[Abnormal vergence eye movement during eyelid closure caused by a pineal tumor].

A case with abnormal eye movement induced by eyelid closure and selective paralysis of downward gaze was reported. CT scan and MRI imaging revealed a pineal tumor with bilateral involvement of the thalamomesenchephalic junctions without obvious damage to the posterior commissure. Histopathologically, the tumor was a germinoma. The pupillary light reflex was absent and near reflex was reduced. Lightning eye movement, skew deviation and convergence-retraction nystagmus were observed. The horizontal eye movement was normal in both eyes. The eye did not move downward below the level of direct forw…

The use of diagnostic testing in asymmetric sensorineural hearing loss.

The etiology of an asymmetric sensorineural hearing loss can often be difficult to determine. Because a wide variety of pathologic processes may be responsible for the hearing loss, numerous diagnostic tests are usually used in the initial evaluation, including pure-tone audiometry, acoustic reflex testing, imaging, serologic testing, and auditory brainstem response testing. The diagnostic evaluations of 225 consecutive cases of asymmetric sensorineural hearing loss are reviewed. A cochlear site-of-lesion was demonstrated in the majority (194) of patients. Because all retrocochlear lesions (31…

Pupil responsiveness in cluster headache: a dynamic TV pupillometric evaluation.

In this study the variations in pupil diameter induced by different stimuli (dark-light adaptation, light reflex, electric stimulation of the sural nerve) were investigated in episodic (in the active or remission phases) and in chronic cluster headache (CH) patients. Pupil size monitoring was performed with a monocular, infrared TV pupillometer, and sural nerve stimuli were applied after the pain threshold had been measured as the flexion reflex threshold of the biceps femoris muscle (RIII reflex). The results were compared with those obtained in patients with "peripheral" (third neuron) Horne…

Manofluorography of deglutition after supraglottic laryngectomy.

Manofluorography (mano: manometry, fluoro: videofluoroscopy, graphy: picture) provides a simultaneous display of manometry and fluoroscopy on one video screen. This study uses manofluorography to analyze the swallowing patterns of nine patients who had undergone supraglottic laryngectomy. The results show that during swallowing the pharyngeal mechanism for preventing aspiration depends upon three processes: (1) tight lingual-laryngeal contact, (2) coordination of the swallowing reflex, and (3) tongue base and pharyngeal constrictor clearing of the hypopharynx and laryngeal inlet. Anterior susp…

The negative on/off effect in cochlear and early stapedial otosclerosis.

Acoustic reflex patterns were reviewed for 100 patients suspected of having cochlear otosclerosis (group 1) and 73 patients with confirmed stapedial otosclerosis and conductive hearing losses of less than 5 years duration (group 2). Three abnormal reflex patterns were exhibited by these patients--"negative on/off," "no response when the probe was in the affected ear," and "no response when the stimulus was in affected ear." Results showed that the negative on/off pattern was equally as common for the two subject groups. The "no response" patterns were more common for the early stapedial otoscl…

[Contralateral neuro-motor apparatus under unilateral restriction of motor activity in rat].

Electromyographic recordings of contralateral m. gastrocnemius of the rat after unilateral section of Achilles' tendon (tenotomy) were studied. Motor reflex, Hoffman reflex and firing of motor units of contralateral m. gastrocnemius were recorded in control and in 10 days after tendotomy. Motor units changed their firing in the background and in reaction to sciatic nerve stimulation. The results of study showed that unilateral section of Achilles' tendon increased the excitability of single motoneurons of contralateral spinal senters.

Late onset startle induced tics.

Three cases of late onset Gilles de la Tourette's syndrome are presented. The motor tics were mainly induced by an unexpected startling stimulus, but the startle reflex was not exaggerated. The tics developed after physical trauma or a period of undue emotional stress. Reflex tics may occur in Gilles de la Tourette's syndrome, but have not been described in late onset Tourette's syndrome. Such tics must be distinguished from psychogenic myoclonus and the culture bound startle syndromes.

ECG-related fasciculation potential.

We report the observation of an unusual fasciculation potential (FP). This FP was time-locked to the electrocardiogram (ECG) in 87% of the discharges. The motor unit action potential (MUAP) of the FP was also elicited as H-reflex. As mechanism we propose spindle afferent discharges mechanically triggered by the arterial pulse, i.e. a reflex fasciculation. We suggest that such a heartbeat-related mechanism could explain synchronous fasciculations described by Norris (Arch. Neurol., 1965, 13: 495-500).

Cranial nerve conduction and needle electromyography in patients with acoustic neuromas: a model of compression neuropathy.

Of 45 patients with acoustic neuromas (0.3-5.0 cm), 73% had facial nerve impairment on electrophysiologic testing, but only 16% had facial weakness. Cranial nerve conduction was the most sensitive measurement, especially prolongation of the ipsilateral R1 latency of the blink reflex compared with that of the contralateral reflex. The severity of nerve conduction abnormality was highly correlated with tumor size. Our results confirm and quantitate the sensitivity of nerve action potential latency in response to chronic nerve compression.

