When the Pediatric Gut Stops: The Pattern of Intestinal Obstruction in Children.

BACKGROUND: Intestinal obstruction is a common pediatric surgical emergency and an important cause of morbidity and mortality. This study wa … e most common causes of intestinal obstruction were appendicular perforation (37, 17.37%), Meckel's diverticulum (33, 15.49%), obstructed inguinal hernia (32, 15.02%), and intussusception (30, 14.08%). In infants (n = 44), obstructed inguinal hernia (9, 20.14%), intussusception (8, 18.18%), anorectal malformation (7, 15.91%), and duodenal obstruction (7, 15.91 …

Differentiating colonic diverticulitis from colon cancer: the value of computed tomography in the emergency setting.

BACKGROUND: The purpose of this study was to establish practical diagnostic criteria to differentiate colonic diverticulitis from colon canc … parameters, the most valuable imaging findings for diverticulitis were the presence of non-inflamed diverticulum, inflamed diverticulum, mild degree of enhancement, and small lymph node size. Three were found to have high specificity but low sensitivity: target enhancement pattern; abscess formation; and free air. When colonic obstruction is present, colon can …

A case report of ectopic pancreatic tissue in the gallbladder.

Heterotropic pancreatic tissue is a rare entity where aberrant pancreatic tissue is located without any anatomical connection to the pancreas. It is usually reported to be found in stomach, duodenum, jejunum and spleen. Rare locations include lung, mediastinum, liver, mesentery, ileum, meckels diverticulum and gallbladder [1, 2]. This anomaly is often an incidental finding during laparotomy or autopsy. We describe a case of an elective laparoscopic cholecystectomy with the histology of the gallbladder showing focal ectopic pancreatic acinar tissue.

Multidetector CT of left atrium.

MDCT angiography has enabled visualization of left atrium in addition to coronary arteries. CT images can be utilized to demonstrate normal anatomy and pathologies. Left atrium variations and pathologies include atrial septal pouch, patent foramen ovale, atrial septal defect, atrial band, lipomatous hypertrophy, cor triatriatum, diverticulum, accessory appendages, atrial thrombus and masses. Left atrium should be carefully evaluated in patients undergoing coronary CT angiography. MDCT is becoming an alternative modality for the evaluation of left atrium.

Microbiota of the Sigmoid Colon Mucus in Complicated Forms of Diverticular Disease.

To study the specificity of the microbiota and its role in the pathogenesis of diverticular disease of the colon, we examined the mucosal microbiota of 26 diverticula from resected sigmoid colon specimens from 13 patients undergoing surgery for various inflammatory complications of the disease. Using 16S rRNA gene sequencing on the Illumina MiSeq platform, we identified some differences indicating a specific pathological process within the diverticulum. However, it remains unclear whether the microbial community triggers inflammation and the development of complications.

Infected umbilical urachal sinus in a neonate.

The urachus is an embryonic tube that connects the upper portion of the bladder to the umbilicus and normally obliterates during embryonic development, forming the median umbilical ligament. Incomplete obliteration of the urachus can result in various anomalies such as congenital patent urachus, umbilical urachal sinus, vesico-urachal diverticulum and urachal cyst. We report a term neonate who presented with swelling and discharge from the umbilical region, which through clinical evaluation and radiological investigations, was diagnosed as an infected umbilical urachal sinus.

Spontaneous Colovesical Fistula With Benign Prostatic Hyperplasia and Prostatitis.

We present an unusual case of colovesical fistula formed by a single diverticulum manifesting as prostatitis and urinary tract infection (UTI) before causing further complications. Diverticulitis is caused by inflammation of the colonic diverticula and manifests as fever and left lower quadrant pain. The patient also developed recurrent pneumaturia; upon examination, a colovesical fistula was observed on computed tomography (CT), which was managed successfully by laparoscopic repair. This case highlights manifestations of colovesical fistula secondary to diverticular disease and the laparoscop…

End-to-Side Ileo-Sigmoid Anastomosis and Late Onset Gangrenous Diverticulitis: A Case Report.

End-to-side ileo-sigmoid anastomosis converts the proximal colon into a blind intestinal segment which is excised during the surgery. If we do not resect the proximal colon, it is expected to behave like a colonic diverticulum, but direct evidence of this assumption is lacking. During an exploratory laparotomy, we detected an end-to-side ileo-sigmoid anastomosis and found that the proximal colon was gangrenous. The patient passed away during the postoperative period, yet their remarkably long period of symptom-free survival remained intriguing.

Per-oral Endoscopic Myotomy: State of the Art.

