Arthrodesis of the ankle with a free vascularized autogenous bone graft. Reconstruction of segmental loss of bone secondary to osteomyelitis, tumor, or trauma.

Reconstruction after massive loss of bone about the ankle is difficult because of the limited amount of surrounding soft tissue and because of technical factors pertaining to adequate internal or external fixation. Conventional techniques are often unsuccessful because of the frequency of associated deep infection and of previous operative procedures. In this report, we describe eleven patients with a large defect of the distal aspect of the tibia who were managed at our institution with arthrodesis of the ankle with free vascularized bone graft. The defect was related to a tumor resection; an

The use of Ender nails in femoral shaft fractures: what are the remaining indications?

The use of Ender nails for the treatment of femoral shaft fractures has been described as technically easier and less time consuming than current intramedullary nailing techniques. We reviewed our results with unlocked Ender nails in 26 stable and 17 unstable fracture patterns an average of 3-4 years after injury. Because of continued instability, 42% of the stable and 76% of the unstable groups required adjunctive stabilization in the form of skeletal traction, a cast, or an external fixator. Additionally, nail migration and shortening and loss of motion at the knee were seen in 14 fractures

The treatment of noninfected pseudarthrosis of the femur and tibia with locked intramedullary nailing.

Sixty-six cases of noninfected nonunions (27 femoral and 39 tibial) were treated with an interlocking nail. In these cases, 92.6% of the femurs and 94.8% of the tibias healed after the first operation in a mean time of 15.4 and 13.0 weeks, respectively. Deep infection complicated five nailings (7.5%, two femur and three tibia), which included three patients with reactivation of latent osteomyelitis. All cases healed after debridement and use of gentamicin PMMA beads. In one case the nail had to be removed and replaced by an external fixator. Residual angulation between 5 degrees and 10 degrees

Segmental tibial shaft fractures.

A series of 54 cases with multiple fractures of the tibia is reported. Sixty-eight per cent of the group were pedestrians hit by a car. Fifty-two per cent were open fractures. Osteosynthesis was performed in 33 cases, whereas 21 were treated conservatively. Plate osteosynthesis was accomplished in 23 cases, seven had intramedullary nailing, and two were treated by Hoffman's external fixation device. Only one patient sustained a severe complication, viz., deep wound infection and osteomyelitis; however, even this infection was transient. All fractures healed except those sustained by two patien

Long-term results of pediculo-body fixation and posterolateral fusion for lumbar spondylolisthesis.

Grob et al. (Eur Spine J 5:281-285, 1996) illustrated a new fixation technique in inveterate cases of grade 2-3 spondylolisthesis (degenerative or spondylolytic): a fusion without reduction of the spondylolisthesis. Fixation of the segment was achieved by two cancellous bone screws inserted bilaterally through the pedicles of the lower vertebra into the body of the upper slipped vertebra. Since 1998 we have been using this technique according to the authors' indications: symptomatic spondylolisthesis with at least 25% anterior slippage and advanced disc degeneration. Afterwards this technique

Traumatic Case of Comminuted, Shattered Navicular Bone Fracture: A Rare Case of Midfoot Catastrophe.

INTRODUCTION: The navicular bone is a tarsal bone of the foot. It is an important bone that supports the medial arch of the foot. Navicular bone fractures are a rare injury. Fractures may occur as a result of stress or trauma. In both scenarios, it may be considered high risk due to poor blood supply to the bone. It is more common in athletes while kicking, sprinting, twisting, or falling. We report a case of a comminuted navicular fracture in an individual fixed with the Joshi external stabilization system (JESS). CASE REPORT: A 40-year-old male presented with injury to the right foot. The p

Atypical presentation of fat embolism syndrome: A case report.

We present a case report of fat embolism syndrome in the Trauma Intensive Care Unit with an atypical presentation. A 21-year-old male patient was admitted with multiple fractures after a motorcycle accident. Shortly after the debridement of the open fracture and external fixation of the bones, the patient presented with hypoxemia, tachypnea, fever, and tachycardia. There were no neurologic abnormalities and no petechial rash. Supportive care, including mechanical ventilation, was required. Initial chest angiotomography did not detect pulmonary thromboembolism. There were discrete bilateral con

Exploring bioactive bone cement pastes: A promising solution for orthopedic surgeries.

Bone cement is a biomaterial that has been used effectively in orthopedic surgery for almost 50 years. The cement hardens after injection in the defect site without needing any external agent. Bone cement is commonly used in implant fixation, joint replacement, trauma surgery, osteoporosis management, and antibiotic delivery. Since the bone anatomy of patients is different, the final hardening of bone cement in the body corrects the abnormalities and gaps between the implant and the bone. The most universal injectable bone cements are based on ceramic or polymer compounds. The chemical composi

Knee arthrodesis with modular megaprosthesis as salvage procedure for the limb following in a patient with an infected knee tumor prosthesis: A case report.

INTRODUCTION: The management of periprosthetic knee infections is a complex event, especially in the context of megaprostheses. An accepted option in these cases is knee arthrodesis, aiming to salvage the limb with acceptable and pain-free functionality. We present a successful case of knee arthrodesis using a hybrid model of modular nail-prosthesis in a patient with massive bone stock loss following the removal of an infected tumor-type prosthesis. CASE PRESENTATION: A 65-year-old male patient with a chronic periprosthetic infection related to a resection prosthesis implanted 5 years earlier

Charcot neuroarthropathy: Surgical and conservative treatment approaches.