The role of prefrontal lobes in psychonervous memory in vertebrates.

The memory phenomena are classified into image memory, emotional memory and conditioned reflex memory. It is assumed that prefrontal area is critically involved in the image memory, and in particular, in short-term image memory. On the other hand, both emotional and conditianed reflex memory do not depend on the prefrontal area but they are controlled by other parts of the neocortex and by paleocortex. The intimate nature of both short-term and long-term memory is discussed.

Neurological examination in the healthy term newborn.

We carried out a cross-sectional study with a sample of 106 normal full-term newborns examined within 24 to 72 hours of birth. The following findings were evaluated: head and chest measurements, muscle strength, tone, tendon reflexes, superficial reflexes, primitive reflexes, and cranial nerves. All 106 newborns were considered neurologically normal. We found no differences in the neurological examination findings for newborns with different gestational ages. Primitive reflexes and appendicular tone in newborns examined at earlier postnatal ages tended to be less intense. We were able to deter…

Physiological mechanisms of onset adaptation and contralateral suppression of DPOAEs in the rat.

An investigation was undertaken to measure medial olivocochlear (MOC) reflexes in anesthetized rats before and after sectioning of the middle-ear muscles. Distortion product otoacoustic emission (DPOAE) magnitude and phase temporal responses were measured ipsilaterally to study MOC-mediated "DPOAE onset adaptation" and in the presence of a contralateral noise to study MOC-mediated contralateral "suppression" (terms as used by previous researchers). Distortion product otoacoustic emission onset adaptation and contralateral suppression had predictable changes in direction of magnitude and phase …

Disc herniation-related back pain impairs feed-forward control of paraspinal muscles.

STUDY DESIGN: A comparative study of lumbar paraspinal muscle reflexes during sudden upper limb loading in healthy control subjects and patients with sciatica. OBJECTIVES: To assess reflex activation of paraspinal muscles during sudden upper limb loading. SUMMARY OF BACKGROUND DATA: Sudden upper limb loading and upper limb voluntary movements cause reflex activation of trunk muscles. A short latency response of approximately 50 msec of lumbar muscles has been observed before, but the reflexes have not been studied in patients with sciatica. METHODS: The paraspinal muscle responses for upper…

[White pupil in an infant].

A whitish pupillary reflex (leukocoria) indicates abnormal reflection from intraocular pathology. In a child, leukocoria may be a sign of serious and even life-threatening eye disease (retinoblastoma), but the most common cause is congenital cataract. Both diagnoses require immediate referral to an ophthalmologist. Leukocoria is best detected by evaluating the reflex from the pupil with a handheld ophthalmoscope. We here present a case story of an infant with leukocoria that proved to be caused by unilateral congenital cataract.

The handgrip test - A historical test for diabetic autonomic neuropathy or a marker of something else?

Cardiovascular autonomic neuropathy (CAN) is a frequent complication of diabetes mellitus and is associated with increased morbidity and mortality in patients with diabetes. Hence, early and correct diagnosis of CAN is crucial. Standard cardiovascular reflex rests (CARTs) have been the gold standard of CAN assessment. Originally, CARTs consisted of five reflex tests, but measuring diastolic blood pressure response to sustained handgrip exercise has no longer been suggested as an established clinical test. Increasing body of evidence suggests that isometric handgrip test should no longer be use…

Asystole Triggered by the Mouth Opening With a Dental Mouth Gag Under General Anesthesia During Pediatric Oral Surgery: Report of a Rare Case.

The trigeminovagal reflex manifests as a sudden onset of bradycardia, hypotension, and cardiac arrest in response to the stimulation of the trigeminal nerve. The incidence of trigeminovagal reflex in maxillofacial surgical procedures is approximately 1.6%. We report a case of asystole in a pediatric patient in whom a dental mouth gag triggered the trigeminovagal reflex during oral surgery. The patient was a 5-year-old boy who was scheduled to undergo extraction of maxillary supernumerary teeth. After tracheal intubation, anesthesia was maintained with sevoflurane and remifentanil. At the begin…

Cardiac Arrest and Death Attributable to the "Diving Response" Triggered During Incision and Debridement of an Abscess of the Forehead.

The authors discuss about a patient who, while undergoing a routine procedure to drain a subcutaneous abscess within his forehead, suffered cardiac arrest that we conclude was caused by an activation of the diving response. This reflex affects homeostatic function which alters respiration and preferentially distributes oxygen stores to the heart and brain. Under some conditions, however, this reflex can also trigger cardiovascular collapse and death. The diving reflex is can begin with triggering receptors that are sensitive to cold water, submersion, or pressure within the nasal cavity and ot…