For more than a decade, per-oral endoscopic myotomy (POEM) has been performed for the treatment of achalasia and other dysmotilities of the esophagus. POEM has become an accepted alternative to Heller myotomy, a salvation technique for patients not responding to Heller myotomy and the favorite intervention for diffuse esophageal spasm and jackhammer esophagus. POEM paved the way for endoscopic pyloromyotomy, Zenker's diverticulum myotomy, and submucosal tunneling with endoscopic resection. This review will cover the technique and the most current literature of this revolutionary third space te…

Modified use of a 4-branched frozen elephant trunk prosthesis for treatment of a right-sided aortic arch aneurysm with an aberrant left subclavian artery.

Right aortic arch aneurysm originating from a Kommerell's diverticulum associated with an aberrant left subclavian artery is a rare congenital entity. We report a case of an asymptomatic 60-year-old female with right aortic arch aneurysm with an aberrant left subclavian artery, treated with a modified frozen elephant trunk technique using a 4-branched prosthesis, with the perfusion branch as an extra-anatomical bypass to the aberrant left subclavian artery. This case demonstrates short-term safety and efficacy of this technique.

Female Urethral Reconstruction: Urethral Diverticulectomy, Urethral Strictures, Vesicovaginal Fistula.

Urethral reconstruction in women encompasses a variety of different entities including female urethral stricture, female urethral diverticulum, and vesicovaginal fistula. Although very different in their causes, they all have in common a vague and nonspecific onset of symptoms and, usually, a delayed diagnosis. Once identified and evaluated, urologists must review the management options with the patient, which range from minimally invasive procedures to complex reconstructive surgeries. In complicated cases, we recommend referral to an experienced specialized center for definitive management.

A Giant Urethral Calculus.

Urethral calculi are rare forms of urolithiasis. Majority of the calculi are migratory from urinary bladder or upper urinary tract. Primary urethral calculi usually occur in presence of urethral stricture or diverticulum. In this article we report a case of a giant posterior urethral calculus measuring 7x3x2 cm in a 47 years old male. Patient presented with acute retention of urine which was preceded by burning micturition and dribbling of urine for one week. The calculus was pushed in to the bladder through the cystoscope and was removed by suprapubic cystolithotomy.

Pancreatic heterotopia of the small intestine: two case reports.

The presence of heterotopic pancreas is unusual with an estimated incidence of 0.2% of upper abdominal operations. Heterotopic pancreas occurs predominantly in the stomach, duodenum and proximal jejunum. Isolated pancreatic heterotopia of the ileum is very rare and is usually found in a Meckel's diverticulum. In most cases, these heterotopias are asymptomatic and are only incidentally detected upon pathological examination or autopsy. In this paper, the authors report two cases of pancreatic heterotopia involving, respectively, the duodenum and ileum that were fortuitously discovered on a surg…

[Caecal diverticulitis in a young woman with suspected acute appendicitis].

Colonic diverticular disease is common amongst the western population in the left hemicolon while right-sided involvement is rare. This case concerns a 20-year-old female, who was hospitalized with suspected acute appendicitis. Perioperative findings included an intramural tumour at the base of the appendix, and an ileocaecal resection was performed. Histopathological examinations revealed a single inflamed diverticulum of the caecum. The case illustrates the pre- and perioperatively differential diagnostic problems of caecal diverticulitis, which can lead to more radical surgery than needed.

Intrapericardial bronchogenic cyst adherent to the ascending aorta.

Bronchogenic cysts arise from an abnormal budding of the ventral diverticulum of the foregut or the tracheobronchial tree during embryogenesis. Pericardial location of these cysts is very rare. We describe a case of a young asymptomatic woman with an intrapericardial cystic mass compressing the right heart. Because of severe adhesions of the mass to the ascending aorta and to the right coronary artery, these structures were injured during surgical resection requiring the replacement of the ascending aorta and a coronary artery by-pass graft. Only the histopathologic findings provided the final…

Jejunal Diverticulitis: A Rare Case of Severe Peritonitis.

A 25-year-old African American female with no prior medical/surgical history presented with abdominal pain and fever. A computed tomography scan of the abdomen and pelvis showed jejunal wall thickening with an air-fluid-filled mass in the adjacent mesentery. At laparotomy, a segmental jejunal resection with the abscess cavity followed by primary anastomosis was performed. Pathological evaluation of the specimen revealed a large mesenteric abscess contiguous with a perforated solitary jejunal diverticulum. We provide a discussion of jejunal diverticulitis as an unusual cause of peritonitis.

Transvaginal sonographic diagnosis of female urethral diverticula.

PURPOSE: We report our experience with transvaginal sonography (TVUS) in the diagnosis of female urethral diverticulum (UD). METHOD: We reviewed the TVUS examinations of 4 patients with a clinical diagnosis of UD and correlated the sonographic findings with the operative findings, especially regarding the UD's size, content, and location. RESULTS: All UDs were demonstrated on TVUS. The size, content, and location of the UD correlated well with the operative findings. CONCLUSION: TVUS is accurate in diagnosis and determination of the size, content, and location of female UD.
