Navigated and robotic-assisted sacroiliac joint fusion: a narrative review.

INTRODUCTION: Sacroiliac joint (SIJ) dysfunction accounts for up to 30% of axial low back pain and is increasingly managed surgically after failure of conservative care. While traditional open SIJ fusion carried significant morbidity, minimally invasive (MIS) approaches have transformed outcomes. More recently, stereotactic navigation and robotic assistance have emerged to enhance implant placement accuracy and reduce radiation exposure. AREAS COVERED: We conducted a narrative review of the literature (PubMed, EMBASE, Cochrane Reviews, inception) on SIJ fusion techniques, implant accuracy, co

Emerging in vitro models of corneal stromal fibrosis: Toward human-relevant platforms for anti-scarring therapeutics.

Corneal stromal fibrosis, characterized by the aberrant transformation of keratocytes into contractile, extracellular matrix-secreting myofibroblasts, is a major cause of irreversible visual impairment worldwide. Despite extensive studies in animal models, interspecies differences in immune regulation, stromal architecture, and wound healing limit their translational relevance. This review highlights recent advances in human-relevant in vitro models that aim to overcome these challenges. In vitro models increasingly mimic critical features of corneal stromal fibrosis, including keratocyte-myof

Intrinsic brain network dynamics modulated by neural stimulation to cerebellum.

The cerebellum, with its distinctive architecture and extensive cortical connections, has long been recognized for its highly structured interconnectivity with the cortex and has been proposed as part of a larger circuit that shapes brain network dynamics. Here, we evaluate dynamic network reconfigurations in resting-state fMRI connectivity pre- and post-noninvasive inhibitory repetitive transcranial magnetic stimulation targeting the right Crus I of the cerebellum. Using dynamic community detection to evaluate the stimulation's effect on modular network structures, we characterize the network

Falls Do Not Just Break Bones--They Break Lives: World Spinal Cord Injury Day (September 5, 2025) and Indian Perspectives.

Spinal cord injury (SCI) is a catastrophic condition that often results in lifelong disability, loss of independence, and profound social and economic consequences. While road traffic accidents remain a major cause, falls are increasingly emerging as a significant and preventable contributor to SCI, particularly in India. The theme of World SCI Day 2025--"Fall Prevention, Spinal Cord Protection"--highlights the urgent need to address this issue through proactive measures. Effective fall prevention requires a multipronged approach that combines home and community safety interventions, medical s

Genome-wide profiling of histone modifications and transcription factor binding at single-cell resolution by DeChIC-seq.

Mapping of protein-DNA interactions at single-cell resolution remains a central challenge in epigenomics, particularly for transcription factors (TFs), whose sparse binding limits reliable detection. Here, we establish DeChIC-seq (DNA Deaminase-based Chromatin Immuno-Conversion sequencing), a conversion-based strategy that uses a protein A-DddAtox fusion to directly record protein-DNA interactions by inducing localized C-to-U conversions near antibody-bound chromatin. Retaining genome-wide background sequence information without immunoprecipitation, DeChIC-seq enables profiling of histone modi

Vaccine Therapy for the Management of Penile Cancer: Evidence, Opportunities and Challenges.

Penile squamous cell carcinoma (PSCC) is a rare malignancy with limited therapeutic options in advanced and recurrent diseases. Advanced PSCC is typically managed with multimodal therapy, including neoadjuvant chemotherapy or chemoradiation followed by surgery; however, durable responses remain uncommon, and outcomes after recurrence are poor. Cancer vaccines represent a promising immunotherapeutic strategy, as these treatments induce tumor-specific immunity and heightened immune surveillance against penile cancer cells. While therapeutic cancer vaccines have not yet demonstrated consistent cl

Experimental Animal Models of Preeclampsia: Mechanisms, Strengths, and Translational Relevance.

