A Case of Binocular Vertical Diplopia after Intratympanic Gentamicin Therapy.

UNLABELLED: Intratympanic gentamicin therapy is a useful alternative treatment for refractory Meniere's disease and is generally well tolerated. Visual disturbances as side effects of this treatment are rarely reported in the literature. In this report we describe the case of a 52-year-old woman with refractory Meniere's disease who developed binocular vertical diplopia following intratympanic gentamicin therapy. Spontaneous resolution of diplopia occurred within 2 weeks. The development of diplopia should be discussed as a potential complication with patients undergoing intratympanic gentamic

Non-canonical actions of Nogo-A and its receptors.

Nogo-A is a myelin associated protein and one of the most potent neurite growth inhibitors in the central nervous system. Interference with Nogo-A signaling has thus been investigated as therapeutic target to promote functional recovery in CNS injuries. Still, the finding that Nogo-A presents a fairly ubiquitous expression in many types of neurons in different brain regions, in the eye and even in the inner ear suggests for further functions besides the neurite growth repression. Indeed, a growing number of studies identified a variety of functions including regulation of neuronal stem cells,

Ménière's disease treatment: a patient-centered systematic review.

Ménière's disease is a disorder of the inner ear affecting hearing and balance to a varying degree. It is characterized by episodes of vertigo, low-pitched tinnitus, and hearing loss. There is currently no gold standard treatment for Ménière's disease. We conducted a systematic search of the Cochrane Database, as a high-quality source of evidence-based therapies, for reviews on the efficacy of etiological therapy or on Ménière's disease or its symptoms. Following recent positive experiences reported by other research teams, we decided to involve a patients' representative in the assessment and

CT and MR Imaging Characteristics of Intravestibular and Cerebellopontine Angle Lipoma.

Intracranial lipoma is an uncommon entity. A rare type of tumor in the internal auditory canal (IAC) and the cerebellopontine angle (CPA) is lipoma. There are a few case reports in the literature related to intravestibular lipoma. Herein, we report a case of lipomas within the cerebellopontine angle and vestibule of the inner ear in a patient with tinnitus and dizziness. The patient was evaluated with a 1.5 T magnetic resonance imaging (MRI) system. MRI and CT showed the masses in the left CPA and the left IAC. These lesions were hyperintense on both T1- and T2 weighted images and showed no en

On the biological plausibility of Wind Turbine Syndrome.

An emerging environmental health issue relates to potential ill-effects of wind turbine noise. There have been numerous suggestions that the low-frequency acoustic components in wind turbine signals can cause symptoms associated with vestibular system disorders, namely vertigo, nausea, and nystagmus. This constellation of symptoms has been labeled as Wind Turbine Syndrome, and has been identified in case studies of individuals living close to wind farms. This review discusses whether it is biologically plausible for the turbine noise to stimulate the vestibular parts of the inner ear and, by e

Intracochlear schwannoma extending to vestibule.

Intralabyrinthine schwannomas are benign tumors arising de novo from the perineural Schwann cell sheath of the intralabyrinthine branches of the vestibulocochlear nerve. The development of magnetic resonance imaging (MRI) has significantly increased the diagnosis of these tumors. Intralabyrinthine schwannomas can be sub-classified into seven groups according to the structures of the inner ear that are affected. The management strategy for intralabyrinthine schwannoma is regular monitoring with MRI, as the tumor grows slowly. In cases where the tumor growth is documented on serial MRI or distur

[Progresses and existed problems on the diagnosis and treatment of Vogt-Koyanagi-Harada disease].

Vogt-Koyanagi-Harada disease is a multi-systems inflammatory disease that affects the eye, skin, inner ear and meninges. The characteristics of this disease are with meningismus, auditory dysfunction and integumentary changes. Bilateral granulomatous panuveitis with serous retinal detachments are the typical ocular findings. Vogt-koyanagi-Harada disease is characterized by the acute onset, poor response to the treatment and high blindness rate. The diagnosis is mainly made on the history and ocular findings with various systemic manifestations. Fundus angiography is often used to assist in the

Case report: epitheliotropic lymphoma in a zebrafish (Danio rerio).

A proliferative disorder of lymphocytes was observed in a macroscopically normal young adult wild-type zebrafish (Danio rerio) during routine histological screening in a two-generation toxicity study. Proliferating lymphocytes were observed to invade the gill arches, infiltrate the cranial skeletal muscle and inner ear, and accumulate at distant sites in the frontal epidermis. The test compound, tetrabromobisphenol A (TBBPA, a flame retardant), is not known as a carcinogen and no tumours were detected in any of the 53 other fish in the study, including tank mates. Although neoplastic lymphoid

Molecular aspects of tinnitus.

Molecular changes caused by sensory trauma and subsequent structural alterations of the central nervous system are only beginning to be identified. In most cases, the generation of tinnitus can be linked to damage of the peripheral auditory system, probably even in cases where hearing impairment cannot be assessed by audiometry. Within a common view, acoustic trauma and salicylate induce abnormal excitability at the level of the brainstem, subcortical and cortical level that may be related to tinnitus. The present review summarizes studies emphasizing a crucial role of molecular events that oc

The effects of superior semicircular canal dehiscence on the labyrinth: does size matter?

BACKGROUND: Superior semicircular canal dehiscence (SSCD) is characterized by lack of bony covering of the superior semicircular canal in the inner ear, resulting in a third mobile window with altered functioning of the superior semicircular canal. Vertigo in association with sound and pressure changes often occurs. This study examines the relationship between dehiscence size and frequency of sound-induced vertigo. METHOD: Retrospective review of 22 patients with SSCD, noting the auditory frequency producing the maximal electronystagmographic response. RESULTS: The study found a correlation

Use of an infrared camera to improve the outcome of facial nerve monitoring.

