Postirradiation sarcoma of the gynecologic tract. A report of 13 cases and a discussion of the risk of radiation-induced gynecologic malignancies.

With improvement in survival after cancer treatment, it is becoming increasingly important to examine treatment-related morbidity and mortality. Sarcomas can develop within the irradiated field after radiation therapy (RT) for gynecologic malignancies. We undertook a study to assess the outcome after treatment of postirradiation sarcoma (PIS) of the gynecologic tract. In reviewing our data and the literature, we compare the absolute risk of PIS and other radiation-associated second malignant neoplasms (SMNs) with the mortality risk of surgery and general anesthesia. Between 1955 and 1987, 114

Feasibility of intracranial surgery in the primate fetus. Model and surgical principles.

A research model for intracranial surgery in the primate fetus was developed and tested in 10 timed-pregnant rhesus monkeys. With general anesthesia and sterile surgical technique, a laparotomy followed by a lower uterine segment hysterotomy was performed at a site avoiding the placenta. The amnion was opened carefully by use of the operative microscope and the fetal head was exposed. A scalp incision was made over the frontal region and a small craniectomy and cortical incision were carried out. In three of the fetuses, ventricular shunts were also placed. A layered closure was performed and

Persistent and recurrent hyperparathyroidism after total parathyroidectomy with autotransplantation.

OBJECTIVE: To conduct a retrospective study of 15 patients with persistent (n = 4) and recurrent (n = 11) hyperparathyroidism. SUMMARY BACKGROUND DATA: Secondary hyperparathyroidism may persist or recur because of hyperfunction of the parathyroid remnant or transplanted parathyroid tissue. It is a great challenge to localize the parathyroid tissue either in the neck or at the arm before surgery. METHODS: From June 1994 to June 2000, 15 patients with recurrent and persistent secondary hyperparathyroidism were selected for surgery for the removal of parathyroid tissue. The indications for surg

IMMEDIATE POST-ANAESTHESIA CONFUSION AND AGITATION IN ADULT SURGICAL PATIENTS: INCIDENCE AND CLINICAL CORRELATES.

BACKGROUND: Cognitive disturbances during recovery from anaesthesia are clinically important but often underrecognized in adult surgical patients. Immediate confusion or agitation during the emergence phase may represent transient neurocognitive changes related to anaesthetic effects rather than true postoperative delirium. Early identification of these disturbances in the post-anaesthesia care unit (PACU) is essential for patient safety and appropriate postoperative monitoring. AIM: This study aimed to determine the incidence of immediate post-anaesthesia confusion and agitation at PACU admi

A Study of the Differences in Central Venous Pressure Between the Distal Lumen and Proximal Lumen of Central Venous Catheters in Pediatric Patients.

BACKGROUND: The choice of a central venous catheter (CVC) lumen to connect the central venous pressure (CVP) measurement line varies by facility. However, if the CVP values differ based on the connected CVC lumen, this variation could significantly affect the interpretation of the CVP measurements, raising major concerns regarding circulatory management of the patient. AIMS: This study aimed to determine whether a difference exists in the CVP values between the proximal and distal lumens of the CVC in small pediatric patients undergoing cardiac surgery. METHODS: Seventeen pediatric patients

Current Practices and Priorities of Anesthetists and Consumers for Infants Undergoing Inguinal Hernia Surgery.

INTRODUCTION: There is a paucity of data on the chosen anesthesia management for infant inguinal hernia surgery. We aimed to characterize self-reported anesthetic practice in Australia and New Zealand. We also aimed to identify the outcomes that matter to both anesthetists and to parents and carers. METHODS: Two separate surveys were administered, one for anesthetists and the other for parents and carers of infants who had undergone an inguinal hernia operation. RESULTS: Eighty-nine surveys were completed by anesthetists. The most common preferred anesthetic technique was general anesthesia

Does dexmedetomidine infusion reduce the postoperative analgesic need in lumbar disc surgery?

BACKGROUND/AIM: Patients experience moderate-to-severe pain, especially in the first days after lumbar disc surgery. Poorly controlled pain in the postoperative period negatively affects patient outcomes. Dexmedetomidine is a highly selective α2 adrenoceptor agonist with demonstrated analgesic efficacy. However, conflicting results have been reported in the current literature regarding the efficacy of dexmedetomidine in this surgery. In this study, we tested the hypothesis that dexmedetomidine safely improves pain scores and reduces opioid consumption in lumbar microdiscectomy. MATERIALS AND

A Latent Profile Analysis of Risk Perception in Breast Cancer-Related Lymphedema Among Women With Breast Cancer.

BACKGROUND: Breast cancer-related lymphedema (BCRL) is a common postoperative complication that impairs arm function and diminishes quality of life. Consistent engagement in self-management practices is crucial for the early prevention of BCRL. Since risk perception is a key determinant of behaviour change, it may serve as a critical target for improving adherence to BCRL self-management among patients with breast cancer. This research aimed to evaluate the profiles and determinants of risk perception related to BCRL in women after breast cancer surgery. METHODS: A cross-sectional study invol

Decreased endothelial cell retinoic acid signaling accelerates progression of single ventricle pulmonary arteriovenous malformations.

BACKGROUND: Pulmonary arteriovenous malformations (PAVMs) are vascular complications that universally develop in patients with single ventricle congenital heart disease after Glenn surgery. However, the pathophysiological mechanisms underlying single ventricle PAVMs are poorly understood. To comprehensively evaluate molecular changes post-Glenn, we performed single-cell RNA sequencing (scRNAseq) on rat lung samples after Glenn surgery. METHODS: Using adult Sprague Dawley rats, we performed scRNAseq on unfiltered lung samples 3 weeks after left-sided Glenn or sham surgery. We compared endothel

Effect of S-Ketamine on Postoperative Delirium in Elderly Patients Undergoing Arthroplasty: A Randomized Controlled Trial.

BACKGROUND: General anesthesia may counteract the neuroprotective effects of S-ketamine, so previous trials may have underestimated the efficacy of S-ketamine to reduce postoperative delirium (POD). The authors hypothesized that administration of S-ketamine in a non-general anesthesia setting would reduce the risk of POD. METHODS: This study was conducted at a high-volume arthroplasty referral center where standardized, evidence-based perioperative protocols are implemented with high fidelity. This study included 372 elderly patients undergoing total hip or knee arthroplasty under neuraxial a
