Metronidazole is an effective method of analgesia following haemorrhoidectomy: a systematic review and meta-analysis.

BACKGROUND: Haemorrhoids are one of the most frequently encountered benign anorectal conditions that negatively impact patients' quality of life. Excisional haemorrhoidectomy (closed or open) is a surgical procedure reserved for the treatment of third- and fourth-degree haemorrhoids, with considerable post-procedure pain reported. The aim of this study was to clarify the association between post operative metronidazole use (both oral and topical) and post-haemorrhoidectomy pain scores through systematic review and meta-analysis of randomised controlled trials (RCTs). METHODS: This study was g

Topical Clascoterone: The First Novel Agent for Acne Vulgaris in 40 Years.

PURPOSE: Acne vulgaris is a ubiquitous condition in men and women starting in adolescence. It is often persistent and refractory to multiple treatment methods. Although multiple medications may be used on- or off-label for treatment, many adverse effects and risks exist with these treatments, and there has not been an agent with a novel mechanism of action introduced in 40 years. Clascoterone is a recently approved topical acne medication with the first novel mechanism of action since isotretinoin. The purpose of this article was to review the clinical data regarding the safety and efficacy of

Topical Diclofenac Patches Versus Oral Diclofenac for Postoperative Dental Pain: A Systematic Review.

Postoperative pain is common following dental procedures, and the route of administration may influence analgesic efficacy and tolerability. This review compared topical diclofenac patches with oral diclofenac tablets for postoperative dental pain. The patches evaluated in the included studies are local topical therapies that release diclofenac into and near the skin at the application site; they should not be interpreted as systemic transdermal delivery systems. Indexed databases, including PubMed, EMBASE, Ovid, Scopus, ISI Web of Knowledge, and the Cochrane Library, were searched through Aug

Ablative fractional laser-assisted treatments for keratinocyte carcinomas and its precursors-Clinical review and future perspectives.

Keratinocyte carcinomas (KC) are the most common malignant human neoplasms. Although surgery and destructive approaches are first-line treatments, topical therapies are commonly used. Due to limited uptake of topical agents across the skin barrier, clearance rates are often sub-optimal. In pre-clinical investigations, ablative fractional laser (AFL)-assisted drug delivery has demonstrated improved uptake of topical drugs commonly used to treat KC. In 22 clinical trials, the effect of AFL-assisted treatments has been investigated for actinic keratosis (AK; n = 14), Bowen's disease (BD; n = 5),

Carbonic anhydrase inhibitors: topically acting antiglaucoma sulfonamides incorporating phthaloyl and phthalimido moieties.

Sulfonamides incorporating 2-carboxy-benzenecarboxamido (phthaloyl) moieties in their molecules were prepared by reaction of phthalic anhydride with aromatic/heterocyclic sulfonamides in mild conditions. Another closely related series of derivatives was prepared by reaction of the same reagents in more energetic conditions, when the corresponding phthalimides were obtained. Some of these compounds showed very good in vitro carbonic anhydrase (CA) isozymes I, II and IV inhibitory properties, with affinities for the enzymes in the low nanomolar range for the best inhibitors. Some of the potent C

Cyclodextrin-based formulation of carbonic anhydrase inhibitors for ocular delivery - A review.

Carbonic anhydrase inhibitors (CAIs) are used as systemic and topical agents for lowering intraocular pressure (IOP) in patients with glaucoma. Owing to the wide distribution of CAs and their physiological functions in various tissues, systemic administration of CAIs may lead to unwanted side effects. Thus, exploration of drugs targeting the specific CA isoenzyme in ocular tissues and application of the same as topical eye drops would be desirable. However, the anatomical and physiological barriers of the eyes can limit drug availability at the site. The very low aqueous solubility of CAI agen

Efinaconazole and Tavaborole.

