A Computational Framework to Analyze the Associations Between Symptoms and Cancer Patient Attributes Post Chemotherapy Using EHR Data.

Patients with cancer, such as breast and colorectal cancer, often experience different symptoms post-chemotherapy. The symptoms could be fatigue, gastrointestinal (nausea, vomiting, lack of appetite), psychoneurological symptoms (depressive symptoms, anxiety), or other types. Previous research focused on understanding the symptoms using survey data. In this research, we propose to utilize the data within the Electronic Health Record (EHR). A computational framework is developed to use a natural language processing (NLP) pipeline to extract the clinician-documented symptoms from clinical notes.

LPAQR-Net: Efficient Vertebra Segmentation From Biplanar Whole-Spine Radiographs.

Vertebra segmentation from biplanar whole-spine radiographs is highly demanded in the quantitative assessment of scoliosis and the resultant sagittal deformities. However, automatic vertebra segmentation from the radiographs is extremely challenging due to the low contrast, blended boundaries, and superimposition of many layers, especially in the sagittal plane. To alleviate these problems, we propose a lightweight pyramid attention quick refinement network (LPAQR-Net) for efficient and accurate vertebra segmentation. The LPAQR-Net consists of three components: (1) a lightweight backbone netwo

A Home-Based Bilateral Rehabilitation System With sEMG-based Real-Time Variable Stiffness.

Bilateral rehabilitation allows patients with hemiparesis to exploit the cooperative capabilities of both arms to promote the recovery process. Although various approaches have been proposed to facilitate synchronized robot-assisted bilateral movements, few studies have focused on addressing the varying joint stiffness resulting from dynamic motions. This paper presents a novel bilateral rehabilitation system that implements a surface electromyography (sEMG)-based stiffness control to achieve real-time stiffness adjustment based on the user's dynamic motion. An sEMG-driven musculoskeletal mode

A Controlled Thermoalgesic Stimulation Device for Exploring Novel Pain Perception Biomarkers.

OBJECTIVE: To develop a new device for identifying physiological markers of pain perception by reading the brain's electrical activity and hemodynamic interactions while applying thermoalgesic stimulation. METHODS: We designed a compact prototype that generates well-controlled thermal stimuli using a computer-driven Peltier cell while simultaneously capturing electroencephalography (EEG) and photoplethysmography (PPG) signals. The study was performed on 35 healthy subjects (mean age 30.46 years, SD 4.93 years; 20 males, 15 females). We first determined the heat pain threshold (HPT) for each s

Isotypes of autoantibodies against novel differential 4-hydroxy-2-nonenal-modified peptide adducts in serum is associated with rheumatoid arthritis in Taiwanese women.

BACKGROUND: Rheumatoid arthritis (RA) is an autoimmune disorder with systemic inflammation and may be induced by oxidative stress that affects an inflamed joint. Our objectives were to examine isotypes of autoantibodies against 4-hydroxy-2-nonenal (HNE) modifications in RA and associate them with increased levels of autoantibodies in RA patients. METHODS: Serum samples from 155 female patients [60 with RA, 35 with osteoarthritis (OA), and 60 healthy controls (HCs)] were obtained. Four novel differential HNE-modified peptide adducts, complement factor H (CFAH)1211-1230, haptoglobin (HPT)78-108

Activation of Superficial and Deep Finger Flexors Through Transcutaneous Nerve Stimulation.

OBJECTIVE: Functional electrical stimulation (FES) is a common technique to elicit muscle contraction and help improve muscle strength. Traditional FES over the muscle belly typically only activates superficial muscle regions. In the case of hand FES, this prevents the activation of the deeper flexor muscles which control the distal finger joints. Here, we evaluated whether an alternative transcutaneous nerve-bundle stimulation approach can activate both superficial and deep extrinsic finger flexors using a high-density stimulation grid. METHODS: Transverse ultrasound of the forearm muscles w

Deep Learning-Based End-to-End Diagnosis System for Avascular Necrosis of Femoral Head.

