Additional distraction osteogenesis after conventional fronto-orbital advancement.

In the conventional fronto-orbital advancement method, there is a limit to advancement because of scalp skin tension and an absence of a supraorbital bar fixating point. In a case of insufficient advancement after the primary operation, a secondary re-advancement must be performed. In such a condition, additional fronto-orbital advancement by distraction osteogenesis has proved to be very useful. The authors used additional distraction osteogenesis in three infant cases: two of nonsyndromic craniosynostosis and one of Apert's syndrome. They were able to perform these operations safely using th…

Treatment of zygomatic fractures without inferior orbital rim fixation.

Zygomatic fractures can be associated with functional and esthetic problems. Recent improvements in surgical techniques and materials have enabled stable fixation of zygmomatic fractures. Multiple-point fixation is most commonly used for internal fixation. Generally, reduction and fixation are performed through lateral brow, subciliary, temporal, or intraoral incisions (three-point fixation). Our experience indicates that postoperative scarring and sensory disturbances are caused by a subciliary incision with inferior orbital rim fixation. It is thus recommended that inferior orbital rim fixat…

Use of ivermectin in the treatment of orbital myiasis caused by Cochliomyia hominivorax.

Orbital myiasis cases by dipterous fly larvae are rarely reported. A case of massive orbital myiasis caused by Cochliomyia hominivorax resulting in complete loss of the globe is described. Oral ivermectin was used successfully to eliminate the larvae prior to enucleation, making the mechanical removal of larvae an unnecessary procedure.

[Orbital decompression in graves disease: indications, techniques, results and complications].

BACKGROUND: The surgical rehabilitation of patients with Graves disease involves orbital decompression and various lid and extraocular muscle procedures. METHODS: We have reviewed the literature and include a presentation of our own results. RESULTS: The indications for orbital decompression include not only functional reasons (optic neuropathy, keratopathy, glaucoma, pain) but also aesthetic and psychosocial reasons without visual problems. Current techniques for orbital decompression (bone versus fat removal) are described and discussed. Results demonstrating a mean reduction of proptosis …

Orbital swelling as a sign of live Dirophilaria repens in subconjunctival tissue.

This report presents a case of live dirophilaria found in the subconjunctival space in a woman, in whom the main symptoms were swelling of the orbital tissue. After subconjunctival anaesthesia the dirophilaria were removed surgically. A histopathological examination revealed an adult, non-gravid female Dirofilaria repens s. acutiuscula. To the authors' knowledge this is the first case of subconjunctival live dirophilaria reported in a human.

Orbital complications in endoscopic sinus surgery using powered instrumentation.

OBJECTIVES/HYPOTHESIS: Powered dissection is increasingly used in endoscopic sinus surgery. Although it has certain clear advantages over conventional instrumentation, powered dissection also presents special liabilities. The objectives of the report are to highlight the special dangers of powered instrumentation near the orbit, to suggest techniques that might minimize these risks, and to review the management of orbital injuries. STUDY DESIGN: Case series. METHODS: A retrospective review was performed of patients who were referred to our institution with orbital injury complicating endosco…

[Emergency treatment of post-traumatic orbital emphysema: a case report].

PURPOSE: Orbital emphysema is a common complication of orbital wall fractures (50% of blow-out fractures). It is generally a benign and self-limited condition. However, it may compromise retinal and optic nerve vascularization and therefore requires rapid management. PATIENT AND METHOD: We report a case of traumatic orbital emphysema resulting in ischemic optic neuritis. The orbital emphysema was drained using a 25-gauge needle mounted on a syringe filled with normal saline, with the plunger removed, and guided by air bubble eruption. RESULTS: Vision was promptly recovered following drainage…

Clinical and pathological features of intracranial chordoma with orbital involvement in a child.

We examined a 44-month-old girl with proptosis of the left eye. Magnetic resonance imaging showed a dumbbell-shaped mass involving the cavernous sinus with orbital extension. Biopsy and immunohistochemistry showed the tumor to be a primitive chordoma. Intracranial chordoma should be considered in the differential diagnosis of children with orbital lesions. Immunohistochemistry is helpful in differentiating chordomas from other primitive round-cell or spindle-cell tumors.

Pediatric orbital floor fractures: nausea/vomiting as signs of entrapment.

OBJECTIVE: We sought to explore whether nausea and vomiting are predictive of entrapment. Study design and setting We retrospectively evaluated the data regarding orbital floor fractures in children younger than 19-years-old presenting at a tertiary care center from 1990 to 2001. Zygomatic, naso-orbital ethmoid, and displaced orbital rim fractures were excluded from our analysis. Data on the fracture type, signs and symptoms, ocular motility, surgical repair, and resolution of diplopia were also collected. RESULTS: Twenty-nine orbital floor fractures were identified. One fourth of the childre…

Strain gauge biomechanical evaluation of forces in orbital floor fractures.

Since the first description of orbital blow-out fractures, there has been much confusion as to their aetiology. Two principal mechanisms have been proposed to explain these fractures, the buckling and hydraulic mechanisms, caused by trauma to the orbital rim and the globe of the eye, respectively. Previous experimental and clinical studies have aimed to support one or other of these two theories. However, these studies have failed to provide quantifiable data to objectively support their conclusions. We present the results of a study of these two proposed mechanisms under identical conditions,…