Intraoperative cranial nerve neuromonitoring (IONM) has significantly reduced morbidity historically associated with skull-base procedures. It provides the surgeon with immediate feedback to preserve both anatomic and physiologic function of monitored nerves. However, when the surgeon cauterizes tissue near the nerve, the monitoring equipment is saturated by the cautery signal, eliminating IONM information at a critical time. The use of an infrared camera under the drapes to observe the face for movement due to nerve stimulation is an invaluable adjunct to traditional IONM. Monitoring may also

Type II transmembrane serine proteases in development and disease.

Recent advances in the mammalian genome projects have resulted in the identification of a surprisingly large number of genes encoding putative new members and even entire new families of proteolytic enzymes. In the past few years, one of these new families, type II transmembrane serine proteases (TTSPs), underwent a particularly rapid transformation from a group of predicted DNA and protein sequences into an established family of cell surface-associated proteases with important roles in the development and homeostasis of mammalian tissues such as heart, skin, inner ear, placenta, and digestive

[Sensorineural hearing loss in diabetes. Prosthetic care in hearing impaired patients].

Sensorineural hearing loss is more common in patients with diabetes than in the control nondiabetic patients, and severity of hearing loss seemed to correlate with progression of disease. This may be due to microangiopathic disease in the inner ear. References for diabetic microangiopathy are presented. Sensorineural hearing loss can often be helped by hearing aids. During the last decade there have been significant developments in hearing aid technology. Progress began with the presentation of programmable hearing aids in the late 1980's. The first hearing aids with fully digital signal proce

[Otogenic (labyrinthine) vertigo--when the ear fails to keep us in equilibrium].

The peripheral vestibular organ within the bony labyrinth of the inner ear is closely connected to the other parts of the equilibrium system. As a result of its constant active interaction with the other elements, it plays a major role in ensuring that we can maintain our balance. In the event of a disorder, otogenic vertigo can occur. Important evidence of a peripheral-vestibular disturbance is provided by the patient's history of dizziness, and confirmation of the suspected diagnosis is achieved by clinical and other appropriate examinations and tests. Common differential diagnoses include b

The genetics of vertigo.

Vertigo is a common principal complaint among patients seen by primary care physicians, neurologists, and otolaryngologists. The most common causes of recurrent episodes of vertigo are benign inner ear disorders, but central nervous system disorders must be excluded. Several common vertigo syndromes are now known to be familial, a feature distinguishing them from other common causes of vertigo. Familial vertigo has been recognized in patients with isolated recurrent attacks of vertigo, genetic deafness syndromes, and in patients with neurological disorders. Although susceptibility loci and mut

Human melanocyte biology, toxicology, and pathology.

The human melanocytes of the skin, hair, eyes, inner ears, and covering of the brain provide physiologic functions important in organ development and maintenance. Melanocytes develop from embryonic neural crest progenitors and share certain traits with other neural crest derivatives found in the adrenal medulla and peripheral nervous system. The distinctive metabolic feature of melanocytes is the synthesis of melanin pigments from tyrosine and cysteine precursors involving over 100 gene products. These complex biochemical mechanisms create inherent liabilities for melanocytic cells if intracel

Preserved otoacoustic emissions in postparotitis profound unilateral hearing loss: a case report.

We report a case of a profound unilateral sensorineural hearing loss following epidemic parotitis, with good response of otoacoustic emissions. The patient was a 12-year-old girl who had developed a unilateral hearing impairment 2 weeks after the onset of mumps. Pure tone audiometry confirmed a profound left sensorineural hearing loss. The affected ear showed an absence of auditory brain stem responses, whereas transient evoked otoacoustic emissions and distortion product otoacoustic emissions were preserved. Epidemic parotitis virus is likely responsible for an impairment of inner hair cells,

Efferent protection from acoustic injury is mediated via alpha9 nicotinic acetylcholine receptors on outer hair cells.

Exposure to intense sound can damage the mechanosensors of the inner ear and their afferent innervation. These neurosensory elements are innervated by a sound-activated feedback pathway, the olivocochlear efferent system. One major component of this system is cholinergic, and known cholinergic effects are mediated by the alpha9/alpha10 nicotinic acetylcholine receptor (nAChR) complex. Here, we show that overexpression of alpha9 nAChR in the outer hair cells of bacterial artificial chromosome transgenic mice significantly reduces acoustic injury from exposures causing either temporary or perman

Functional results in stapedotomy with and without CO(2) laser.

The authors have studied the use of the CO(2) laser in performing primary stapedotomy in 451 patients affected by otosclerosis, operated between 1996 and 2000. Footplate perforation was practised with a traditional method, by means of a microdrill, in 169 subjects. A CO(2) laser was instead used in other 282 cases; the authors effected the platinotomy with a single spot, repeated if necessary, with the diameter adjusted to the caliber of the prosthesis. This procedure allowed a smooth-edged perforation to be obtained. In the 282 laser-operated patients, there was a significantly higher proport

The site of the hearing loss in Refsum's disease.

Refsum's disease is a disorder of lipid metabolism with pigmentary retinopathy, demyelinating neuropathy, ataxia, and hearing loss. Previous histological studies have located the site of hearing impairment in the inner ear, but it has never been confirmed audiologicaly in the literature. In this reported case of Refsum's disease, despite hearing loss and absence of response in ABR, robust otoacoustic emissions were measured. Together with these and other audiological findings, we conclude that our case might be the first reported case of Refsum's disease with auditory neuropathy. The site of t
