Postpartum biomechanical adaptations of the anterior abdominal wall in a rat model: Implications for diastasis rectus abdominis.

BACKGROUND: The linea alba (LA) is a fibrous connective tissue structure that joins the rectus abdominis muscles and contributes to the mechanical integrity of the abdominal wall. Physiological changes during pregnancy can disrupt this structure. As fetal growth increases intra-abdominal pressure, the inter-rectus distance widens due to stretching of the anterior abdominal wall and LA. When this widening becomes substantial, it can result in diastasis rectus abdominis (DRA). The purpose of this study was to characterize postpartum biomechanical changes in the central anterior abdominal wall, w

Treatment of Abdominal Segmental Hernia, Constipation, and Pain Following Herpes Zoster with Paravertebral Block.

Herpes zoster (HZ) most commonly occurs in elderly patients and involves sensory neurons resulting in pain and sensory changes. Clinically significant motor deficits and visceral neuropathies are thought to be relatively rare. A 72-year-old man presented with abdominal segmental hernia, constipation, and pain following HZ in the left T9-10 dermatome. Sixteen days before presentation, he had developed a painful herpetic rash in the left upper abdominal quadrant. Approximately 10 days after the onset of the rash, constipation occurred and was managed with daily oral medication with bisacodyl 5 m

Severe Focal Abdominal Weakness Presenting as an Abdominal Hernia.

A 56-year-old female presented to her primary care physician (PCP) with years of a chronic right-sided abdominal mass. Initial imaging with an abdominal computed tomography (CT) did not reveal a true hernia but did show significant abdominal muscle wasting, causing outpouching of her abdominal wall. General surgery was consulted and noted a lack of response of the abdominal musculature on the right side when asked to brace her abdomen, prompting referral to physiatry to evaluate a potential neuromuscular source of the mass. Physical examination revealed a protuberant abdomen on the right great

Changes in the abdominal wall after anterior, posterior, and combined component separation.

Knowledge of the changes that occur in the abdominal wall after component separation (CS) is essential for understanding the mechanisms of action of the various CS techniques, the changes observed on computed tomography images, and, perhaps most importantly, the anatomic and physiologic changes observed in patients who have undergone CS. Purpose Component separation (CS) techniques are essential adjuncts during most abdominal wall reconstructions. They allow the fulfillment of most modern abdominal wall reconstruction principles, especially primary closure of defects and linea alba restoration

Damage Control Laparotomy and Management of the Open Abdomen.

Management of the open abdomen has been used for decades by general surgeons. Techniques have evolved over those decades to improve control of infection, fluid loss, and improve the ability to close the abdomen to avoid hernia formation. The authors explore the history, indications, and techniques of open abdomen management in multiple settings. The most important considerations in open abdomen management include the reason for leaving the abdomen open, prevention and mitigation of ongoing organ dysfunction, and eventual plans for abdominal closure.

Prospective Cohort Study Quantifying the Effect of the LevaLap 1.0 on the Distance between the Abdominal Wall and Intra-abdominal Viscera.

STUDY OBJECTIVE: More than 13 million laparoscopic procedures are performed globally every year. The LevaLap 1.0 device may facilitate safe abdominal access when using the Veress needle for initial abdominal insufflation during laparoscopic surgery. We undertook this study to test the hypothesis that use of the LevaLap 1.0 would increase the distance from the abdominal wall to underlying viscera and the retroperitoneum, including from major vessels. DESIGN: Prospective cohort study. SETTING: Referral center. PATIENTS: Eighteen patients scheduled to undergo an interventional radiology proced

Respiratory muscle activity in voluntary breathing tracking tasks: Implications for the assessment of respiratory motor control.