CLINICAL PROBLEM: Preeclampsia is a heterogeneous hypertensive disorder of pregnancy characterized by new-onset hypertension and end-organ dysfunction, affecting 5% to 10% of pregnancies globally. It is associated with significant maternal and fetal morbidity and mortality, and delivery remains the only definitive treatment. Understanding the underlying mechanisms is essential for the development of effective therapies. RECOMMENDATIONS: Given the heterogeneity of preeclampsia and the absence of a single experimental model that fully recapitulates the human disease, the purpose of this review

Global analyses of genomic and epigenomic influences on gene expression reveal Serpina3n as a major regulator of cardiac gene expression in response to catecholamine challenge during heart failure.

Heart failure arises from maladaptive remodelling driven by genetic and epigenetic networks. Using a systems genetics framework, we mapped how DNA variants and CpG methylation shape cardiac transcriptomes during beta adrenergic stress in the Hybrid Mouse Diversity Panel, a cohort of over 100 fully inbred mouse strains. Expression QTLs (eQTLs), methylation QTLs (mQTLs) and methylation-driven eQTLs (emQTLs) were generated from over 13k expressed genes and 200k hypervariable CpGs in left ventricles. We discovered hundreds of regulatory 'hotspots' that control large portions of the genome, includi

Non-viral and mRNA-based strategies for next-generation CAR-T cell therapy.

Chimeric antigen receptor (CAR)-T cell therapy has transformed the treatment landscape for hematologic malignancies, but its broader application remains limited by the complexity, cost, manufacturing time, and integration-related risks associated with viral vector-based engineering. These constraints have become increasingly important as CAR-T strategies are being extended to solid tumors, autoimmune diseases, and in vivo CAR-T generation, while mRNA therapeutics and non-viral delivery technologies continue to advance. However, non-viral platforms and mRNA engineering are often discussed separ

Advances in microneedle design for the delivery of drugs, proteins, and cells in the treatment of hypertrophic scars.

Hypertrophic scars (HS) represent a significant clinical challenge due to their complex pathophysiology and resistance to conventional therapies, often resulting in persistent symptoms such as itching, pain, and impaired joint mobility that compromise patients' quality of life. Current treatment modalities, including compression therapy, pharmacological agents, radiation, silicone gel, and laser therapies, have faced limitations primarily due to inadequate drug penetration into the dense fibrotic scar tissue. In this context, microneedle-mediated controlled delivery systems have emerged as a p

The Evolving Role of Musculoskeletal Ultrasound in Gout: From Diagnosis to Management-A Narrative Review.

Gout, a prevalent and treatable form of crystal-induced arthritis, results from monosodium urate (MSU) crystal deposition in articular and periarticular tissues. Early diagnosis is crucial to prevent progression to chronic gouty arthritis, tophus formation, and structural joint damage. Musculoskeletal ultrasound (MUS) has emerged as a sensitive, noninvasive, and cost-effective imaging modality that enables the visualization of urate crystal deposition, evaluation of treatment response, and prediction of disease flares. This narrative review summarizes recent advances in MUS for the diagnosis a

Feasibility and Implementation Considerations for Long-Acting Antiretroviral Therapy in Pakistan: A Qualitative Study of Healthcare Providers and Programmatic Stakeholders.

IntroductionLong-acting injectable antiretroviral therapy (LAI-ART), especially cabotegravir plus rilpivirine, is primarily recommended as a switch option for virally suppressed individuals and may improve adherence, privacy, and treatment satisfaction. Contemporary international guidance notes a restricted, closely monitored role for selected viraemic patients with persistent adherence challenges. Feasibility in low-resource systems remains uncertain. We explored perceived feasibility and acceptability of LAI-ART among HIV care providers and regulatory stakeholders in Pakistan.MethodsWe condu

The ESCRT pathway and HIV-1 budding: A paradigm of viral exploitation of cellular regulation.

The ESCRT (Endosomal Sorting Complexes Required for Transport) pathway is a conserved cellular machinery essential for reverse-topology membrane fission events, such as multivesicular body biogenesis and cytokinesis. Human Immunodeficiency Virus Type 1 (HIV-1) co-opts this machinery to mediate the final scission step of virion budding from the host cell membrane. This review presents our current understanding of how HIV-1 hijacks the ESCRT system, moving beyond a simple recruitment model to a dynamic interplay with host cell regulation. We explore the spatial organization of the budding machin