Charcot neuroarthropathy (CN) is an inflammatory condition affecting the joints of patients with peripheral neuropathy; its prevalence is as high as 7.5%. It is commonly seen in patients with diabetes and poses a significant public health burden. CN often leads to severe morbidity, with complications including ulcerations, infections, and lower extremity amputations, and a 5-year mortality rate of 29.0%. The etiology of this condition is a combination of inflammation, neurotrauma, and altered bone metabolism, necessitating early identification and accurate diagnosis through clinical evaluation

Current Concepts in the Treatment of Traumatic C2 Vertebral Fracture : A Literature Review.

The integrity of the high cervical spine, the transition zone from the brainstem to the spinal cord, is crucial for survival and daily life. The region protects the enclosed neurovascular structure and allows a substantial portion of the head motion. Injuries of the high cervical spine are frequent, and the fractures of the C2 vertebra account for approximately 17-25% of acute cervical fractures. We review the two major types of C2 vertebral fractures, odontoid fracture and Hangman's fracture. For both types of fractures, favorable outcomes could be obtained if the delicately selected conserva

Missed Monteggia Fracture Dislocation in a 10-year-old Child - A Step-by-Step Approach to Properly Solving a Complex Problem: A Case Report.

INTRODUCTION: Monteggia injuries are relatively rare in the pediatric population. They can be missed leading to complications arising from a chronic radial head dislocation. There is limited information about their proper management in the literature. Thus, their treatment remains controversial. CASE REPORT: We present a case of a 10-year-old boy who was examined in the orthopedic outpatient clinic of our hospital, due to limited range of motion of his left elbow and difficulty in extending his left thumb, after a forearm injury which occurred 9 months before and was initially treated conserv

Efficacy of Infection Eradication in Antibiotic Cement-Coated Intramedullary Nails for Fracture-Related Infections, Nonunions, and Fusions.

Antibiotic cement-coated intramedullary nails (ACCINs) are increasing in popularity as a viable solution for the treatment of fracture-related infections (FRIs), infected long bone nonunions, and arthrodeses without an external fixator. ACCINs effectively manage to fulfill three of the basic principles for eradicating osteomyelitis: dead space management, antibiotic delivery, and bone stability. We performed a retrospective review of 111 patients who were treated with ACCINs between January 2014 and December 2020. In our series, 87.4% (n = 97) of patients achieved healed and uninfected bone or

Gender dysphoria: laparoscopic sigmoid vaginoplasty. Another utility of indocyanine green.

AIM: Gender dysphoria is the disagreement between the gender of birth and the one with which the patient identifies. For its management it is mandatory to have a multidisciplinary team. Gender confirmation surgery with penoscrotal skin flap is the procedure of choice, and a sigmoid vaginoplasty is a feasible alternative. The new technologies and the help of indocyanine green (ICG) fluorescence can help to guarantee a correct neovagina vascularization. The objective of this paper is to present the surgical technique of laparoscopic sigmoid vaginoplasty assisted by ICG. METHODS: We present two

Outcome of Induced Membrane Technique in Treatment of failed previously operated Congenital Pseudarthrosis of the Tibia.

BACKGROUND: Although a remarkable success in the treatment of congenital pseudarthrosis tibia (CPT) had been achieved, failure rate is still high and the likelihood of amputation is still considerable. The current study evaluates the outcome of induced membrane technique in the treatment of failed previously operated patients of congenital pseudarthrosis of the tibia. We hypothesized that induced membrane technique will improve union rates in CPT with failed previous multiple operations. PATIENTS AND METHODS: Nineteen consecutive patients of failed previously operated CPT were prospectively i

C1/C2 osteomyelitis secondary to malignant otitis externa complicated by atlantoaxial subluxation-a case report and review of the literature.

We present a case of C1/C2 osteomyelitis secondary to malignant otitis externa complicated by atlantoaxial subluxation. This case is unique because surgical fixation of the spine was delayed, and despite clearance of the infection with antibiotics, the patient developed cervical myelopathy and required instrumented spinal fusion surgery. He presented with 1 month of fever, headache and worsening neck stiffness. An MRI of his cervical spine showed C1/C2 osteomyelitis with atlantoaxial subluxation. He was initially treated non-operatively with prolonged intravenous antibiotics and external immob

Incidence of and Risk Factors for Complex Regional Pain Syndrome Type 1 after Surgery for Distal Radius Fractures: A Population-based Study.

This study aimed to evaluate the incidence rates of and risk factors for complex regional pain syndrome type 1 (CRPS-1) after surgery for distal radius fractures (DRFs). Using data from January 2007 to December 2014, we analysed the data from the Korean Health Insurance Review and Assessment (HIRA) service. After extracting the data of patients aged ≥18 years whose diagnostic and operation codes for DRFs were entered into the HIRA database, we analysed the incidence rates of and risk factors for CRPS-1. From 2007 to 2014, 172,194 DRFs were treated surgically. Within 1 year postoperatively, 1,1

The efficacy, safety, and cost benefits of splints for fractures of the distal radius in children: A systematic review and meta-analysis protocol.

BACKGROUND: Distal radius fractures (DRFs) is one of the most common bone injuries in children, which may lead to deformity and other complications if the treatment is not prompt or appropriate. Splints external fixation is a common conservative treatment for such fractures. Therefore, we conducted a systematic review and meta-analysis to explore the efficacy, safety and cost benefits of splints in the treatment of DRFs in children. METHODS: PubMed, Web of Science, Embase, Cochrane Library, Cochrane Central Register of Controlled Trials (CENTRAL), and ClinicalTrials.gov, Chinese National Know