PURPOSE: The purpose of this article is to review the safety, efficacy, and role of efinaconazole and tavaborole in the treatment of onychomycosis. SUMMARY: Onychomycosis is a fungal infection of the nail caused by dermatophytes, yeasts, and nondermatophyte fungi. Distal and lateral subungual onychomycosis (DLSO) accounts for the majority of the cases. These infections cause structural damage to the nail which makes treatment difficult. Both oral and topical agents exist for the treatment of onychomycosis. Oral medications have generally been more effective, yet adverse effects and drug inter

Successful treatment of cutaneous lesions of dermatomyositis with topical pimecrolimus.

Dermatomyositis (DM) is an idiopathic inflammatory process characterized by proximal muscle weakness and cutaneous lesions, such as the Gottron's sign, heliotrope rash, and erythematous photosensitive rash. Administration of systemic agents for the treatment of underlying systemic diseases leads to remission of the cutaneous lesions in many cases. However, cutaneous lesions may remain refractory to treatment. Pimecrolimus is a calcineurin inhibitor with combined anti-inflammatory and immunomodulatory activity. It has high affinity to the skin and low permeation potential, even in patients with

Infectious vaginitis. Selecting therapy and preventing recurrence.

Vaginal candidiasis, trichomoniasis, and bacterial vaginosis can be difficult to manage because of predisposing factors and frequent recurrence. Thus, in addition to drug therapy, management should include attempts to minimize factors that contribute to recurrence. Both topical and oral agents are available for vaginal candidiasis and bacterial vaginosis, and both routes of administration appear equally safe and effective. Topical therapy may be used for these conditions in pregnant women. Only oral therapy is optimal for trichomoniasis. However, during pregnancy, the infection may be managed

Noninvasive ocular drug delivery: potential transcorneal and other alternative delivery routes for therapeutic molecules in glaucoma.

Drug delivery to the eye is made difficult by multiple barriers (such as the tear film, cornea, and vitreous) between the surface of the eye and the treatment site. These barriers are difficult to surmount for the purposes of drug delivery without causing toxicity. Using nanotechnology tools to control, manipulate, and study delivery systems, new approaches to delivering drugs, genes, and antigens that are effective and safe can be developed. Topical administration to the ocular surface would be the safest method for delivery, as it is noninvasive and painless compared with other delivery meth

Evaluation of Topical Reconstituted HDL as a Treatment for Diabetic Wounds in Murine and Porcine Models.

Chronic, non-healing foot ulcers are the most common complication of diabetes and are associated with high morbidity, mortality, and substantial health-care costs. Impaired wound healing in diabetes results from a complex pathophysiology involving persistent inflammation, interrupted re-epithelialization, impaired neovascularization, and defective granulation tissue formation. Reconstituted high-density lipoprotein (rHDL), with its antioxidant and anti-inflammatory properties, may counteract these processes and thereby promote wound repair. We investigated the effect of topical administration

Topical CCL3 Is Well-Tolerated and Improves Liver Function in Diabetic Mice: Evidence from a 14-Day Toxicity Study.

Diabetic wounds exhibit impaired immune function, delayed neutrophils recruitment, and heightened infection risk which compromises early infection control and delays healing. We have demonstrated that topical CCL3 treatment restores neutrophil influx, reduces bacterial infection by ~99%, and accelerates wound healing in diabetic mice. As per Food and Drug Administration (FDA) Guidelines for Investigational New Drug (IND), we conducted a 14-day acute toxicity study in diabetic mice following a single topical administration of CCL3 at effective low dose (1 µg) and high dose (10 µg) per wound. Randomized clinical trial of topical tranexamic acid after reduction mammoplasty.

BACKGROUND: The antifibrinolytic drug tranexamic acid is currently being rediscovered for both trauma and major surgery. Intravenous administration reduces the need for blood transfusion and blood loss by about one-third, but routine administration in surgery is not yet advocated owing to concerns regarding thromboembolic events. The aim of this study was to investigate whether topical application of tranexamic acid to a wound surface reduces postoperative bleeding. METHODS: This was a randomized double-blind placebo-controlled trial on 30 consecutive women undergoing bilateral reduction mamm
