A prospective 5-year study of exercise performance following Melody valve implant.

BACKGROUND: The long-term benefits of Melody valve implant for right ventricular outflow tract conduit obstruction or insufficiency on exercise capacity are undefined. METHODS: As part of the Melody valve clinical trial, 136 patients with congenital heart disease underwent serial cardiopulmonary exercise testing prior to, 6 months after, and annually for up to 5 years postimplant. RESULTS: Mean age at Melody valve implantation was 22.4 ± 0.9 years (range 7-53 years). The 95 patients who completed the study protocol provide the basis of this report. An initial improvement in % predicted workl

Surgical closure of a ventricular septal defect in early childhood leads to altered pulmonary function in adulthood: A long-term follow-up.

BACKGROUND: The long-term outlook after surgical closure of ventricular septal defect (VSD) has traditionally been considered benign. However, there is an increasing awareness of not only late cardiac dysfunction, but also pulmonary abnormalities. The primary aim of this study was to describe pulmonary function in adults with a surgically repaired VSD, and secondarily to determine the effects of salbutamol on the potential abnormalities. METHODS: All patients (operated for a VSD in early childhood) and controls (age- and gender-matched) underwent static and dynamic spirometry, impulse oscillo

Aerobic exercise training does not alter vascular structure and function in chronic obstructive pulmonary disease.

What is the central question of this study? Chronic obstructive pulmonary disease (COPD) is associated with endothelial dysfunction, arterial stiffness and systemic inflammation, which are linked to increased cardiovascular disease risk. We asked whether periodized aerobic exercise training could improve vascular structure and function in patients with COPD. What is the main finding and its importance? Eight weeks of periodized aerobic training did not improve endothelial function, arterial stiffness or systemic inflammation in COPD, despite improvements in aerobic capacity, blood pressure and

Determinants of functional capacity after mitral valve annuloplasty or replacement for ischemic mitral regurgitation.

OBJECTIVE: To identify the exercise echocardiographic determinants of long-term functional capacity, in patients with chronic ischemic mitral regurgitation, after restrictive mitral valve annuloplasty (RMA) or mitral valve replacement (MVR). METHODS: We retrospectively analyzed 121 patients with significant chronic ischemic mitral regurgitation, who underwent RMA (n = 62) or MVR (n = 59), between 2005 and 2011. Preoperatively, all patients underwent a resting echocardiographic examination, and a 6-minute walking test (6-MWT) to measure distance. Resting and exercise stress echocardiography, a

Relationship between serum cardiac troponin T level and cardiopulmonary function in stable chronic obstructive pulmonary disease.

BACKGROUND: High-sensitivity cardiac troponin T (hs-cTnT) in serum is a useful marker of acute myocardial injury, yet information is limited in patients with chronic obstructive pulmonary disease. We aimed to explore the association between hs-cTnT levels and cardiac and pulmonary dysfunction in patients with stable chronic obstructive pulmonary disease and at-risk individuals. METHODS: We examined community-dwelling adults with/without chronic obstructive pulmonary disease, with a life-long smoking history, current symptoms of dyspnea during exertion, prolonged coughing, and/or sputum. Mechanisms of right ventricular electromechanical dyssynchrony and mechanical inefficiency in children after repair of tetralogy of fallot.

BACKGROUND: Right bundle branch block and right ventricular (RV) dysfunction are common after tetralogy of Fallot repair (rTOF). We hypothesized that right bundle branch block is associated with specific RV mechanical dyssynchrony and inefficient contraction. METHODS AND RESULTS: We studied rTOF children and age-matched controls. QRS duration and morphology were assessed. RV mechanical dyssynchrony, indicated by early septal activation (right-sided septal flash), RV lateral wall prestretch/late contraction, postsystolic shortening, and intraventricular delay were analyzed using 2-dimensional

Chronic Thromboembolic Pulmonary Hypertension: A Narrative Review.

Chronic thromboembolic pulmonary hypertension (CTEPH) is a rare complication of acute pulmonary embolism (PE), which is chronically underdiagnosed. It is characterized by organized thromboembolic lesions within the pulmonary arteries, resulting in both obstruction of pulmonary arterial blood flow and pathologic small vessel remodeling. Patients present with dyspnea and decreased exercise tolerance; however, CTEPH patients are generally very comorbid, and these nonspecific symptoms often fail to initiate the diagnostic investigations. Late presentation results in poor outcomes with pulmonary hy…

Global prevalence of dyspnoea: a systematic review and meta-analysis.

BACKGROUND: Dyspnoea is a common subjective experience associated with decreased exercise tolerance and mortality. Although several studies have explored its prevalence in the general population, there has been limited synthesis over broad income settings, particularly low-income countries. This review and meta-analysis aims to address these gaps by reviewing the global prevalence of dyspnoea across low, middle, and high-income countries and associated factors. METHODS: A systematic review and meta-analysis were completed evaluating the prevalence of dyspnoea in the general population and ass…