How the involuntary (bulbospinal) and voluntary (corticospinal) pathways interact in respiratory muscle control is not established. To determine the role of excitatory corticobulbar pathways in humans, studies typically compare electromyographic activity (EMG) or evoked responses in respiratory muscles during hypercapnic and voluntary tasks. Although ventilation is matched between tasks by having participants track signals of ventilation, these tasks may not result in matched respiratory muscle activity. The aim of this study was to describe respiratory muscle activity and ribcage and abdomina

Prehabilitation of complex ventral hernia patients with Botulinum: a systematic review of the quantifiable effects of Botulinum.

PURPOSE: Complex ventral hernia repair (CVHR) encompasses patient optimization, primary fascial closure (PFC), mesh reinforcement and component separation technique (CST), if needed. High rates of complications after CST are still reported. Prehabilitation by managing pre-operative modifiable risk factors, like abdominal wall compliance, possibly reduces these rates. Compliance can be modified by intramuscular injection of Botulinum in the lateral abdominal wall muscles (LAWM). Paralysis leads to elongation of these muscles, which may facilitate PFC and/or prevent CST. Evidence to use Botulinu

Evaluating the influence of trunk intra-muscular and intra-abdominal pressure on spinal geometric compensation.

Biomechanical modelling studies have revealed the impact of passive mechanical properties of spinal soft tissues on spinal configuration. This study extends prior work by evaluating the involvement of trunk abdominal and intramuscular pressure (IMP), on spinal geometric compensation, using a validated finite element spine model. The model included the vertebrae, rib cage, IVD, pelvis, ligaments, abdominal cavity and abdominal and spinal muscles. Over a fixed pelvis, the model underwent a 60° forward flexion. Muscles and the abdominal cavity were modelled as fluid-filled solid entities containi

Volume change in the rectus abdominis muscle after deep inferior epigastric perforator flap harvest.

Deep inferior epigastric perforator (DIEP) flap is associated with less donor site morbidity than transverse rectus abdominis myocutaneous flap. However, abdominal muscle atrophy and donor site complications caused by denervation during pedicle dissection cannot be avoided. This retrospective study investigated the change in the rectus abdominis muscle volume after DIEP flap harvest. Of 395 patients who underwent unilateral DIEP flap breast reconstruction between August 2007 and July 2017, 25 patients with >2-year postoperative abdominal computed tomography data were evaluated. Preoperative an

Abdominal Wall Pain and Primary Dysmenorrhea: The Importance of Myofascial Trigger Points.

Myofascial trigger points are among the often-overlooked causes of anterior abdominal wall pain and primer dysmenorrhea. It is crucial to evaluate patients with a myofascial perspective in addition to a good history and a detailed physical examination. Myofascial trigger points of the abdominal oblique muscles and rectus abdominis muscle should be considered for patients with abdominal wall pain and primer dysmenorrhea. It should also be kept in mind that myofascial pain syndrome may be the primary pathology responsible for the pain, or it may be an entity accompanying another underlying patho

Activity of respiratory-related oropharyngeal and laryngeal motoneurones during fictive vomiting in the decerebrate cat.

Activities of respiratory laryngeal and oropharyngeal respiratory nerves were studied during fictive vomiting elicited by supradiaphragmatic vagus nerve stimulation in the decerebrate cat. Inspiratory laryngeal nerves were strongly inhibited throughout the retching and expulsion phase. Glossopharyngeal, hypoglossal and expiratory laryngeal nerves were coactivated with the phrenic and abdominal nerve bursts. The pharyngeal branch of the vagus nerve discharged during the phrenic and abdominal inter-burst of the retching phase, and was silent during the abdominal expulsion. These activities permi

[The effect of massage on the neuromuscular apparatus and blood coagulating system of patients with chronic salpingo-oophoritis].

74 patients with chronic salpingo-oophoritis (CS) in remission were assessed as to effects of classic massage of the uterine and epoöphoronic reflexogenic zones on clinical pattern of the disease, bioelectric activity of the abdominal muscles, spine and pelvis, blood coagulation. The combined massage produced a more potent effect on electric activity of the above muscles and blood coagulation than massage on a single area. It is better to start massage from the abdomen and continue in the lumbosacral area.
