbul Protocol standards for forensic evidence of torture in Kyrgyzstan.

The Kyrgyz government declared a policy of "zero tolerance" for torture and began reforms to stop such practice, a regular occurrence in the country's daily life. This study presents the results of 10 forensic evaluations of individuals alleging torture; they represent 35% of all criminal investigations into torture for the January 2011-July 2012 period. All individuals evaluated were male with an average age of 34 years. Police officers were implicated as perpetrators in all cases. All individuals reported being subjected to threats and blunt force trauma from punches, kicks, and blows with o

Trauma history and psychopathology in war-affected refugee children referred for trauma-related mental health services in the United States.

There is an increasing need to deliver effective mental health services to refugee children and adolescents across the United States; however, the evidence base needed to guide the design and delivery of services is nascent. We investigated the trauma history profiles, psychopathology, and associated behavioral and functional indicators among war-affected refugee children presenting for psychological treatment. From the National Child Traumatic Stress Network's Core Data Set, 60 war-affected refugee children were identified (51.7% males, mean age = 13.1 years, SD = 4.13). Clinical assessments

Long-term cognitive sequelae: abused children without PTSD.

Many lines of research suggest that childhood abuse and neglect are associated with later developing psychiatric diagnoses, academic problems, cognitive difficulty, and possible brain changes as measured through brain imaging. Data were collected on children (N = 41) who completed a neuropsychological evaluation. Of those evaluated, 18 had a documented history of physical and/or emotional abuse or significant neglect and 23 had no history of abuse/neglect. When controlling for Full-Scale IQ (FSIQ), the abused children had significantly lower scores on measures of executive functioning (Wiscons

Observations and insights about strengthening our soldiers SOS.

The Special Issue (June 2011) of the Journal of Clinical Psychology in Medical Settings titled Strengthening Our Soldiers (SOS) and Their Families: Contemporary Psychological Advances Applied to Wartime Problems revealed the following important concerns: 1) Who is at risk for psychological sequelae during and following service in the U.S. military? 2) How to deliver the best treatment for our soldiers and veterans with PTSD, Traumatic Brain Injury and Pain? 3) How to train the trainers? and 4) What are the current priorities for service delivery, research and funding? Assessment strategies and

An evolving integrative treatment program for military sexual trauma (MST) and one veteran's experience.

Military sexual trauma (MST) increases the risk for Posttraumatic Stress Disorder (PTSD) and multiple other comorbidities, presenting substantial challenges for nurses and psychiatric and medical clinicians. A specialized VA Medical Center outpatient program is patterned after Herman's three-phased, empirically-supported, recovery treatments. We use a case example of a female veteran MST survivor to illustrate our treatment model. She presented to our program meeting diagnostic criteria for PTSD, Major Depressive Disorder, and a history of substance abuse. Post-treatment she demonstrated impro

[The etiological models of posttraumatic stress disorder].

The author summarizes the social-cognitive, behavioral, information processing and psychobiological models of PTSD. All the theories assume that PTSD symptoms are mainly caused by the unique characteristics of trauma memories and the difficulty of integrating them into the other autobiographic memories. Thus integrating the trauma into the other autobiographic memories plays a crucial role in the therapy of trauma.

[Dissociative disorder and traumatic events. A study of Spanish population].

Dissociative Disorder is relatively uncommon. It has been associated to the presence of traumatic events and especially to sexual abuse in childhood. Our study presents a clinical sample of 36 patients with Dissociative Disorder, prospectively evaluated with standardized scales in an outpatient department of general psychiatry. The sample is mainly made up of married (86.1%) women (34/36), from medium-low class with important comorbidity (38.9% affective disorders, 52.8% conversive disorders, 41.7% anxiety disorders and 38.9%, personality disorders). Our results show a high rate of childhood t

Does overgeneral autobiographical memory result from poor memory for task instructions?

Considerable previous research has shown that retrieval of overgeneral autobiographical memories (OGM) is elevated among individuals suffering from various emotional disorders and those with a history of trauma. Although previous theories suggest that OGM serves the function of regulating acute negative affect, it is also possible that OGM results from difficulties in keeping the instruction set for the Autobiographical Memory Test (AMT) in working memory, or what has been coined "secondary goal neglect" (Dalgleish, 2004). The present study tested whether OGM is associated with poor memory for

Women and addiction: a trauma-informed approach.

Historically, substance abuse treatment has developed as a single-focused intervention based on the needs of addicted men. Counselors focused only on the addiction and assumed that other issues would either resolve themselves through recovery or would be dealt with by another helping professional at a later time. However, treatment for women's addictions is apt to be ineffective unless it acknowledges the realities of women's lives, which include the high prevalence of violence and other types of abuse. A history of being abused increases the likelihood that a woman will abuse alcohol and othe

A cluster analysis of symptom patterns and adjustment in Vietnam combat veterans with chronic posttraumatic stress disorder.

This study investigated whether a subgroup of veterans with malignant posttraumatic stress syndrome, as described by Rosenheck (1985) and Lambert et al. (1996), could be identified via cluster analysis within two samples of Vietnam veterans with combat-related posttraumatic stress disorder (PTSD). In the initial subsample (n = 157), four clusters were identified, including a subgroup that scored consistently higher on measures of interpersonal violence and current physical problems. Similar results were found in the cross-validation subsample (n = 156). These results provide support for the th

Anger management and temper control: critical components of posttraumatic stress disorder and substance abuse treatment.

Recent studies have shown associations among combat experience, PTSD, anger and hostility, and involvement in violence. Clinical observations of veterans enrolled in the Substance Use/Posttraumatic Stress Disorder Team (SUPT) program at the San Francisco Veterans Affairs Medical Center revealed relatively high levels of anger and aggressive behavior, including physical assaults and property damage. In response to this anger and aggressive behavior, an anger management treatment was added to the SUPT program's treatment of substance abuse and PTSD. Anger management consisted of a 12-week cognit

The transformation of affect in posttraumatic nightmares.

The author questions the classical psychoanalytic assumptions that anxiety in posttraumatic nightmares arises from the same conflict represented in the manifest content of the nightmare, that such nightmares have no latent content, and that the day residue is insignificant. Such assumptions obscure the function of these nightmares, in which the dreamwork transforms shame into fear. Case material from two patients highlights the instigating role of posttraumatic dissociative states and shame in the day residue of posttraumatic nightmare sufferers. The author also emphasizes that conflict in the

Psychiatric outcome of burned children and adolescents.

Recent medical and surgical advances allow many severely burned patients to survive who formally would have died. Assessment of psychiatric outcomes with these patients may provide ways of measuring effects of acute burn care methods on later quality of life, specify more accurately their emotional needs during rehabilitation, and stimulate further research. Thirty children, aged 7 to 19, with severe burns are compared with 30 nonburned subjects matched for age, sex, SES, and parents' marital status according to DSM-III criteria. The burned children had significantly higher levels of overanxio

Implosive therapy in the treatment of combat related fears in a world war II veteran.

A 55-year old veteran with combat related fears of 36 years' duration was successfully treated in three sessions of implosive therapy, each lasting approximately 1 hr. This improvement followed 15 hospitalizations over the preceding 30 month period, and previous unsuccessful treatment efforts including prescription of antabuse, an inpatient alcohol treatment program, and an agoraphobia treatment program conducted on an outpatient basis. Treatment gains were maintained over a 2 year follow-up period.
