CSF alpha-Synuclein Seed Amplification Assay results in routine clinically collected samples.

BackgroundLewy Body pathology can be detected by alpha-synuclein-Seed Amplification Assay (αSyn-SAA) in cerebrospinal fluid (CSF) with high sensitivity and specificity in cohort studies. Yet, little is known about the results that can be expected from αSyn-SAA in CSF in samples outside cohort studies as obtained in clinical routine.ObjectiveThis study analyzed the concordance of αSyn-SAA findings in CSF with clinical diagnosis in patients from clinical routine with diverse neurologic and psychiatric conditions.MethodsIn this cross-sectional study, CSF from patients who underwent lumbar punctur

Noninvasive Electrophysiological Biomarkers of Olfactory Responses Across Cognitive States in Alzheimer Dementia: Cross-Sectional Study.

BACKGROUND: Alzheimer disease (AD) is characterized by progressive cognitive decline, with olfactory dysfunction emerging among its earliest symptoms. Conventional olfactory tests rely on subjective self-reports and patient cooperation, limiting their clinical applicability. Noninvasive olfactory bulb (OB) recordings may provide objective physiological measures for early detection of cognitive decline. OBJECTIVE: This study investigated the relationship between olfactory dysfunction and cognitive decline across the AD spectrum and evaluated whether noninvasive OB recordings obtained via a wea

Kinesiophobia phenotypes and fall history in older adults with osteoporosis: a cross-sectional study using latent profile analysis.

OBJECTIVE: Falls are a leading cause of morbidity among older adults with osteoporosis. While traditional risk assessment focuses on biological factors such as bone mineral density (BMD), psychological factors-particularly kinesiophobia-have received increasing attention. However, most studies conceptualize kinesiophobia as a continuous construct, potentially overlooking clinically meaningful heterogeneity. This study aimed to identify kinesiophobia phenotypes and examine their association with fall history in older adults with osteoporosis. METHODS: This cross-sectional study included 306 ol

Cognitive and fine motor performance in people above 65 years of age with and without HIV.

Cognitive and motor performances decline with ageing, and this may be exacerbated in people with HIV (PWH) due to several factors. The study aimed to compare cognitive and fine motor performance between older adults with and without HIV. We conducted a cross-sectional study of participants ≥ 65 years in the GEPPO cohort using Mini-Addenbrooke's Cognitive Examination (MACE) and Grooved Pegboard Test (GPT). Quality of life, depression, anxiety, and sleep quality were also measured. PWH (n = 239) were younger (73.7 vs. 80.6 years) and more commonly males at birth (85 vs. 25%) than PWoH (n = 52).

Development and Crossover Evaluation of an Artificial Intelligence-Assisted System for Solid Pancreatic Lesion Detection and Pancreatic Parenchyma Recognition in Endoscopic Ultrasonography (With Video).

BACKGROUND AND STUDY AIMS: Pancreatobiliary endoscopic ultrasonography (EUS) is technically demanding, and supervised training opportunities are limited. We developed an artificial intelligence (AI) overlay system for detecting solid pancreatic lesions (SPL) and recognizing pancreatic parenchyma (PP) and evaluated its effect on reader performance. PATIENTS AND METHODS: Across six centers, two deep learning-based models were trained using expert-annotated EUS frames. We then conducted a randomized, two-sequence, two-period crossover reader study in which eight endosonographers (five novices an

The STent Or NEphrostomy Study: A Randomised Controlled Trial Evaluating the Effectiveness of Percutaneous Nephrostomy Versus Internal Stent in Patient with Urolithiasis and an Indication for Decompression.

BACKGROUND AND OBJECTIVE: Obstruction by urolithiasis may be the reason for a percutaneous nephrostomy (PCN) or the placement of an internal stent. High-quality evidence is scarce. The objective of this study is to investigate whether a PCN is noninferior to an internal stent with regard to effectiveness (time to clinical recovery). METHODS: In this nationwide randomised controlled noninferiority trial performed in 11 secondary and tertiary care hospitals in the Netherlands, we randomised 204 patients for either a PCN or an internal stent. The primary outcome was the time to recovery. KEY FI

Exploring the impact of myoelectric prosthesis controllers on visuomotor behavior.

BACKGROUND: Prosthesis users often rely on vision to monitor the activity of their prosthesis, which can be cognitively demanding. This compensatory visual behaviour may be attributed to an absence of feedback from the prosthesis or the unreliability of myoelectric control. Unreliability can arise from the unpredictable control due to variations in electromyography signals that can occur when the arm moves through different limb positions during functional use. More robust position-aware control systems have been explored using deep learning methods, specifically ones that utilize data from di