As the first diagnostic imaging modality of avascular necrosis of the femoral head (AVNFH), accurately staging AVNFH from a plain radiograph is critical yet challenging for orthopedists. Thus, we propose a deep learning-based AVNFH diagnosis system (AVN-net). The proposed AVN-net reads plain radiographs of the pelvis, conducts diagnosis, and visualizes results automatically. Deep convolutional neural networks are trained to provide an end-to-end diagnosis solution, covering tasks of femoral head detection, exam-view identification, side classification, AVNFH diagnosis, and key clinical notes g

Acoustic Emissions From Loaded and Unloaded Knees to Assess Joint Health in Patients With Juvenile Idiopathic Arthritis.

OBJECTIVE: We studied and compared joint acoustical emissions (JAEs) in loaded and unloaded knees as digital biomarkers for evaluating knee health status during the course of treatment in patients with juvenile idiopathic arthritis (JIA). METHODS: JAEs were recorded from 38 participants, performing 10 repetitions of unloaded flexion/extension (FE) and loaded squat exercises. A novel algorithm was developed to detect and exclude rubbing noise and loose microphone artifacts from the signals, and then 72 features were extracted. These features were down-selected based on different criteria to tr

Assessing the suitability of general practice electronic health records for clinical prediction model development: a data quality assessment.

BACKGROUND: The use of general practice electronic health records (EHRs) for research purposes is in its infancy in Australia. Given these data were collected for clinical purposes, questions remain around data quality and whether these data are suitable for use in prediction model development. In this study we assess the quality of data recorded in 201,462 patient EHRs from 483 Australian general practices to determine its usefulness in the development of a clinical prediction model for total knee replacement (TKR) surgery in patients with osteoarthritis (OA). METHODS: Variables to be used i

Discriminative Feature Network Based on a Hierarchical Attention Mechanism for Semantic Hippocampus Segmentation.

The morphological analysis of hippocampus is vital to various neurological studies including brain disorders and brain anatomy. To assist doctors in analyzing the shape and volume of the hippocampus, an accurate and automatic hippocampus segmentation method is highly demanded in the clinical practice. Given that fully convolutional networks (FCNs) have made significant contributions in biomedical image segmentation applications, we propose a notably discriminative feature network based on a hierarchical attention mechanism in hippocampal segmentation. First, considering the problem that the hi

Public health utility of cause of death data: applying empirical algorithms to improve data quality.

BACKGROUND: Accurate, comprehensive, cause-specific mortality estimates are crucial for informing public health decision making worldwide. Incorrectly or vaguely assigned deaths, defined as garbage-coded deaths, mask the true cause distribution. The Global Burden of Disease (GBD) study has developed methods to create comparable, timely, cause-specific mortality estimates; an impactful data processing method is the reallocation of garbage-coded deaths to a plausible underlying cause of death. We identify the pattern of garbage-coded deaths in the world and present the methods used to determine

1D Convolutional Neural Networks for Detecting Nystagmus.

Vertigo is a type of dizziness characterised by the subjective feeling of movement despite being stationary. One in four individuals in the community experience symptoms of dizziness at any given time, and it can be challenging for clinicians to diagnose the underlying cause. When dizziness is the result of a malfunction in the inner-ear, the eyes flicker and this is called nystagmus. In this article we describe the first use of Deep Neural Network architectures applied to detecting nystagmus. The data used in these experiments was gathered during a clinical investigation of a novel medical de

A clinical decision support system is associated with reduced loss to follow-up among patients receiving HIV treatment in Kenya: a cluster randomized trial.

BACKGROUND: Loss to follow-up (LFTU) among HIV patients remains a major obstacle to achieving treatment goals with the risk of failure to achieve viral suppression and thereby increased HIV transmission. Although use of clinical decision support systems (CDSS) has been shown to improve adherence to HIV clinical guidance, to our knowledge, this is among the first studies conducted to show its effect on LTFU in low-resource settings. METHODS: We analyzed data from a cluster randomized controlled trial in adults and children (aged ≥ 18 months) who were receiving antiretroviral therapy at 20 HIV

Facial Weakness Analysis and Quantification of Static Images.

