Histaminoid responses to atracurium, vecuronium and tubocurarine.

Sixty patients scheduled for elective surgery underwent intradermal testing with 0.1 ml of the following solutions diluted in 0.9% saline: vecuronium and tubocurarine (1 in 1,000), atracurium (1 in 1,000 and 1 in 10,000), thiopentone (1 in 100) and also a 0.9% saline control. Thirty minutes later, an area of erythema of greater than 1.5 cm, or a wheal exceeding 1.0 cm in diameter, was recorded as a positive reaction. The patients then randomly received equipotent doses of atracurium, vecuronium or tubocurarine during a standardized anaesthetic induction. Any cutaneous reaction and the percenta

INDIVIDUAL ARTICLE: Ammonium Lactate in Clinical Practice: Optimizing Treatment of Xerotic and Hyperkeratotic Skin Conditions.

Ammonium lactate is a well-established, time-tested dermatologic therapy with a substantial body of clinical evidence supporting its use in xerotic and hyperkeratotic skin conditions. Although its use in routine practice has evolved over time, interest in ammonium lactate has been renewed by advances in the understanding of lactate biology and its role in skin physiology. Xerosis is characterized by reduced water content, impaired corneocyte turnover, and structural disruption of the stratum corneum, leading to scaling, roughness, and discomfort. As a pH-balanced source of L-lactic acid, ammon

Konjac Glucomannan as a Gel-Forming Biopolymer for Transdermal Drug Delivery: Properties, Formulation Strategies, and Translational Challenges.

Konjac glucomannan is a natural polysaccharide increasingly relevant as a matrix forming material for skin applied delivery systems. Its potential is linked to its glucose and mannose backbone, abundant hydroxyl groups, partial acetyl substitution, high molecular weight, and capacity for chain association. These features regulate hydration, swelling, rheology, viscoelasticity, film formation, gelation, and drug release. This narrative review examines konjac glucomannan from a structure based formulation perspective, covering physicochemical attributes, modification strategies, formulation plat

A mechanism-inspired integrated dual-frequency ultrasound transducer for cavitation-enhanced transdermal drug delivery.

Ultrasound has gained widespread application in the field of transdermal drug delivery (TDD), including dermatological therapy and aesthetic medicine, owing to its non-invasive nature and cost-effectiveness. Despite its widespread use, clinical researchers have largely relied on empirical selection of low-frequency ultrasound (tens to hundreds of kHz) for single-frequency sonophoresis, and a mechanistic understanding of how ultrasound frequency regulates cavitation-from nucleation to bubble collapse-remains lacking, which limits the rational design of ultrasound-based TDD systems. In this stud

Enhanced Transdermal Delivery of Liraglutide for Sustained Obesity Management.

Liraglutide, a glucagon-like peptide-1 (GLP-1) receptor agonist, is recognized for its effectiveness in glycemic control and weight reduction. However, the pain associated with the traditional subcutaneous administration of liraglutide and needle phobia in some patients will seriously affect medication adherence. Transdermal delivery represents a promising alternative to subcutaneous injection for its convenience, noninvasiveness, and painlessness, and it can bypass the first-pass metabolism and multiple biological barriers of oral administration and be delivered in a controlled manner, result

Efficacy and Safety of Topical Dapsone in Dermatology: A Scoping Review of Clinical Studies.

BACKGROUND: Dapsone (4,4'-Diamino diphenyl sulfone) has been utilized in managing a wide variety of mucocutaneous conditions, usually as a systemic product. Topical dapsone is commercially available as 5% and 7.5% gel and is FDA-approved for acne vulgaris. These topical formulations have shown less systemic absorption and fewer adverse events. Considering this, topical dapsone might benefit localized disease, immunosuppressed individuals, and those who avoid exposure to systemic therapies. METHODS: Following PRISMA guidelines, a comprehensive literature search was conducted across PubMed, Web

INDIVIDUAL ARTCLE: Pathophysiologic Targets of Acne Treatment.

Acne vulgaris is an extremely common dermatologic condition. Individuals with acne present not only to dermatologists, but also to internists, family medicine physicians, pediatricians, estheticians, and beauty counters alike in search of a treatment. The diagnosis of acne is relatively straightforward, leading many to believe that acne is a simple condition. However, the pathophysiology of acne is anything but simple. Decades of research has ultimately revealed a complex interaction of pathogenic factors that lead to acne. This includes sebum production, C. acnes colonization, inflammation, a

Transdermal Hydrogen Sulfide Delivery Enabled by Open-Metal-Site Metal-Organic Frameworks.

Hydrogen sulfide (H2S) is an endogenously produced gasotransmitter involved in many physiological processes that are integral to proper cellular functioning. Due to its profound anti-inflammatory and antioxidant properties, H2S plays important roles in preventing inflammatory skin disorders and improving wound healing. Transdermal H2S delivery is a therapeutically viable option for the management of such disorders. However, current small-molecule H2S donors are not optimally suited for transdermal delivery and typically generate electrophilic byproducts that may lead to undesired toxicity. Her

Polymeric microneedles for the eye: An overview of advances and ocular applications for minimally invasive drug delivery.

Ocular drug delivery is challenging due to the presence of tissue barriers and clearance mechanisms. Most widely used topical formulations need frequent application because of poor permeability, short retention, and low bioavailability. Invasive intraocular injections and implants that deliver drugs at the target site are associated with infections, inflammation, and even vision loss post-use. These gaps can be addressed by a delivery platform that can efficiently deliver drug with minimal tissue damage. Microneedles were introduced as a delivery platform for overcoming dermal barriers with mi

Recent Updates on Transdermal Drug Delivery Approaches for the Management of Gout and its Clinical Perspective.

Oral and injectable drug administration have recently been replaced with transdermal drug delivery (TDD) approaches, which are less intrusive, less likely to be rejected by patients, and easier to administer. There is still room for improvement in the treatment of gout with the use of a TDD system. Gout has become a worldwide epidemic and a severe threat to human beings. Gout treatment can be accomplished in various ways, including orally and intravenously. Several traditional options are still useless, cumbersome, and potentially dangerous. Hence, gout therapeutic options are desperately requ

Enhanced skin penetration of Finasteride loaded DMSO-liposomes for the treatment of androgenic alopecia: comparison with conventional liposomes.

Long-term treatment with finasteride (FIN) for androgenic alopecia is restricted due to its systemic side effects. To address this problem, DMSO-modified liposomes were prepared in the present study to improve the topical delivery of FIN. DMSO-liposomes were prepared by a modification of the ethanol injection method. It was hypothesized that the permeation-enhancing property of DMSO could promote drug delivery to deeper skin layer where hair follicles are present. Liposomes were optimized by quality by design (QbD) approach and biologically evaluated in a rat model of testosterone-induced alop
