Tubular and mucinous breast cancer: results of a cohort of 917 patients.

OBJECTIVES:: To analyze axillary lymph node involvement (ALNI) rate and survival for mucinous (MC) and tubular (TC) breast carcinomas considered being of very good prognosis and for which an axillary surgical exploration could be questioned. METHODS:: Our multicentric cohort consisted of 21,135 patients with clinically node-negative invasive breast cancer, without neoadjuvant therapy, between 1999 and 2013 in 10 French centers. ALNI rate and survival were analyzed according to patient and tumor characteristics. RESULTS:: Our cohort consisted of 672 TC and 245 MC. Patients were older and tumo

Role of sentinel lymphadenectomy combined with intraoperative ultrasound in the assessment of locally advanced breast cancer after neoadjuvant chemotherapy.

BACKGROUND: Sentinel node (SN) biopsy for patients with locally advanced breast cancer after neoadjuvant chemotherapy results in a lower detection rate and higher false-negative rate. The aims of the study were to explore the role of SN biopsy in these patients in Taiwan and to assess the role of intraoperative ultrasound examination of the non-SN level. METHODS: We used a blue dye to identify the SNs in 127 patients with T3 locally advanced breast cancer initially treated with neoadjuvant chemotherapy. After SN biopsy, we used intraoperative ultrasound to explore the non-SN region for additi

Positron emission tomography in patients with breast cancer using (18)F-3'-deoxy-3'-fluoro-l-thymidine ((18)F-FLT)-a pilot study.

BACKGROUND: This pilot study investigated the feasibility of (18)F-3'-deoxy-3'-fluoro-l-thymidine ((18)F-FLT) as a positron emission tomography (PET) tracer for the visualisation of breast cancer. METHODS: Patients with breast cancer underwent (18)F-FLT-PET prior to surgery. The uptake of (18)F-FLT was determined in the primary tumour and in the axilla. RESULTS: Eight tumours were visualized by (18)F-FLT-PET with a mean uptake value (SUV(mean)) of 1.7 and mean tumour-non-tumour ratio (TNT) of 5.0. In seven patients, axillary lymph-node metastases were found at pathological examinations, howe

The evolution of surgery in the management of neck metastases.

In spite of advancement in science, molecular medicine and target therapies, surgical treatment of metastases using different techniques, from selective neck dissection to extended radical neck dissections, form a major part in the management of neck metastases. This is due to the fact that, so far, there is no treatment more effective for resectable neck metastases, than surgery. Since most head and neck cancer patients die due to loco-regional progression of disease, and a very large majority of them do not live long enough to develop distant metastases, the status of neck lymph nodes remain

Is systematic lymph node dissection mandatory or is sampling adequate in patients with stage I non-small-cell lung cancer?

A best evidence topic in thoracic surgery was written according to a structured protocol. The question addressed was: Is systematic lymph node dissection (SLND) mandatory or is sampling adequate in stage I non-small-cell lung cancer (NSCLC)? Two hundred and eleven papers were identified, of which 12 papers represented the best evidence to answer the clinical question. The authors, journal, date and country of publication, patient group studied, study type, relevant outcomes and the results of these papers are tabulated. There are 7 retrospective cohort reviews, 3 meta-analyses and 2 randomized

What should we intend for minimally invasive treatment of colorectal cancer?

Non-inferiority of laparoscopic treatment of colorectal cancer (CRC) has been demonstrated in randomized controlled trials although operative and perioperative management varies widely among centers. Literature data in English language published up to April 15, 2014 were analyzed in order to give an up to date analysis that would highlights the key aspects of a modern and factual minimally invasive treatment of CRC. Laparoscopic resection is the first choice treatment of colon cancer. Laparoscopic resection of rectal cancer should be considered an investigational procedure to be performed in h

Prognostic factors in cutaneous desmoplastic melanoma: a study of 252 patients.

BACKGROUND: Desmoplastic melanoma (DM) is a rare subtype of melanoma that is characterized by malignant spindle cells separated by prominent, fibrocollagenous stroma. Primary melanomas either may be entirely desmoplastic or almost entirely desmoplastic (pure DM [pDM]) or may exhibit a desmoplastic component admixed with a nondesmoplastic component (combined DM [cDM]). METHODS: Patients who were diagnosed between 1993 and 2007 at a single institution with clinically localized, primary cutaneous melanoma (PCM) that contained a desmoplastic component and who underwent sentinel lymph node (SLN) b…

A patient with stage 1a endometrial carcinoma in whom a solitary recurrent lesion was detected in the external iliac lymph node after MPA therapy.

BACKGROUND: In the treatment of endometrial carcinoma, young patients desire the preservation of the uterus, and therefore hormonal therapy has been administered. CASE REPORT: The patient was a 39-year-old nullipara diagnosed with stage 1a endometrial carcinoma. The patient desired the preservation of the uterus, and oral administration of MPA was prescribed for 18 weeks, which after the cancer tissue disappeared. However, about 1 year and 6 months later, the patient was diagnosed as having recurrent endometrial carcinoma in the left external iliac lymph node. CONCLUSION: In the literature,

Usefulness of Geriatric Parameters in Preoperative Evaluation of Patients Undergoing Minimally Invasive Surgery for Endometrial Cancer: A Retrospective Cohort Study.

