Intradialytic hypotension: is midodrine beneficial in symptomatic hemodialysis patients?

Symptomatic hypotension during hemodialysis is a disabling complication in end-stage renal disease (ESRD) patients, especially in certain groups of patients who are at higher risk for this problem. Autonomic dysfunction is thought to play a significant role. We evaluated the efficacy of midodrine, an oral agent with selective alpha-adrenergic agonist activity used in the treatment of neurogenic orthostatic hypotension, on 10 hemodialysis patients with persistent intradialytic hypotension. The patients were given a dose of midodrine (mean dose, 5.5 mg; range, 5 to 10 mg) 30 minutes before each

Contractile function and insulin sensitivity are inversely regulated by insulin in primary myoblast human skeletal muscle microtissues.

BACKGROUND: Insulin resistance in skeletal muscle is a hallmark of Type 2 diabetes mellitus (T2D). Three-dimensional skeletal muscle microtissues (hMMTs) offer advantages over two-dimensional myotube cultures, including enhanced physiological relevance and amenability to repeated contractile stimulation. However, standard hMMT differentiation protocols rely on supraphysiological insulin concentrations. This creates a fundamental conundrum, since the very conditions required to generate functional hMMTs are expected to render them insulin-unresponsive, limiting their utility for modeling T2D. W

A randomized controlled trial of adjunctive speleotherapy in asthma, COPD and long COVID.

Speleotherapy (underground climate therapy) is a non-pharmacological intervention for chronic respiratory diseases. This randomized controlled trial investigated whether a 3-week speleotherapy course (6 sessions, 2 h/week) improves respiratory outcomes in patients on standard background therapy with asthma, COPD, or Long COVID, and whether it affects blood CO2 levels. The control group did not receive speleotherapy but continued their standard therapy. A total of 208 patients (asthma: n = 107; COPD: n = 59; Long COVID: n = 42) were enrolled across nine centers in Germany, Austria, and Italy. A

Effect of exertion on the biomechanics of walking with load carriage and running in young, healthy women.

Musculoskeletal injuries, a major impediment to medical military readiness, are often associated with muscle fatigue caused by prolonged, repeated activities, including running and walking with load carriage. Here, we investigated the biomechanics of women running without a load and walking with load carriage to identify kinetic and kinematic parameters affected by exertion-related changes under both conditions. We collected motion-capture data and computed tomography images from 20 young, healthy women running without a load for 5 km at a subject-specific pace and walking with a 22.7-kg load …

Mind-Muscle-Environment Interactions: Psychophysiological Determinants of Optimal Pacing in Olympic Winter Endurance Sports.

Pacing is a critical determinant of performance in Olympic endurance winter sports such as cross-country skiing, biathlon, and speed skating. Although the physiological ("Движок") and psychological ("operator") determinants of pacing have frequently been examined in isolation, growing evidence highlights the need for an integrated psychophysiological perspective. This structured narrative review first outlines the defining characteristics of Olympic winter sports and, within this context, synthesizes current knowledge on the psychophysiological mechanisms governing pacing to inform optimal pre

The neuromechanics of the soleus for fall prevention in aging.

Falls are a leading cause of injury-related hospitalization, morbidity, and mortality in older adults, with impaired postural control serving as a key predictor of fall risk. The triceps surae, and particularly the soleus, plays a central role in maintaining upright stance by generating continuous plantarflexion moments that stabilize the body's center of mass. This mini-review summarizes evidence for the neuromechanical contributions of the soleus to postural stability and how these functions decline with age. Mechanically, the soleus acts as a brace for balance, providing sustained torque th