Facial weakness is a symptom commonly associated to lack of facial muscle control due to neurological injury. Several diseases are associated with facial weakness such as stroke and Bell's palsy. The use of digital imaging through mobile phones, tablets, personal computers and other devices could provide timely opportunity for detection, which if accurate enough can improve treatment by enabling faster patient triage and recovery progress monitoring. Most of the existing facial weakness detection approaches from static images are based on facial landmarks from which geometric features can be c

Automatic Segmentation and Visualization of Choroid in OCT with Knowledge Infused Deep Learning.

The choroid provides oxygen and nourishment to the outer retina thus is related to the pathology of various ocular diseases. Optical coherence tomography (OCT) is advantageous in visualizing and quantifying the choroid in vivo. However, its application in the study of the choroid is still limited for two reasons. (1) The lower boundary of the choroid (choroid-sclera interface) in OCT is fuzzy, which makes the automatic segmentation difficult and inaccurate. (2) The visualization of the choroid is hindered by the vessel shadows from the superficial layers of the inner retina. In this paper, we

Online Cost-Effectiveness ANalysis (OCEAN): a user-friendly interface to conduct cost-effectiveness analyses for cervical cancer.

BACKGROUND: Most cost-effectiveness analyses in the context of cervical cancer prevention involve the use of mathematical models to simulate HPV infection, cervical disease and prevention strategies. However, it is common for professionals who would need to perform these analyses to not be familiar with the models. This work introduces the Online Cost-Effectiveness ANalysis tool, featuring an easy-to-use web interface providing health professionals, researchers and decision makers involved in cervical cancer prevention programmes with a useful instrument to conduct complex cost-effectiveness a

A predictive analytics model for differentiating between transient ischemic attacks (TIA) and its mimics.

BACKGROUND: Transient ischemic attack (TIA) is a brief episode of neurological dysfunction resulting from cerebral ischemia not associated with permanent cerebral infarction. TIA is associated with high diagnostic errors because of the subjective nature of findings and the lack of clinical and imaging biomarkers. The goal of this study was to design and evaluate a novel multinomial classification model, based on a combination of feature selection mechanisms coupled with logistic regression, to predict the likelihood of TIA, TIA mimics, and minor stroke. METHODS: We conducted our modeling on c

Predicting the amputation risk for patients with diabetic foot ulceration - a Bayesian decision support tool.

BACKGROUND: Diabetes mellitus is a major global health issue with a growing prevalence. In this context, the number of diabetic complications is also on the rise, such as diabetic foot ulcers (DFU), which are closely linked to the risk of lower extremity amputation (LEA). Statistical prediction tools may support clinicians to initiate early tertiary LEA prevention for DFU patients. Thus, we designed Bayesian prediction models, as they produce transparent decision rules, quantify uncertainty intuitively and acknowledge prior available scientific knowledge. METHOD: A logistic regression using o

Adaptive-Guided-Coupling-Probability Level Set for Retinal Layer Segmentation.

Quantitative assessment of retinal layer thickness in spectral domain-optical coherence tomography (SD-OCT) images is vital for clinicians to determine the degree of ophthalmic lesions. However, due to the complex retinal tissues, high-level speckle noises and low intensity constraint, how to accurately recognize the retinal layer structure still remains a challenge. To overcome this problem, this paper proposes an adaptive-guided-coupling-probability level set method for retinal layer segmentation in SD-OCT images. Specifically, based on Bayes's theorem, each voxel probability representation

Tear Film Classification in Interferometry Eye Images Using Phylogenetic Diversity Indexes and Ripley's K Function.

Dry eye syndrome is one of the most frequently reported eye diseases in ophthalmological practice. The diagnosis of this disease is a challenging task due to its multifactorial etiology. One of the most applied tests is the manual classification of tear film images captured with the Doane interferometer. The interference phenomena in these images can be characterized as texture patterns, which can be automatically classified into one of the following categories: strong fringes, coalescing strong fringes, fine fringes, coalescing fine fringes, and debris. This work presents a method for classif
