[Velopharyngeal insufficiency in children].

Velopharyngeal insufficiency (VPI) represents an incomplete closure between the soft palate and the posterior pharyngeal wall. Its etiology can be anatomical (cleft palate), neurologic, or iatrogenic (after adenoidectomy). The evaluation of a VPI begins with a through speech and language assessment and can be complemented by instrumental investigations. VPI treatment relies on its early identification, followed by a specific speech therapy management. Surgery is performed in case of no improvement with speech therapy or in case of an anatomical defect not allowing the child to improve. IVP man…

Mycotic flora in tonsils and adenoids. A microbiological and histological evaluation.

The tonsils of 147 patients with chronic tonsillitis and the adenoids of 134 children with recurrent respiratory infections were cultured for fungal organisms and studied microscopically. A control group consisted of 68 healthy persons. Positive cultures were obtained in 64% of patients with chronic tonsillitis, in 21% of children treated with adenoidectomy, and in 69% of the control group. Candida albicans, Saccharomyces sp. and Aspergillus were the most common organisms. The histological investigations revealed no evidence of pathogenecity in these organisms. They were found in the tonsillar…

Surgical Management of Pediatric Obstructive Sleep Apnea Beyond Adenotonsillectomy: The Nose, Nasopharynx, and Palate.

While adenotonsillectomy is the primary treatment of pediatric obstructive sleep apnea (OSA), persistent OSA after surgery is common and may … ome of the surgical management options include submucous inferior turbinate resection, septoplasty, adenoidectomy, and expansion sphincter pharyngoplasty.

[Chronic tonsillar hypertrophy as a cause of cor pulmonale, pulmonary oedema, and hypersomnia in children (author's transl)].

Chronic hypertrophied tonsils and adenoids causing partial airway obstruction produced serious illness and a confusing clinical picture in a 51/2-year-old boy. Cardinal signs were cor pulmonale, pulmonary oedema and marked cardio-respiratory changes due to hypoxaemia and hypercapnia, in addition to hypersomnia. Marked improvement of the clinical picture and the abnormal signs followed directly upon tonsillectomy and adenoidectomy. Chronic alveolar hypoventilation is presumably the main cause of the described condition and of others of extracardiac origin with similar signs and symptoms.

Coblation Versus Microdebrider in Pediatric Intracapsular Tonsillotomy: A Comparison of Efficiency, Bleeding, and Revision Surgery.

OBJECTIVE: Intracapsular tonsillectomy or "tonsillotomy" is an increasingly popular alternative to extracapsular tonsillectomy due to decreased bleeding risk and pain. Coblation and microdebrider are most frequently used, but studies comparing them are limited. We aimed to compare efficiency, postoperative bleeding, and need for revision surgery between these techniques. STUDY DESIGN: Retrospective case series. SETTING: Quaternary children's hospital between 2018 and 2024. METHODS: This study included pediatric patients undergoing tonsillotomy with adenoidectomy using the microdebrider (13 …

Comparative Safety of Flexible Laryngeal Mask and Tracheal Intubation in Pediatric Obstructive Sleep Apnea Surgery Across Different Age Groups.

OBJECTIVE: The aim of this study is to evaluate the safety of using a flexible laryngeal mask versus tracheal intubation in pediatric patients undergoing tonsillectomy and adenoidectomy across different age groups. METHODS: A total of 180 children, divided equally into 3 age groups (1-3 y, 4-6 y, and 7-12 y), undergoing tonsillectomy and adenoidectomy were enrolled. Each age group was further subdivided into 2 cohorts: the flexible laryngeal mask group and the tracheal intubation group, each comprising 30 children. Parameters such as operation duration, anesthesia recovery time, and postopera…

OBJECTIVE: To characterize postoperative airway obstruction and evaluate management strategies in pediatric patients with cleft palate follo … uction (68.0%): revision palatoplasty/flap revision (8/25, 35%), tonsillectomy and partial cephalic adenoidectomy (10/25, 40%), and partial cephalic adenoidectomy only (5/25, 20%). In total, 11 were medically managed and 3 were observed without intervention. In nine patients with paired PSGs, there was no difference in pre-TAP and post-TAP AHI or OAHI. Patients …

Subanesthetic-dose propofol infusion for preventing emergence agitation in children: a retrospective observational study.

PURPOSE: Anesthesia maintenance using propofol and a propofol bolus dose at the end of surgery have been shown to prevent emergence agitation (EA). However, the preventive effect of subanesthetic propofol infusion during sevoflurane anesthesia on EA remains unknown. We aimed to evaluate the effect of subanesthetic propofol infusion on EA in children. METHODS: We retrospectively compared the incidences of severe EA requiring pharmacological intervention in children who underwent adenoidectomy, tonsillectomy with or without adenoidectomy, or strabismus surgery between maintenance with sevoflura…

Otopathogen interactions in the nasopharynx of children, and the predictive value of nasopharyngeal aspirate culture for the aetiology of upper respiratory infections.

AIM: To evaluate nasopharyngeal aspirate cultures for screening otopathogen carriage in the adenoid in children 2-7 years of age. METHODS: Thirty-seven children, 2-7 years of age, scheduled for adenoidectomy were enrolled into this prospective study at Rockhampton, Australia. Adenoid biopsy and nasopharyngeal aspirate bacteriology were assessed by conventional culture. Demographic and environmental data were collected by questionnaire. Statistical analyses for descriptive, comparison and logistic regression tests between microbial, demographic, environmental and clinical groups were applied. …

[Treatment of pediatric obstructive sleep apnea hypopnea syndrome].

OBJECTIVE: To evaluate treatment options of pediatric obstructive sleep apnea hypopnea syndrome (OSAHS) and their curative effect. METHODS: In this study, there are 59 confirmed pediatric obstructive sleep apnea hypopnea syndrome cases diagnosed by polysomnography (PSG). Among them, 54 cases were treated by tonsillectomy or/and adenoidectomy directed by transoral endoscopy. Continue positive airway pressure (CPAP) was used for 2 cases in long-term. Three cases were treated with other conservative therapy. OSA-18 (quality of life for children with obstructive sleep apnea 18 items) was adopted …

Naso- and oropharyngeal dimensions in children with obstructive sleep apnea.

Sixty children (3-11 years) were evaluated to determine variations in naso- and oropharyngeal dimensions associated with tonsil and adenoid … r in obstructed patients with adenotonsillar hypertrophy (P less than 0.003). In patients requiring adenoidectomy, the nasopharyngeal volume prior to adenoidectomy was significantly smaller in patients with obstructive symptoms (P less than 0.001). Postadenoidectomy, no significant difference was found in the nasopharynx volume amongst all subjects. These data …

Processing of adenoid and tonsil specimens in children: a national survey of standard practices and a five-year review of the experience at the Children's Hospital of Pittsburgh.

The best means of pathologically examining routine tonsillectomy and adenoidectomy specimens in children remains controversial. Otolaryngologists fear missing an unsuspected diagnosis. However, the cost-effectiveness of microscopic analysis, given the rare incidence of unsuspected diagnosis, is questionable. If a significant pathologic diagnosis is missed, the medicolegal implications could be significant. A questionnaire was sent to 111 members of the American Society of Pediatric Otolaryngology. Additionally, we reviewed our experience at the Children's Hospital of Pittsburgh for the 5-year …
