Clinical practice guidelines of foot care practice for patients with type 2 diabetes: A scoping review using self-care model.

BACKGROUND: Type 2 diabetes can lead to severe foot complications, making self-care education, guided by clinical guidelines, crucial. However, fragmented and dispersed recommendations challenge effective implementation of these guidelines. Bringing together recommendations and presenting them according to a self-care model can provide a solid framework and facilitate the interpretation of results. AIMS: to map the international guidelines that provide recommendations to nurses to enable people with type 2 diabetes for foot self-care and synthesize the recommendations according to the key con

Modeling Interventions to Reduce the Spread of Multidrug-Resistant Organisms Between Health Care Facilities in a Region.

IMPORTANCE: Multidrug-resistant organisms (MDROs) can spread across health care facilities in a region. Because of limited resources, certain interventions can be implemented in only some facilities; thus, decision-makers need to evaluate which interventions may be best to implement. OBJECTIVE: To identify a group of target facilities and assess which MDRO intervention would be best to implement in the Shared Healthcare Intervention to Eliminate Life-threatening Dissemination of MDROs in Orange County, a large regional public health collaborative in Orange County, California. DESIGN, SETTING

Magnetic Stent Removal in a Nurse Led Clinic; a Nine Month Experience.

Ureteric stents are frequently inserted post endourological procedures. However, subsequent endoscopic stent removal requires a second procedure for the patient and the availability of necessary resources. Longer duration of indwelling stents can lead to increased risk of symptoms and complications. The use of magnetic stents removed with a magnetic retrieval device (BlackStar©), offers an alternative which obviates the need for cystoscopy. We assessed the outcomes for this novel method of stent removal in our institution. A retrospective analysis was performed of all patients undergoing magne

Variation in use of antipsychotic medications in nursing homes in the United States: A systematic review.

BACKGROUND: The use of antipsychotic medications (APMs) in nursing home residents in the U.S. is an increasingly prominent issue and has been associated with increased risk of hospitalization, cardiovascular events, hip fractures, and mortality, among other adverse health events. The Food and Drug Administration has placed a black box warning on these drugs, specifying that they are not meant for residents with dementia, and has asked providers to review their treatment plans. The purpose of this systematic PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-analyses)-based revie

Blocked urinary catheters: can they be better managed?

This article discusses one of the problems associated with urinary catheterisation. It focuses on catheter blockage and explains the effects of this on patients and the health service and aims to guide nurses in their management of this complex issue. The length of time a catheter remains functional is unique to the individual and imperative to good catheter care. Coupled with this the cause of the blockage needs to be identified before a treatment plan can be formulated. Encrustation is identified as a major problem and the reliability of using the pH value of urine to monitor is discussed. A

Administering 'top-ups' of epidural analgesia.

Repeated annual audit cycles revealed an unacceptable failure rate of 38%-46% in epidural analgesia for surgical patients in our organisation. Reasons for failure included unilateral block, missed segments and catheter migration. In spite of interventions to remedy the situation, the success rate could not be improved. The aim of the initiative outlined in this article was to improve the efficacy of epidural analgesia and to reduce the failure rate. We found that following the appropriate training and assessment, nurse-administered diamorphine top-ups are a safe and effective way to improve th

Review of epidemiological studies on the occupational risk of tuberculosis in low-incidence areas.

This review summarizes the epidemiological evidence for occupationally acquired tuberculosis and considers the implications for the prevention of tuberculosis. The relevant epidemiological studies were identified on the basis of the Medline data bank, starting with the year 1966. The evaluation of occupational groups with an elevated tuberculosis risk is exclusively based on epidemiologic studies of good or acceptable quality, applying clearly defined criteria of methodological quality. In summary, the available epidemiological evidence suggests that the risk of tuberculosis is elevated in the

Understanding urine testing.

Urine is a natural waste product of the body and urine testing is a simple and quick way of monitoring disease and identifying new illness. The results from a urine test can prompt a change in a patient's medical management. Urine testing is not a new procedure: as far back as the ancient Egyptians there are descriptions of polyuria. In 7th Century AD, Protosharis suggested that it was important to look at the colour of urine and by 1674 Thomas Willis, a professor at Oxford University, recorded that the urine of his patients with diabetes tasted sweet (Beer, 1996).

Parents on the web: risks for quality management of cough in children.

BACKGROUND: Health information on the Internet, with respect to common, self-limited childhood illnesses, has been found to be unreliable. Therefore, parents navigating on the Internet risk finding advice that is incomplete or, more importantly, not evidence-based. The importance that a resource such as the Internet as a source of quality health information for consumers should, however, be taken into consideration. For this reason, studies need to be performed regarding the quality of material provided. Various strategies have been proposed that would allow parents to distinguish trustworthy

Feedback to nursing staff as an intervention to reduce catheter-associated urinary tract infections.

Because of high incidence of catheter-related urinary tract infections (UTIs) in our Veterans Affairs medical center, we began providing nursing staff with unit-specific UTI rates. In our preintervention period, the first quarter of 1995, 38 infections occurred in 1186 catheter-patient-days or 32/1000 catheter-patient-days (95% CI, 22.9-43.7). Thereafter, nursing staff members were provided with a quarterly report with catheter-related UTI rates depicted graphically by unit. In the 18 months after this intervention, the mean UTI rate decreased to 17.4/1000 catheter-patient-days (95% CI, 14.6-2

Myths surrounding the use of urinary catheters: a summary of key beliefs that inhibit acute care nurses from altering their use of urinary catheters.

Nurses frequently base their use and care of indwelling urinary catheters on long held beliefs that contemporary research is disproving. As an excellent place of attachment, the catheter permits rapid migration of organisms into the bladder, independent of the position of the catheter bag. For treatment of bladder distension, intermittent catheterization is preferable to leaving an indwelling catheter in place. Regardless of the amount of urine obtained, intermittent catheters are usually not intended to remain in the bladder. Irrigation of a bypassing urinary catheter forces organisms into th

Advances in assessment systems and non-pharmacological intervention strategies for cancer-related fatigue following transarterial chemoembolization in hepatocellular carcinoma: a narrative review.

PURPOSE: Transarterial chemoembolization (TACE) is a primary treatment for intermediate to advanced hepatocellular carcinoma (HCC); however, cancer-related fatigue (CRF) is a prevalent and distressing symptom following TACE that significantly impairs patients' recovery and health-related quality of life. This review aimed to systematically summarize the current evidence on CRF screening, assessment, and non-pharmacological interventions specifically tailored to the post-TACE HCC population, and to propose an integrated, nurse-led clinical management framework. METHODS: A narrative review was
