BACKGROUND: Obstructive sleep apnoea syndrome (OSAS) affects an estimated 2-4% of the middle aged population. Meta-analyses of randomised controlled trials have shown that the severe presentation of the syndrome (apnoea hypopnoea index (AHI) >30/hour) is effectively treated with continuous positive airway pressure (CPAP). Until recently there have been insufficient data to determine whether CPAP improves sleepiness in the larger subgroup with mild to moderate OSAS (AHI 5-30/hour).
METHODS: A systematic search of Medline and a hand search identified seven randomised controlled trials where CPA

BACKGROUND: A randomised controlled study was undertaken to examine the effect of nasal continuous positive airway pressure (CPAP) on 24 hour systemic blood pressure (BP) in patients with obstructive sleep apnoea (OSA).
METHODS: Patients were fitted with an ambulatory BP measuring device as outpatients during normal activities and recorded for 24 hours before starting therapeutic or subtherapeutic (4 cm H(2)O) CPAP treatment. BP monitoring was repeated before completion of 12 weeks of treatment. The primary end point was the change in 24 hour mean BP.
RESULTS: Twenty three of 28 participants

This project investigated the relationship between exposure to light and 24-h patterns of sleep and crying in young, healthy, full-term babies living at home and following a normal domestic routine. Measures included an ankle worn activity monitor, an external light monitor and the Barr Baby Day Diary in which parents recorded periods of sleep, crying, feeding and other behaviours at 5-min intervals throughout the 24-h period. Fifty-six babies (26 males and 30 females) were monitored across three consecutive days at 6, 9 and 12 weeks of age. There was an early evening peak in crying which was

The obstructive sleep apnea syndrome (OSAS) is caused by an intermittent and repetitive obstruction of the upper respiratory tract during sleep, which leads to a complete (apnea) or partial (hypopnea) block of air flow. It is quite prevalent, being seen in 4-6% of males and 2% of females. Structural abnormalities present in the upper respiratory tract and obesity are the fundamental etiological factors. Clinical manifestations are due to sleep fragmentation and oxygen desaturation which cause the apnea. Day hypersomnia, snoring and episodes of apnea described by the spouse are the three basic

We hypothesized that sleep-related violent behavior associated with parasomnias occurs as the result of a diathesis and is precipitated by stressors and mediated by disturbed nonrapid eye movement (NREM) sleep physiology. Sixty-four consecutive adult patients (mean age 30 years) who were investigated for sleepwalking or sleep terrors were categorized according to clinical history into three groups: serious violence during sleep to other people or to property or self (n = 26); harmful, but not destructive behavior (n = 12); and nonviolent behavior (n = 26). Log linear analysis showed that a dia

Creutzfeldt-Jakob disease (CJD) is a prion-related subacute encephalopathy producing widespread neuronal degeneration and spongiform pathological changes, especially in the neocortex. Progressive dementia, motor signs and electroencephalographic (EEG) alterations characterize the full stage of the disease. A series of eight 24-hour polygraphic recordings were carried out in the last 3 months of life of a 68-year-old female patient affected by CJD that was confirmed neuropathologically. Genetic classification demonstrated this patient to have a sporadic form of the disease. The polygraphic reco

A self-completion sleep evaluation questionnaire (SEQ), consisting of 10 cm line analogue rating scale questions, was constructed to investigate subjects' responses to aspects of sleep and early morning behaviour. The questions were grouped into 4 chronological areas: the ease of getting to sleep (GTS), the perceived quality of sleep (QOS), the ease of awakening from sleep (AFS), and the integrity of early morning behaviour following wakefulness (BFW). Five hundred and one SEQs were completed during several investigations into the comparative effectiveness of hypnotic drugs. The classical fact

BACKGROUND: Shift work disorder and insufficient sleep are prevalent among nurses, leading to fatigue, reduced well-being, and potential safety concerns. The increasing use of wearable sleep-tracking devices presents an opportunity to evaluate nurses' sleep quality objectively.
OBJECTIVE: The primary objective was to evaluate the feasibility of wearable-based sleep monitoring and to obtain preliminary evidence of agreement with validated actigraphy among nurses. The secondary objective was to describe nurses' sleep characteristics and to examine exploratory associations between sociodemograph

BackgroundPatients on hemodialysis commonly experience poor sleep quality, fatigue, and depression. Chronotype may influence these outcomes, but its role in this population remains unclear.ObjectivesThis study examined associations among chronotype, sleep quality, fatigue, and depression, and tested the mediating role of sleep quality.MethodsIn this cross-sectional study of 157 patients on hemodialysis (mean age, 63.99 years; 58.6% male), chronotype (Morningness-Eveningness Questionnaire), sleep quality (Pittsburgh Sleep Quality Index [PSQI]), fatigue (Fatigue Severity Scale [FSS]), and depres

The aim of the present study was to determine the direct effect of night-work on the occurrence of obstructive apneas during sleep after a night shift in fast-rotating shift workers with sleep-related breathing disorders. Eight obstructive sleep apnea patients were examined with the use of a polysomnograph during sleep under two conditions: after day-shift work and after night-shift work. Both sleep studies were conducted within 2 to 3 weeks of each other. In four of the 8 subjects, during sleep after a night-shift, an increase in apnea/hypopnea index was found. Night work significantly increa

SHINE syndrome is a rare neurodevelopmental disorder caused by mutations in DLG4, which encodes the postsynaptic scaffolding protein PSD-95. Key symptoms include sleep problems, hypotonia, intellectual disability, neurological disorders, and epilepsy, hence the name 'SHINE.' Here, we developed and characterized a mouse model of SHINE syndrome carrying the patient-derived DLG4V692Wfs*12/+ variant associated with a severe form of the disorder. The mutant transcript escapes nonsense-mediated decay but results in reduced PSD-95 protein expression, faithfully reproducing the molecular phenotype obs

Childhood maltreatment and adult sleep disturbance are significant, related public health concerns, but the extent to which they are associated over time is unknown. We investigated whether childhood maltreatment is associated with sleep disturbance over 20 years in a large, diverse, community sample of 1654 midlife women from 1996 to 2017 in a multicenter cohort study: The Study of Women's Health Across the Nation. Childhood maltreatment was assessed via The Childhood Trauma Questionnaire, and insomnia symptoms were assessed via self-report at 14 visits. Insomnia symptom trajectories were der

Menopausal symptoms can substantially impair women's quality of life, yet large-scale, population-based data from German-speaking regions are lacking. We used data of a cross-sectional online survey among 26,338 women in Germany, Austria, and Switzerland, using validated instruments such as the Menopause Rating Scale (MRS II), Insomnia Severity Index (ISI), and Alcohol Use Disorders Identification Test (AUDIT-C). Additional questions covered weight change, employment, nutrition, and physical activity. We investigated correlations and group differences using descriptive statistics and univariat