INTRODUCTION: Patients operated for endometrial cancer (EMCA) are typically elderly with multiple comorbidities, potentially impacting surgical outcomes and survival. This study evaluated the prognostic value of frailty and frailty-related scores in predicting perioperative morbidity and survival in EMCA patients undergoing minimally invasive surgery. METHODS: This retrospective cohort study included 289 patients from the Sentinel Database treated for EMCA at Bern University Hospital (2012-2020). Patients underwent minimally invasive hysterectomy with sentinel lymph node dissection (39%) or a…

Severe Acute Radiation Dermatitis Following Ultra-Hypofractionated Whole Breast Radiation Therapy in a Patient With Systemic Lupus Erythematosus.

BACKGROUND: Whole-breast ultra-hypofractionated radiation therapy is an accepted adjuvant option for early-stage breast cancer, but evidence remains limited for patients with systemic lupus erythematosus (SLE), particularly those receiving chronic disease modifying antirheumatic drug (DMARD) therapy. CLINICAL SCENARIO: A 76-year-old woman with clinically quiescent SLE/Sjögren syndrome on mycophenolate mofetil was diagnosed with left breast invasive ductal carcinoma, ER/PR-positive and HER2-negative. After breast-conserving surgery and sentinel lymph node biopsy, pathology showed a 13-mm grade…

Gamma probe and ultrasound guided fine needle aspiration cytology of the sentinel node (GULF) trial - overview of the literature, pilot and study protocol.

BACKGROUND: Sentinel node (SN) biopsy (SNB) detects clinically occult metastases of breast cancer and melanoma in 20-30%. Wound infections, seroma and lymph edema occur in up to 10%. Targeted ultrasound (US) of the SN, (with fine needle aspiration cytology (FNAC) if appropriate) has been investigated as a minimally invasive alternative, but reported sensitivity rates are too low to replace SNB. Our hypothesis is that the use of a handheld gamma probe concomitant with US may improve sensitivity. Our aim is to provide an overview of the current literature on preoperative nodal staging of clinica

SENOPAUSE: Evaluation of an ASCO-Derived Clinical Proxy for Axillary Risk Stratification in a Retrospective Cohort.

BACKGROUND: Sentinel lymph node biopsy (SLNB) omission is increasingly considered in carefully selected postmenopausal patients with low-risk, early-stage breast cancer. However, the prevalence of occult nodal disease and wether nodal information alters adjuvant management remain key concerns in real-world implementation. METHODS: We conducted a retrospective, two-center observational study using an institutional REDCap database. Consecutive patients treated surgically between 2023 and 2024 were screened and a strict proxy of ASCO eligibility criteria for SLNB omission was applied (postmenopa…

De-escalating axillary surgery in early breast cancer: external validation of a retrospective cohort.

BACKGROUND: Sentinel lymph node biopsy (SLNB) is the standard for axillary staging in early breast cancer, yet its relevance is increasingly questioned in patients with favorable tumor biology when nodal status does not influence systemic treatment decisions. METHODS: We conducted a retrospective, single-center observational study including 162 women with clinically and sonographically node-negative (cN0) pT1 invasive breast cancer treated between 2019 and 2024 at a French university hospital. All patients underwent axillary staging and had complete molecular profiling. Among the 25 patients …

Predicting Lymph Node Metastases in Patients with Biopsy-Proven Ductal Carcinoma In Situ of the Breast: Development and Validation of the DCIS-met Model.

PURPOSE: In patients with a biopsy-proven ductal carcinoma in situ (DCIS), axillary staging is frequently performed, but in hindsight often turns out to be superfluous. The aim of this observational study was to develop a prediction model for risk of lymph node metastasis in patients with a biopsy-proven DCIS. METHODS: Data were received from the Dutch Pathology Databank and the Netherlands Cancer Registry. The population-based cohort consisted of all biopsy-proven DCIS patients diagnosed in the Netherlands in 2011 and 2012. The prediction model was evaluated with the area under the curve (AU

Vascular Endothelial Growth Factor Receptor-3 Expression Predicts Sentinel Node Status in Primary Cutaneous Melanoma.

This study analysed the expression of vascular endothelial growth factor-A (VEGF), VEGFR-2, and VEGFR-3 in primary cutaneous melanomas with positive and negative sentinel node status (SLN) (a total of 58 specimens divided into 2 groups of 29 for each status). The specimens were collected from the pathological archive of the department of Dermatology, Venereology and Allergology of the University Medical Center Heidelberg. A quantification score was developed for protein expression, by considering the percentage of positive melanoma cells (0: 0%, 1: up to 1%, 2: 2-10%, 3: 11-50%, and 4: > 50%)
